Le pharmacien d'officine est un acteur majeur du parcours de santé du patient et de son suivi thérapeutique. L'objectif principal était d'évaluer l'impact de l'intégration du pharmacien d'officine dans la prise en charge de la personne âgée venant consulter en hôpital de jour gériatrique et bénéficiant d'une optimisation pharmaceutique. Les patients venant consulter à l'hôpital de jour (HDJ) des Charpennes entre janvier et juin 2016 pour un bilan de chute et/ou mémoire et dont le nom de la pharmacie était disponible ont été inclus. Les pharmacies d'officines ont été contactées pour recueillir le traitement délivré les derniers mois. Les informations ont été standardisées et intégrées aux optimisations pharmaceutiques pour transmission au médecin traitant par l'intermédiaire du compte-rendu d'hospitalisation ainsi qu'au pharmacien d'officine. Pour évaluer la démarche, l'évolution des traitements réellement délivrés au patient a été suivie par un deuxième appel à 3 mois. Enfin la satisfaction des pharmacies d'officine a été recueillie. Quarante patients d'âge moyen 81,8 +/- 6,8 ans ont été inclus, 85% présentaient des troubles cognitifs, 60% vivaient avec leurs conjoints à domicile et 25% vivaient seuls à domicile. Au regard des prescriptions des patients, les taux des médicaments habituels prescrits et délivrés à l'officine ont été augmentés significativement à de 3 mois après l'HDJ respectivement de 84% à 93% (p-value

Book excerpt: L'hôpital de jour (HdJ) de détection de la fragilité et de prévention de la dépendance permet de détecter les patients âgés fragiles et de mettre en place des mesures préventives personnalisées pour lutter contre la perte d'autonomie et retarder le processus d'entrée dans la dépendance. La revue de médication est réalisée quotidiennement à l'hôpital de jour par le pharmacien clinicien en concertation avec le gériatre. Elle permet de détecter les prescriptions potentiellement inappropriées (PPI) selon des critères explicites et implicites, puis de proposer aux médecins traitants une optimisation de la prescription selon les types de problèmes détectés. L'objectif principal de cette étude était de montrer l'impact de cette revue sur l'optimisation des prescriptions par le médecin traitant. Le taux d'acceptation des propositions a été déterminé 3 mois après avoir vu le patient en contactant les pharmaciens d'officine et les médecins traitants. 191 patients ont été inclus. 69% d'entre eux présentaient au moins une PPI (0-9). Parmi les 141 patients ayant eu au moins une proposition d'optimisation, 1 patient sur 2 avait au moins 57 % des interventions proposées acceptées trois mois après le passage à l'HdJ. Les optimisations concernaient le plus souvent l'arrêt ou le remplacement d'un médicament dont la balance bénéfice/risque est défavorable (56.5% des patients), et l'arrêt d'un médicament non indiqué (26%). Lorsque les médecins n'avaient pas modifié la prescription, cela concernait le plus souvent les médicaments de la classe cardiovasculaire (20%), neurologique (17.7%) et du système digestif (12%). Les arguments les plus fréquents étaient qu'ils ne souhaitaient pas modifier la prescription du spécialiste (15% des patients), que le médicament était bien indiqué (11%), ou que la tentative d'arrêt s'était soldée par un échec (9%). Notre étude montre que de nombreux patients avaient une prescription potentiellement inappropriée. Cependant, dans plus de la moitié des cas, les médecins généralistes acceptent les recommandations préconisées pour optimiser les prescriptions. L'hospitalisation des patients à la plateforme fragilité semble donc être un dispositif efficient pour optimiser la prescription médicamenteuse en soins primaires.

Book excerpt: L'évaluation gériatrique standardisée en hôpital de jour est le moment idéal pour faire une réévaluation optimale des thérapeutiques médicamenteuses. L'objectif de ce travail est d'évaluer l'acceptation des propositions d'optimisation thérapeutique en médecine de ville. Pour chaque admission en HDJ, un externe en pharmacie filière officine réalise la première étape de conciliation médicamenteuse à savoir qu'il établit le bilan médicamenteux de chaque patient. La pertinence de chaque médicament est ensuite réévaluée selon des critères implicites (antécédents, état clinique, résultats biologiques, objectifs thérapeutiques définis) et des critères explicites (recommandations d'expert, listes de médicaments inappropriés chez la personne âgée). Les modifications préconisées sont synthétisées sur une « fiche de proposition d'optimisation thérapeutique » systématiquement transmise au médecin ́ généraliste ainsi qu'au pharmacien d'officine du patient. Les pharmaciens d'officine ont été recontactés dans un délai d'à minima 3 mois après l'hospitalisation afin de suivre l'évolution des prescriptions. Après l'analyse de 60 prescriptions de patients, 52 (86.7%) étaient sous- optimales (overuse, misuse, underuse). Au total, 230 propositions d'optimisation ́ thérapeutique ont été formulées soit en moyenne 3.83 ± 2.03 par patient (1 ; 10). Parmi les classes thérapeutiques les plus concernées par les propositions d'optimisation, on recense la vitamine D (14,8%), les antidépresseurs (12,5%), les vaccins (12,5%), les médicaments du système cardiovasculaire (11,6%), les anxiolytiques (6.5%), les IPP (6,5%) et les antalgiques ́ (5,6%). Le suivi des propositions d'optimisation par les médecins généralistes a été total pour 40% des patients, partiel (au moins 1 proposition non acceptée) dans 45% des cas et nul pour 15% des patients. La forte proportion de prescriptions sous-optimales montre l'importance de leur réévaluation pour prévenir le risque de iatrogénie.
