Book excerpt: The papers compiled in this supplementum are a selection of the best con th tributions presented at the 19 Central-European Neurological Symposium (CNS 19) held on June 29 - July 1, 1989 in Vienna. The main topic of this conference was degenerative and age-associated neurological diseases. In recent decades life expectancy has dramatically increased, at least in the industrialized countries. This has led to extreme distortions of the so-called population pyramids that no longer look like such but begin to almost resemble cylinders. As a consequence of this "overaging" of the population, diseases that are associated with age have become much more common than before. It was thus more than reasonable to devote a congress of the CNS series to these important neurological diseases. The following fields of interest are covered: Age-associated memory impairment (AAMI), Alzheimer's and other dementias, Parkinson's disease and other movement disorders, stroke and others. Concerning the de men tias, some papers deal with diagnosis employing neuro-imaging methods such as MRI, CT, PET and SPECT, others using electrophysiological methods. An important aspect in the early preclinical diagnosis of dementia is the inclusion of neuropsychological tests to enhance the chance of effec tive early treatment. Also drugs that are now under clinical investigation are discussed and preliminary results are presented.

Thus, the evidence base on how to prevent or delay these conditions has been limited at best, despite the many claims of success made in popular media and advertising. Today, however, a growing body of prevention research is emerging. Preventing Cognitive Decline and Dementia: A Way Forward assesses the current state of knowledge on interventions to prevent cognitive decline and dementia, and informs future research in this area. This report provides recommendations of appropriate content for inclusion in public health messages from the National Institute on Aging.

Enriching the body of scientific knowledge around brain health and cognitive ability has the potential to improve quality of life and longevity for many millions of Americans and their families. The Centers for Disease Control and Prevention estimate that as many as 5 million Americans were living with Alzheimer's disease in 2014. That same year, more than 800,000 children were treated for concussion or traumatic brain injuries in U.S. emergency departments. Each year, more than 795,000 people in the United States have a stroke. Developing more effective treatment strategies for brain injuries and illnesses is essential, but brain health is not focused exclusively on disease, disorders, and vulnerability. It is equally important to better understand the ways our brains grow, learn, adapt, and heal. Addressing all of these domains to optimize brain health will require consideration about how to define brain health and resilience and about how to identify key elements to measure those concepts. Understanding the interactions between the brain, the body, and socioenvironmental forces is also fundamental to improving brain health. To explore issues related to brain health throughout the life span, from birth through old age, a public workshop titled Brain Health Across the Life Span was convened on September 24 and 25, 2019, by the Board on Population Health and Public Health Practice in the Health and Medicine Division of the National Academies. This publication summarizes the presentation and discussion of the workshop.

Are many elderly people whom we regard as normal actually in the early stages of AD? Alzheimer's disease does not develop overnight; the early phases may last for years or even decades. Recently, clinical investigators have identified a transitional condition between normal aging and and very early Alzheimer's disease that they have called mild cognitive impairment, or MCI. This term typically refers to memory impairment beyond what one would expect in individuals of a given age whose other abilities to function in daily life are well preserved. Persons who meet the criteria for mild cognitive impairment have an increased risk of progressing to Alzheimer's disease in the near future. Though many questions about this condition and its underlying neuropathology remain open, full clinical trials are currently underway worldwide aimed at preventing the progression from MCI to Alzheimer's disease. This book addresses the spectrum of issues involved in mild cognitive impairment, and includes chapters on clinical studies, neuropsychology, neuroimaging, neuropathology, biological markers, diagnostic approaches, and treatment. It is intended for clinicians, researchers, and students interested in aging and cognition, among them neurologists, psychiatrists, geriatricians, clinical psychologists, and neuropsychologists.
