Book excerpt: Obesity is associated with an increased risk for breast cancer in postmenopausal women and for triple negative and inflammatory breast cancer subtypes in premenopausal women. Obesity is also linked to a poor clinical prognosis. The rise of obesity worldwide signifies the need to better understand these links of obesity to breast cancer. To study how obesity impacts breast cancer risk, we examined mammary glands from mice fed a high fat diet (HFD) and reduction mammoplasty tissue from women with known BMI to identify how obesity alters normal mammary tissue. Within the mammary epithelium, we observed increased stem/progenitor activity with enhanced numbers of cells expressing estrogen receptor alpha (ER[small alpha]), as well as, loss of basal/myoepithelial cell populations. We also observed increased macrophage infiltration surrounding mammary ducts. Using macrophage depletion strategies, we identified macrophage-secreted decorin as a regulator of TGF[capital beta] activity specific to obesity. Many of these changes in obese mammary tissue appeared to be reversible with weight loss. To study carcinogenesis in obesity, we utilized a well-characterized trp53-/- transplant model of mouse mammary tumorigenesis. Mice fed the HFD had increased tumor incidence compared to controls. Eight weeks after transplant, preneoplastic hyperplastic mammary ducts were significantly increased in obese mice with luminal epithelial cell expansion, loss of basal cells, and enhanced stem/progenitor activity. Similar to non-tumor bearing mice, we observed an increased recruitment of macrophages to the preneoplastic lesions in obese mammary tissue compared to controls. To understand how macrophage-driven inflammation due to obesity promoted tumor formation, mice were treated with IgG or anti-F4/80 antibodies to deplete macrophages following the transplant of p53-/- mammary epithelial cells. Obese mice treated with anti-F4/80 antibodies demonstrated enhanced mammary epithelial stem/progenitor activity, elevated expression of ER[small alpha], and increased numbers of cells expressing [capital gamma]H2AX, a marker for DNA damage. Depletion of macrophages in the HFD-fed mice lead to tumor formation at 8 weeks post-transplant. Cumulatively, these results suggest macrophages decrease the presence of DNA damaged cells in obesity, which may decrease tumor formation or prolong tumor latency. Together our studies show altered breast cancer risk through influences on mammary microenvironment and stem and progenitor cells under obesity.

The proportion of people who are overweight/obese is increasing, and the amount of physical activity is decreasing in most populations, including urban populations in many developing countries. The increasing prevalence of overweight/obesity is presumably due to the increasing availability of highly palatable, high-energy foods, and an increasing sedentary lifestyle due to mechanisation of both workplace and leisure activities. Overweight/obesity and reduced physical activity increases the risk of cancers in various organs. Maintaining a healthy body weight and regular physical activity is the second most important way to prevent cancer, after tobacco control. The suggestions of possible public health actions to tackle these risk factors include the promotion of balanced diets, which are not excessive in energy, and broad education and planning to enable and encourage physical activity during work and leisure. Recommendations and a full discussion of these topics are included in the sixth volume in this series of Handbooks.

With the on-going debate about the requirement of EMT for cancer metastasis, an emerging focus on intermediate states of EMT and its reverse process mesenchymal-epithelial transition (MET) offer new ideas for metastatic requirements and the dynamics of EMT/MET during the entire metastatic cascade. Therefore, we would like to initiate discussions on viewing EMT and its downstream signaling networks as a fulcrum of cellular plasticity, and a facilitator of the adaptive responses of cancer cells to distant organ microenvironments and various therapeutic assaults. We hereby invite scientists who have prominently contributed to this field, and whose valuable insights have led to the appreciation of epithelial-mesenchymal plasticity as a more comprehensive mediator of the adaptive response of cancer cells, with huge implications in metastasis, drug resistance, tumor relapse, and patient survival.

Book excerpt: Angiogenesis is the physiological process where new blood vessels grow from existing ones, in order to replenish tissues suffering from inadequate blood supply. Perhaps the most studied angiogenic process occurs in solid tumors whose growing mass and expanding cells create a constant demand for additional supply of oxygen and nutrients for survival. However, other physiological and clinical conditions, such as wound healing, ischemic events, autoimmune and age-related diseases also involve angiogenesis. Angiogenesis is a well-structured process that begins when oxygen and nutrients are depleted, leading to the release of chemokines and growth factors that attract immune cells, particularly macrophages and endothelial cells to the site. Macrophages that are recruited to the site, as well as tissue cells and endothelial cells, secrete pro-angiogenic mediators that affect endothelial cells and promote angiogenesis. These mediators include growth factors such as vascular endothelial cell growth factor (VEGF), matrix metalloproteinases (MMPs), as well as low levels of mediators that are usually seen as pro-inflammatory but are pro-angiogenic when secreted in low levels (e.g. nitric oxide (NO) and TNFa). Thus, macrophages play a major role in angiogenesis. Macrophages exhibit high plasticity and are capable of shifting between different activation modes and functions according to their changing microenvironment. Small differences in the composition of activating factors (e.g. TLR ligands such as LPS, anti-inflammatory cytokines, ECM molecules) in the microenvironment may differently activate macrophages to yield classically activated macrophages (or M1 macrophages) that can kill pathogen and tumor cells, alternatively activated macrophages (or M2 macrophages) that secrete antiinflammatory cytokines, resolution macrophages (rM?) that are involved in the resolution of inflammation, or regulatory macrophages (e.g. Myeloid-Derived Suppressor Cells - MDSCs) that control the function of other immune cells. In fact, macrophages may be activated in a spectrum of subsets that may differently contribute to angiogenesis, and in particular non-classically activated macrophages such as tumor-associated macrophages (TAMs) and Tie2-expressing monocytes (TEMs) can secrete high amounts of pro-angiogenic factors (e.g. VEGF, MMPs) or low levels of pro-inflammatory mediators (e.g. NO or TNFa) resulting in pro-angiogenic effects. Although the importance of macrophages as major contributors and regulators of the angiogenic process is well documented, less is known about the interactions between macrophages and other cell types (e.g. tumor cells, normal epithelial cells, endothelial cells) that regulate angiogenesis. We still have only limited understanding which proteins or complexes mediate these interactions and whether they require cell-cell contact (e.g. through integrins) or soluble factors (e.g. the EGF-CSF-1 loop), which signaling pathways are triggered in each of the two corresponding cell types, and how this leads to secretion of pro- or antiangiogenic factors in the microenvironment. The regulation of such interactions and through them of angiogenesis, whether through post-translational modifications of proteins or via the involvement of microRNA, is still unclear. The goal of this Research Topic is to highlight these interactions and their regulation in the context of both physiological and pathological conditions.
