La thérapeutique initiale de cette parodontite a pour but de stopper le processus de destruction de tissus de soutient de l'organe dentaire en luttant contre l'infection via l'élimination du biofilm et du tartre adhérent aux surface radiculaire. Le laser Er :YAG 2,94μm est l'un des nombreux lasers utilisables en parodontologie, sa mise sur le marché remonte au milieu des années 90. L'étude de la littérature présentée dans notre travail doit nous permettre de déterminer si le laser Er :YAG 2,94μm peut être l'un des instruments de prise en charge d'un patient souffrant de parodontite chronique. Une première étude de 1994 aboutit à des résultats très prometteurs sur le sujet et encourage à réaliser d'autres études plus précises. Histologiquement tout d'abord, on peut conclure des différentes études que l'état de surface apès irradiation laser est hautement biocompatible favorisant la recréation d'une attache epithélio-conjonctive de qualité. Il a aussi été démontré que l'irradiation au laser Er :YAG possédait une réelle activité antibactérienne, notamment sur les bactéries parodontopathogènes. Ensuite, la comparaison in vivo d'un traitement laser aux autres démarches plus conventionnelles permet de conclure que le laser Er :YAG 2,94μm est au moins aussi efficace que les autres techniques de traitement de la parodontite chronique (instrumentation manuelle ou ultrasonore). En effet, parmi les études longétudinales jusqu'à 2 ans post-intervention, aucune n'a desservi l'utilisation du laser. L'intérêt du laser Er :YAG 2,94μm dans le traitement de la parodontite chronique de l'adulte reste avant tout clinique et le choix d'un tel appareil en pratique quotidienne sera motivé par d'autres critères plus subjectifs qui reste à étidier.
