Accumulated big data are now estimated to be 4.4 zettabytes in the digital universe; and trends predict an exponential increase in the future. Health data are gathered from professional routine care and other expanded sources including the social determinants of health, such as Internet of Things. Biomedical research has recently moved through three stages in digital healthcare: (1) data collection; (2) data sharing; and (3) data analytics. With the explosion of stored health data, dental medicine is edging into its fourth stage of digitization using new technologies including augmented and virtual reality, artificial intelligence, and blockchain. Big data collaborations involve interactions between a diverse range of stakeholders with analytical, technical and political focus. In oral healthcare, data technology has many areas of application: prognostic analysis and predictive modeling, the identification of unknown correlations of diseases, clinical decision support for novel treatment concepts, public health surveys and population-based clinical research, as well as the evaluation of healthcare systems. The objective of this Special Issue is to provide an update on the current knowledge with state-of-the-art theory and practical information on human and social perspectives that determine the uptake of technological innovations in big data science in the field of dental medicine. Poor and minority children are less likely to have access to oral health care than are their nonpoor and nonminority peers. Older adults, people who live in rural areas, and disabled individuals, uniformly confront access barriers, regardless of their financial resources. The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care. These recommendations support the creation of a diverse workforce that is competent, compensated, and authorized to serve vulnerable and underserved populations across the life cycle. The recommendations provided in Improving Access to Oral Health Care for Vulnerable and Underserved Populations will help direct the efforts of federal, state, and local government agencies; policy makers; health professionals in all fields; private and public health organizations; licensing and accreditation bodies; educational institutions; health care researchers; and philanthropic and advocacy organizations.

Book excerpt: Background: Millions of Americans continue to live with one or more oral diseases, including dental caries, which lead to periodontal disease. Periodontal disease continues to have a significant impact on those with chronic disease states, such as diabetes. Multiple reasons exist for why a significant improvement in oral health has not occurred since the original call to action was released. These reasons include lack of provider buy-in, access to dental providers, poor patient oral health literacy, and lack of finanical resources. Purpose: The purpose of this scholarly project was to integrate an oral health-screening tool for healthcare providers to use to guide oral health screening examinations in an effort to improve interdisciplinary care for patients who have been diagnosed with diabetes. This project was also designed to improve the documentation of discrete data elements of the oral health exam within the electronic health record (EHR) utilized in the Coordinated Care Organization (CCO). Methods: Using the Rosswurm and Larrabee Model for Change to Evidence Based Practice (EBP), a process improvement project was conducted implementing the Oral Health Assessment Tool (OHAT) in primary care practice setting within the CCO. Lewin's Change theory provided additional guidance in asking the healthcare providers to implement this change project into their practice. Findings: The project was conducted as a pilot project in a clinic located in Southern Oregon over a four-week period of time where a total of fifty-five patients with a diagnosis of Diabetes Mellitus were evaluated. Of this number, thirty-one patients were screened using the OHAT flowsheet. A healthcare provider experience questionnaire was also administered at the conclusion and provided necessary feedback in how to improve upon the processes that were employed during the rollout of the project. Recommendations: Further study is needed in using the OHAT in additional clinical sites within the primary care practice setting in order to compare the original results with the new data. Additional continuing education on oral health for healthcare providers should include how to conduct oral health assessments, thus improving the comfort level of the primary care providers (PCP) and clinical staff in using the OHAT. Further work is also warranted on how to improve the communication between medical and dental care providers.

The current research was designed to describe the development and implementation of the Terry Reilly Health Services (TRHS) Latah Medical-Dental-Behavioral Health integrated clinic (Latah Clinic). Data was collected through structured interviews with a purposive sample of employees from TRHS; observations of a Latah Clinic care team meeting; an environmental scan of facility space; and an analysis of intake forms used at the Latah Clinic. Seven employees from TRHS were invited to participate in structured interviews. Five participants completed the interviews, three face-to-face and two via e-mail. Checklists were developed to document evidence of integration during the Latah Clinic care team meeting, environmental scan and analysis of intake forms. The findings from this study suggest that the Latah Clinic is fully integrated and demonstrates high levels of collaboration. The clinic would like to continue to grow and improve, therefore, suggestions to aide in this endeavor were provided. Study limitations such as small sample size and the lack of information from the perspective of the patient must be considered when interpreting the findings. TRHS Latah Medical-Dental-Behavioral Health Clinic is a rarity in the United States. The strategies used in the TRHS integration process serve as a model for other practices and health care organizations as they evolve toward becoming patient-centered medical-dental homes."--Boise State University ScholarWorks.
