Book Comprehensive Geriatric Assessment

Download or read book Comprehensive Geriatric Assessment written by Alberto Pilotto and published by Springer. This book was released on 2017-12-29 with total page 182 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book offers an up-to-date review on the principles and practice of multidimensional assessment and management of the older individual, which represents the cornerstone of modern clinical practice in the elderly. The early chapters cover the main elements and scope of the comprehensive geriatric approach and explain the pathways of care from screening and case finding through to in-depth assessment and treatment planning. Subsequent chapters review the evidence of how best to apply the multidimensional assessment and management approach in defined healthcare settings and within specific clinical areas, such as cancer and surgery. Finally, the education and training challenges are reviewed and the prospects for future clinical service and research in this important field are examined. The book is very timely given the recent advances in application of this approach, which reflect the growing international realization that older people are “core business” in many clinical areas where the role of specialist geriatric medicine has hitherto been limited. Accordingly, the book will be relevant to a wide range of clinicians. The authorship comprises many of the best known and widely published experts in their respective fields.

Available in PDF, EPUB and Kindle. Book excerpt: The interRAI ChYMH-DD is intended to be used with children and youth with developmental disabilities in mental health settings to support comprehensive care planning, outcome measurement, quality indicators, and case mix classification to estimate relative resource intensity. It employs specific observation periods in order to provide reliable and valid measures of clinical characteristics that reflect the child's or youth's strengths, preferences, and needs. In keeping with other interRAI instruments, the basic time frame for assessment was set at 3 days unless otherwise indicated. Triggers for numerous Collaborative Action Plans to support care planning decisions are also embedded in the instrument. There are two versions of the ChYMH-DD assessment form. Typically, the In-patient form would be used for a child or youth who currently resides in a residential facility or psychiatric facility/unit, and the Community-Based form for a child or youth who resides in a community setting.

Book Concurrent Treatment of PTSD and Substance Use Disorders Using Prolonged Exposure COPE

Download or read book Concurrent Treatment of PTSD and Substance Use Disorders Using Prolonged Exposure COPE written by Sudie E. Back and published by Oxford University Press, USA. This book was released on 2014-10-08 with total page 193 pages. Available in PDF, EPUB and Kindle. Book excerpt: Concurrent Treatment of PTSD and Substance Use Disorders Using Prolonged Exposure (COPE) is a cognitive-behavioral psychotherapy program designed for patients who have posttraumatic stress disorder (PTSD) and a co-occurring alcohol or drug use disorder. COPE represents an integration of two evidence-based treatments: Prolonged Exposure (PE) therapy for PTSD and Relapse Prevention for substance use disorders. COPE is an integrated treatment, meaning that both the PTSD and substance use disorder are addressed concurrently in therapy by the same clinician, and patients can experience substantial reductions in both PTSD symptoms and substance use severity. Patients use the COPE Patient Workbook while their clinician uses the Therapist Guide to deliver treatment. The program is comprised of 12 individual, 60 to 90 minute therapy sessions. The program includes several components: information about how PTSD symptoms and substance use interact with one another; information about the most common reactions to trauma; techniques to help the patient manage cravings and thoughts about using alcohol or drugs; coping skills to help the patient prevent relapse to substances; a breathing retraining relaxation exercise; and in vivo (real life) and imaginal exposures to target the patient's PTSD symptoms.

Book Options for Aged Care in China

Download or read book Options for Aged Care in China written by Elena Glinskaya and published by World Bank Publications. This book was released on 2018-11-28 with total page 349 pages. Available in PDF, EPUB and Kindle. Book excerpt: China is aging at an unprecedented rate. Improvements in life expectancy and the consequences of the decades-old family planning policy have led to a rapid increase in the elderly population. According to the United Nations World Population Prospects, the proportion of older people age 65 and over will increase by about one-fourth by 2030, and the elderly will account for about one quarter of the total population by 2050. Population aging will not only pose challenges for elder care but also have an impact on the economy and all aspects of society (World Bank, 2016a). The government is aware of the need to develop an efficient and sustainable approach to aged care. To this end, the General Office of the State Council issued the 12th Five-Year Plan for the Development of Aged Care Services in China and the Development Plan for a System of Social Services for the Aged (2011-2015). It is now in the process of formulating the 13th Five-Year National Plan on Aging, which will further elaborate and finalize the reform roadmap for 2016 to 2020. The Plan is expected to be finalized and launched by June 2016. The National Development and Reform Commission (NDRC) helped draft these plans and is now leading the development of policy measures for the provision of social services for the elderly. This volume has been prepared to support the translation of the broad ideas on aged care provision expressed in the 12th and 13th Five-Year Plans and other government plans into reality and to help the government tackle the challenges described above. It strives to identify a policy framework that fits the Chinese context and can be put in place gradually. Specifically, it aims to provide an up-to-date understanding of the evolving aged care landscape in China; review international experiences in long-term care provision, financing, and quality assurance and assess their relevance to China’s current situation; discuss implications of current developments and trends for the future of aged care in China; and propose policy options based on available evidence and best practices.

Book InterRAI Clinical Assessment Protocols CAPs for Use with Community and Long Term Care Assessment Instruments Hong Kong Chinese Edition

Download or read book InterRAI Clinical Assessment Protocols CAPs for Use with Community and Long Term Care Assessment Instruments Hong Kong Chinese Edition written by John N. Morris and published by . This book was released on 2016-05 with total page 192 pages. Available in PDF, EPUB and Kindle. Book excerpt: This Hong Kong Chinese edition is translated from the English "interRAI Clinical Assessment Protocols (CAPs) for Use with Community and Long-Term Care Assessment Instruments." CAPs are designed to help the assessor interpret systematically all the information recorded with the instruments' assessment forms. CAPs are not intended to automate care planning; rather, they help the clinician focus on key issues identified during the assessment process, so that decisions as to whether and how to intervene can be explored with the individual. This book contains the Clinical Assessment Protocols (CAPs) for use with the following interRAI assessment instruments: Home Care (HC), Long-Term Care Facilities (LTCF), Community Health (CHA), and the Assisted Living Supplement to Community Health (CHA-AL). The CAPs in this book can also be used with the prior generation of the interRAI assessment instruments used in long-term care facilities (the MDS 2.0) and home care (the HC 2.0). This book includes introductory material followed by the CAPs themselves, which are organized into the four broad areas of Functional Performance, Cognition/Mental Health, Social Life, and Clinical Issues. The Hong Kong Chinese Edition has been customized for Hong Kong.
