Book excerpt: The technical paper distils key messages from the World Health Organization publication “Voice, agency, empowerment – handbook on social participation for universal health coverage”, along with feedback from a multistakeholder consultation process to identify priority actions for Member States to institutionalize social participation in decision-making processes for health. Social participation is defined as empowering people, communities and civil society, through inclusive participation in decision-making processes that affect health, across the policy cycle and at all levels of the system. It is core to primary health care and promotes equitable progress towards universal health coverage, producing more responsive health policies and programmes, and helping to foster population trust with the health system. While Member States have endorsed the principle of social participation, translation into practice has been inadequate. The following priority actions are identified: (a) Strengthen government capacities to design and implement social participation (b) Secure equitable, diverse and inclusive representation (c) Ensure that social participation informs decision-making for health across the policy cycle (d) Systematize and sustain regular social participation, including through legal frameworks (e) Invest adequate, stable and predictable financial resources for social participation (f) Facilitate capacity strengthening and financial resources for civil society (g) Monitor and use data and evidence routinely to evaluate participatory processes and their impact on decisions made.

However, avoidable adverse events, errors and risks associated with health care remain major challenges for patient safety globally. The Seventy-second World Health Assembly in 2019 adopted resolution WHA72.6 on global action on patient safety and mandated for development of a global patient safety action plan. This global action plan was adopted by Seventy-Fourth World Health Assembly in 2021 with a vision of “a world in which no one is harmed in health care, and every patient receives safe and respectful care, every time, everywhere”. The purpose of the action plan is to provide strategic direction for all stakeholders for eliminating avoidable harm in health care and improving patient safety in different practice domains through policy actions on safety and quality of health services, as well as for implementation of recommendations at the point of care. The action plan provides a framework for countries to develop their respective national action plans on patient safety, as well to align existing strategic instruments for improving patient safety in all clinical and health-related programmes.

Book excerpt: Having a child remains one of the biggest health risks for women worldwide. Fifteen hundred women die every day while giving birth. That's a half a million mothers every year. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work.The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.

Book excerpt: Significant programmatic experience and research evidence regarding HIV and infant feeding have accumulated since WHO's recommendations on infant feeding in the context of HIV were last revised in 2006. In particular, evidence has been reported that antiretroviral (ARV) interventions to either the HIV-infected mother or HIV-exposed infant can significantly reduce the risk of postnatal transmission of HIV through breastfeeding. This evidence has major implications for how women living with HIV might feed their infants, and how health workers should counsel these mothers. Together, breastfeeding and ARV intervention have the potential to significantly improve infants' chances of surviving while remaining HIV uninfected. While the 2010 recommendations are generally consistent with the previous guidance, they recognize the important impact of ARVs during the breastfeeding period, and recommend that national authorities in each country decide which infant feeding practice, i.e. breastfeeding with an ARV intervention to reduce transmission or avoidance of all breastfeeding, should be promoted and supported by their Maternal and Child Health services. This differs from the previous recommendations in which health workers were expected to individually counsel all HIV-infected mothers about the various infant feeding options, and it was then for mothers to decide between them. Where national authorities promote breastfeeding and ARVs, mothers known to be HIV-infected are now recommended to breastfeed their infants until at least 12 months of age. The recommendation that replacement feeding should not be used unless it is acceptable, feasible, affordable, sustainable and safe (AFASS) remains, but the acronym is replaced by more common, everyday language and terms. Recognizing that ARVs will not be rolled out everywhere immediately, guidance is given on what to do in their absence.
