L'objectif de notre étude était d'étudier le strain VG à la phase aiguë de l'IDM, et d'identifier d'éventuels paramètres prédictifs d'une reperfusion artérielle au stade aigu et d'une viabilité à 7 jours. Une échocardiographie était réalisée au stade aigu de l'IDM, avant coronarographie urgente, et une seconde entre le 5e et 7e jour. Sur les images échographiques analysées secondairement, 4 paramètres de strain (pics systolique, total, diastolique, et temps au pic total) étaient mesurés dans les plans longitudinal, radial et circonférentiel. Vingt-sept patients ont été inclus. L’objectif de l’étude est de tester l’intérêt de la mesure non invasive de la réserve de flux coronaire (RFC), par écho-doppler transthoracique, à la phase aigue de l’infarctus du myocarde antérieur (IDM) revascularisé. Méthode : Chez 48 patients consécutifs présentant un premier IDM antérieur, une mesure non invasive de la RFC et une échographie cardiaque standard ont été réalisées dans les 24 heures suivant la revascularisation par angioplastie. La RFC était définie par le rapport de la vitesse du flux diastolique en phase d’hyperhémie, obtenue par injection intra veineuse d’adénosine (140 μg/kg/min sur 2 minutes) sur la vitesse du flux diastolique à l’état basal. La récupération de contractilité était définie par une amélioration de 10% de la FEVG, le remodelage ventriculaire (RV) par une majoration de 15% du volume télésystolique (VTS) lors du suivi, et les évènements cardiaques par la survenue d’une insuffisance cardiaque, d’un décès de cause cardiaque, d’une revascularisation ou d’une récidive d’infarctus sur le même territoire. Résultats : Trois analyses ont été réalisées pour étudier l’intérêt de la RFC sur les différents critères (récupération de FEVG, remodelage et évènements). Les patients étaient âgés de 58 ± 13 ans, ils avaient des caractéristiques cliniques et des traitements identiques dans les différents groupes. Le niveau d’atteinte de l’IVA n’était pas différent entre les groupes, pour chaque critère. Les patients avec récupération de la FEVG avaient une RFC initiale significativement plus élevée que ceux qui n’ont pas récupéré (2,07±0,58 vs 1,46±0,18 p

Book excerpt: Many noteworthy advances in our knowledge of the pericardium, its functions and diseases and their relation to heart failure have been made since the first edition of this book appeared in 1981; and no other book that covers in detail the physiology and pathophysiology has since been published. The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.
