Book excerpt: Les objectifs sont : 1) Valider par rapport à l'IRM la fraction d'éjection ventriculaire gauche (FEVG), à l'aide d'un nouveau logiciel d'échocardiographie tridimensionnelle. 2) Evaluer l'intérêt d'une mesure tridimensionnelle des volumes et de l'asynchronisme ventriculaire gauche en pré et post resynchronisation, puis à 6 mois. METHODE : 1) La mesure de FEVG et des volumes ventriculaires par échographie tridimensionnelle chez 32 patients a été comparée à celle de l'IRM. 2) Une échocardiographie bidimensionnelle puis tridimensionnelle avec mesure d'asynchronisme a été réalisée chez 25 patients en pré et post-implantation avec optimisation des délais ventriculo-ventriculaires (VV), puis à 6 mois d'une stimulation biventriculaire. RESULTATS : 1) La reproductibilité inter et intraobservateur de la mesure échographique des fractions d'éjection et volumes ventriculaires est excellente (ICC = 0,99). Book excerpt: La resynchronisation biventriculaire s'est récemment positionnée dans l'arsenal thérapeutique de certains patient insuffisants cardiaques, réfractaires à un traitement médical optimal. Cependant, les critères actuels utilisés par les recommandations des sociétés savantes pour la sélection des patients candidats à une resynchronisation sont très imparfaits et il persiste un taux inacceptable de patients non répondeurs d'environ un tiers. Beaucoup d'espoirs sont actuellement portés sur l'échographie cardiaque pour améliorer la sélection des patients candidats à cette thérapeutique. Notre étude a travaillé sur l'intérêt de l'échocardiographie tridimentionnelle. Elle a inclus 18 patients insuffisants cardiaques éligibles à une thérapie de resynchronisation. Les patients présentant des critères de désynchronisation à l'échocardiographie 3D représentaient le groupe témoin (resynchronisation activée). Les patients ne présentant pas ces critères ont été randomisés en simple aveugle : un groupe avec resynchronisation activée et un groupe avec resynchronisation désactivée. Le suivi a été de 6 mois. Cette étude a permis de confirmer l'effet bénéfique de la resynchronisation sur les paramètres d'évaluation tels que l'évolution de la classe NYHA par exemple (p=0.045). On note également une réponse à la resynchronisation paraissant meilleure chez les patients présentant des critères d'asynchronisme à l'échocardiographie 3D.
