There is much concern about population aging and its consequences for nations, for governments, and for individuals. It has often been noted that population aging will inevitably affect the economic stability of most countries and the policies of most state governments. What is less obvious, but equally important, is that population aging will profoundly affect families. Who will care for the growing numbers of tomorrow's very old members of societies? Will it be state governments? The aged themselves? Their families? The purpose of this volume is to examine consequences of global aging for families and intergenerational support, and for nations as they plan for the future. Four remarkable social changes during the past fifty years are highlighted: (1) Extension of the life course: A generation has been added to the average span of life over the past century; (2) Changes in the age structures of nations: Most nations today have many more elders, and many fewer children, than fifty years ago; (3) Changes in family structures and relationships: Some of these differences are the result of trends in family structure, notably higher divorce rates and the higher incidence of childbearing to single parents; (4) Changes in governmental responsibilities: In the last decade, governmental responsibility appears to have slowed or reversed as states reduce welfare expenditures. How will families respond to twenty-first-century problems associated with population aging? Will families indeed be important in the twenty-first century, or will kinship and the obligations across generations become increasingly irrelevant, replaced by "personal communities"? Book excerpt: China, which is fast on its way to becoming the most powerful economic force in the world, has four unique characteristics that distinguish it from other countries in Asia: (1) The proportion of aging population is growing faster than that of Japan (the country previously recognized as having the fastest rate) and much faster than nations in western Europe. (2) An early arrival of an aging population before modernization has fully taken place, with social policy implications. It is certain that China will face a severely aged population before it has sufficient time and resources to establish an adequate social security and service system for older people. (3) There will be fluctuations in the total dependency ratio. The Chinese government estimates are that the country will reach a higher dependent burden earlier in the twenty-first century than was previously forecast. (4) The government’s fertility policy (single child per family) and its implementation has a strong influence on the aging process. Fewer children are being born, but with more elderly people a conflict arises between the objectives to limit population increase and yet maintain a balanced age structure (Peng and Guo 2001). The intersection of these fourfold factors means that the increased aging population is giving rise to serious concerns among Chinese social policy makers. There is a chronic lack of good resource materials that attempt to make sense of social policy in its relationship to examining the problems and possibilities of human aging grounded in an analysis of economic of social policy in China and impact on rural and urban spaces. Such analysis of China will be covered by conceptual, theoretical, and empirical approaches. The book will also discuss substantive topics of housing, community care, family care, pensions, and mental health. The book brings together a truly world class array of researchers to provide discussions of critical implications of aging social policy and the economic impact in China.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.

Aboderin explores this topic specifically for Africa. In the wake of social or economic change, societies experience shifts in the degree to which families support their elders. Questions about the proper balance of family and state responsibility, however, persist, especially in light of socio-demographic trends and constraints in public expenditure. In most of sub-Saharan Africa, in contrast to other world regions, economic security policies for older people have not yet been formulated, despite declines in material family support along with rising poverty to which a growing elderly population is exposed. In part, this betrays the crucial lack of understanding about how and why these shifts in support have occurred in African societies--and, thus, a profound uncertainty about what balance of individual, family, and state responsibilities will be culturally appropriate and effective in ensuring economic security for older Africans both now and in the future. Aboderin aims to address these gaps in understanding. She provides an empirical and theoretical analysis of the micro and macro level processes that have underpinned recent declines in old age family support in African societies and likely parameters of future familial support. She also addresses more fundamental theoretical questions about how we should think about the relationships between intergenerational support, norms and values, and societal change. Intergenerational Support in Africa will be of interest to students of African studies, economic policy and theory concerning eldercare, sociology, and social welfare development.
