Book excerpt: Traumatic brain injury (TBI) is traditionally viewed as an anatomic and neuropathological condition. Caring for TBI patients is a matter of defining the extent of an anatomical lesion, managing this lesion, and minimizing secondary brain injury. On the research side, the effects of TBI often are studied in the context of neuronal and axonal degeneration and the subsequent deposition of abnormal proteins such as tau. These approaches form the basis of our current understanding of TBI, but they pay less attention to the function of the affected organ, the brain. Much can be learned about TBI by studying this disorder on a systems neuroscience level and correlating changes in neural circuitry with neurological and cognitive function. There are several aspects of TBI that are a natural fit for this perspective, including post-traumatic epilepsy, consciousness, and cognitive sequelae. How individual neurons contribute to network activity and how network function responds to injury are key concepts in examining these areas. In recent years, the available tools for studying the role of neuronal assemblies in TBI have become increasingly sophisticated, ranging from optogenetic and electrophysiological techniques to advanced imaging modalities such as functional magnetic resonance imaging and magnetoencephalography. Further progress in understanding the disruption and subsequent reshaping of networks is likely to have substantial benefits in the treatment of patients with TBI-associated deficits. In this Frontiers Topic, we intend to highlight the systems neuroscience approach to studying TBI. In addition to analyzing the clinical sequelae of TBI in this context, this series of articles explores the pathophysiological mechanisms underlying network dysfunction, including alterations in synaptic activity, changes in neural oscillation patterns, and disruptions in functional connectivity. We also include articles on treatment options for TBI patients that modulate network function. It is our hope that this Frontiers Topic will increase the clinical and scientific communities’ awareness of this viable framework for deepening our knowledge of TBI and improving patient outcomes.

Book excerpt: Status Epilepticus (SE) is a neurological emergency and has high morbidity and mortality. The International League Against Epilepsy (ILAE) recently updated their definition to specify that, “SE is a condition resulting either from the failure of the mechanisms responsible for seizure termination or from the initiation of mechanisms, which lead to abnormally, prolonged seizures.” Such phenomena can lead to long-term neurological complications due to neuronal death, glia, neurological injury, aberrant neuroplasticity, oxidative stress and inflammation, and alteration of neuronal networks. Depending upon the type and duration of SE, these mechanisms are quite variable. Therefore, in response to the updated definition of SE, novel avenues of research are required to address the specified involvement of the underlying mechanisms and pathophysiology resulting in the development of and outcomes from SE. Improving the basic science understanding of SE will facilitate essential clinical trials. One can envision such experiments to include device and compound-based technological interventions directed at aborting the seizure activity and improving clinical outcomes. Benzodiazepines remain one of the cornerstones of treatment, and studies are underway to study new delivery options, including intranasal, buccal, and intramuscular midazolam, in addition to rectal diazepam, with the goal of aborting the seizure activity outside the hospitals, as rapidly as possible. Approved and off-label anticonvulsants, such as phenytoin, phenobarbital, valproate, topiramate, levetiracetam, lacosamide, steroids, immunosuppressants, and neuroprotective compounds, have also shown some efficacy at treating SE. However, substantial challenges remain in optimally managing SE and minimizing the short- and long-term complications. Such difficulties can be overcome by innovative approaches targeting the underlying mechanisms of neuronal excitability, glia, neuronal death, neuroplasticity, oxidative stress, inflammation, and neuroinflammation.

The role of calcium as first messenger is mediated by the G-protein coupled receptor, the extracellular calcium-sensing receptor (CaSR). The CaSR is a multifaceted molecule that senses changes in the concentration of a wide variety of environmental factors including di- and trivalent cations, amino acids, polyamines, and pH. In calcitropic tissues with obvious roles in calcium homeostasis such as parathyroid, kidney, and bone it regulates circulating calcium concentrations. The germline mutations of the CaSR cause parathyroid disorders demonstrating the importance of the CaSR for the maintenance of serum calcium homeostasis. The CaSR has an important role also in a range of non-calcitropic tissues, such as the intestine, lungs, central and peripheral nervous system, breast, skin and reproductive system, where it regulates molecular and cellular processes such as gene expression, proliferation, differentiation and apoptosis; as well as regulating hormone secretion and lactation. This Research Topic is an overview of the CaSR and its molecular signaling properties together with the various organ systems where it plays an important role. The articles highlight the current knowledge regarding many aspects of the calcitropic and non-calcitropic physiology and pathophysiology of the CaSR.
