Book excerpt: Les anticoagulants oraux directs constituent une classe de médicaments relativement récente dont l'utilisation connait une forte croissance et qui constituent une alternative intéressante aux anti-vitamine K. La gestion de ces AOD dans le cadre de la réalisation d'actes chirurgicaux reste malgré tout problématique ; en chirurgie dentaire notamment, la conduite à tenir n'est pas parfaitement établie. L'étude détaillée de la pharmacologie des AOD permet de mesurer pleinement l'intérêt de ces médicaments. Grâce notamment à une fenêtre thérapeutique large, leur usage se révèle plus simple, plus souple et plus sûr que les AVK. La connaissance de ces propriétés pharmacologiques permet d'établir un guide des pratiques à suivre en chirurgie dentaire. La conduite à tenir s'avère au final assez simple, l'interruption du traitement n'étant généralement pas indiquée pour les actes de chirurgie les plus courants. Les données cliniques restent cependant insuffisantes à ce jour, et les résultats des études actuellement en cours pourraient se révéler précieux pour l'amélioration de la prise en charge des patients sous AOD en chirurgie dentaire.
