Book excerpt: Parkinson's disease (PD) is an age-related neurodegenerative disorder characterized by progressive motor disability with a spectrum of non-motor symptoms. Similar to Parkinson's disease, essential tremor, dystonia, Huntington's disease, and other aging-related movement disorders are widespread in the elderly with relatively unideal medicine-treatment effects. In recent years, deep brain stimulation (DBS) has been established as an effective treatment for PD and other aging-related movement disorders, especially with motor symptoms. However, there are still many problems to be solved in research work and clinical practice in this field. For example, it is unclear regarding the most effective stimulation targets or the underlying mechanism of DBS. And it is still unknown how to choose the optimal targets among STN, GPi, PSA, VIM, etc, or if we could choose the nerve fiber for stimulation besides the nuclei, And despite wearable sensors could be a powerful tool for the digital evaluation of movement disorders or provide the feedback biomarkers for DBS, the closed-loop biomarkers for DBS are still lacking. Furthermore, apart from PD, the DBS treatment in gene-related dystonia, Huntington's disease, and other movement disorders also needs to be further explored.

Despite these activities are performed daily, there is a high requirement of motor and neural systems in order to perform both tasks efficiently. Both walking and posture require a complex interaction of musculoskeletal and neural systems. However, the mechanisms used to control these tasks, as well as how they are planned and coordinated, are still a question of discussion among health professionals and researchers. In addition, this discussion is more interesting when the effects of aging are included in the context of locomotion and the postural control. The number of older individuals is 841 million in 2015, which is four times higher than the 202 million that lived in 1950. Aging causes many motor, sensorial and neural deficits, which impair locomotion and postural control in the elderly. The severity of this framework is worsened when the aging goes along with a movement disorder, such as Parkinson disease, Chorea, Dystonia, Huntington disease, etc. Therefore, the aim of this book is to highlight the influence of different aspects on planning, controlling and performing locomotion and posture tasks. In attempting to improve current knowledge in this field, invited authors present and discuss how environmental, sensorial, motor, cognitive and individual aspects influence the planning and performance of locomotor and postural activities. The major thrust of the book is to address the mechanisms involved in controlling and planning motor action in neurological healthy individuals, as well as in those who suffer from movement disorders or face the effects of aging, indicating the aspects that impair locomotion and postural control. In addition, many patients display non-motor symptoms, including a diminished sensation of smell, gastrointestinal problems, various disorders of sleep and some cognitive impairment. These clinical features - particularly the motor signs - manifest after a progressive death of many dopaminergic neurones in the brain. Although currently available, conventional therapies can reduce the signs of the disease, the progression of this neuronal death has proved difficult to slow or stop, and the condition is relentlessly progressive. Hence, there is a real need to develop a treatment that is neuroprotective, one that slows the pathology of the disease effectively. At present, there are several neuroprotective therapies in the experimental pipeline, but these are for the patients of tomorrow. This book focuses on two therapies that are readily available for the patients of today. They involve the use of exercise and light (i.e. photobiomodulation, the use of red to infrared light therapy (λ=600-1070nm) on body tissues). The two therapies are tied together in several ways. First, in animal models of Parkinson's disease, they each have been shown to offer the key feature of neuroprotection, stimulating a series of built-in protective mechanisms within the neurones, that helps their survival, to self-protect and/or self-repair. There are also some promising indications of neuroprotection and many beneficial outcomes in parkinsonian patients. Further, both exercise and light therapies are similar in that they are non-invasive and safe to use, with no known adverse side-effects, making their combination with the conventional therapies, such as dopamine replacement drug therapy and deep brain stimulation, all the more feasible. Given the heterogeneity of Parkinson's disease in humans, tackling the condition from a range of different angles - with a number of different therapies - would only serve to enhance the positive outcomes. This book considers the use of exercise and light therapies, proposing that they have the potential to make a powerful "dynamic duo", offering a most effective neuroprotective treatment option to patients.
