Despite the reliability of this technique, several studies showed marginalbone loss and gingival aesthetics after one year of treatment, so there is a discussion about whether it can be considered a techniqueof alveolar preservation.Conclusions and Clinical implications (500 characters max.)In conclusion immediate implant placement is a reliable and predictable technique. Long-term data showed that patients treated withimmediate implants presented stability for their prosthetic rehabilitation, showed no aesthetic compromise and were associated with a clinical and radiological stability of the peri-implant tissues.A surgical protocol for the placement of the immediate implant placement should be studied and created, which must include, among other factors, the clinical skills of the clinician, positioning of the dental implant in the extraction socket and soft tissue management. In order to compensate for the volume loss, GBR and minimally invasive techniques need to be used to achieve successful esthetic result.

Abstract: Background: Type I immediate implant placement has gained popularity because it may reduce treatment time, number of surgeries and post-extraction bone loss. However, this is potentially challenged by inadequate keratinized mucosa for flap adaptation and difficulties in achieving primary stability. Moreover, it has been proven that post-extraction bone loss is an inevitable biological process, which affects treatment outcomes. Objectives: To estimate survival and success rates of implants and the implantsupported prostheses, the prevalence of biological, technical and aesthetic complications, and the magnitude of soft and hard tissue changes following implant placement immediately into fresh extraction sockets. Material & methods: An electronic search in MEDLINE (PubMed) and the Cochrane library from 1991 to July 2010 was performed to include prospective studies on immediate implants with a mean follow-up time of at least 1 year. The survival rates were computed using the STATA statistical software. Weighted means of soft and hard tissue changes were obtained by the inverse variance method. Results: A total of 46 prospective studies, with a mean follow-up time of 2.08 years, were included. The annual failure rate of immediate implants was 0.82% (95% CI: 0.48%-1.39%), translating into the 2-year survival rate of 98.4% (97.3%-99.0%). Among the 5 factors analyzed [reasons for extraction, antibiotic use, position of implant (anterior versus posterior, maxilla versus mandible), type of loading], only the regimen of antibiotic use affected the survival rate significantly. Lower failure rates were found in groups that were provided with a course of post-operative antibiotics. The success of implant therapy was difficult to assess due to scarce reporting on biological, technical and aesthetic complications. Soft tissue changes occurred mostly in the first 3 months after the provision of restoration, and then stabilized towards end of the first year. Marginal bone loss predominantly took place in the first year after implant placement, with a magnitude generally less than 1mm. Controversy on hard tissue preservation with platform-switching technique remained unsolved. Conclusions: Despite the high survival rate observed, more long-term studies are necessary to determine the success of implant treatment provided immediately after tooth extraction. Another advantage would be to preserve the horizontal and vertical dimension of the alveolar ridge after tooth extraction since several studies have shown dimensional changes of the alveolar ridge after tooth extraction. The temporary restoration plays an important role maintaining soft tissue esthetics around the new dental complex.Purpose:The aim of this retrospective, clinical study was to describe the healing socket guide technique using an anatomical and transmucosal guide (screw-retained provisional restorations) on immediate implants and to evaluate clinical parameters and esthetic outcomes of the peri-implant soft tissues over a period of four months, when the prosthetic device was removed for first time.Material and Methods:Thirty patients were treated in private practice with 49 immediate implants (Zimmer-Biomet Dental, Inc.) and immediate loading using immediate (screw-retained) provisional restorations. The presence of the hole on the occlusal surface of the provisional restoration, guided the implant placement in the right position, especially in multiple sites. The anatomical and transmucosal shape of the new dental device determine compression of the surrounding tissues on the buccal aspect and interdental papillae and contain the biomaterial during the healing stage into the fresh extraction socket. The characteristics of the provisional restoration constructed to allow a healing guide of the soft tissue, creating a new complex of the peri-implant soft tissues (Socket Healing Guide). Immediate single or multiple splinted, screw-retained provisional restorations were used and loaded when implants achieved good initial stability (more than 35 Ncm) and removed after 4 months. The length of the implants ranged from 10-13mm and the implant diameter was 3.75-4.7mm. The teeth were extracted for many reasons, even presence of pathological conditions, which determined the cororal partial loss of the buccal bone wall. Gingival biotype (thick and thin) of the teeth was recorded. Recording of periodontal parameters (PPD, FMPS, FMBS, PHM, PHD and gingival biotype) were carried out by the surgeon and a cross-examiner in all patients before surgical treatment (baseline) and compared with the four month-follow up when the provisionals were removed for the first time. Then, the horizontal dimension of the soft tissue was evaluated from the most prominent buccal point to the same lingual point of the dental cast with the use of a digital micrometer. In order to make accurate measurements at the same position, a silicone jig was built and rested on the adjacent teeth. The esthetic outcome of the soft tissues around each implant-supported single crown was evaluated using the Pink Esthetic Score (PES) when the final restoration was delivered.Results:The survival rate of the implants at 4 months was 100%. The full mouth plaque score and full mouth bleeding score were at the baseline (FMPS 17.2, FMBS 28.2) and after 4 months (FMPS 15.1, FMBS 23.2), respectively. The PH measurement of mesial and distal papillae at baseline and 4 months of follow-up statistically significantly increased compared to baseline from 1.50 to 1.20 mm, respectively. The results of the horizontal cast-based measurements (in mm) were 12.04u00b12.06 at baseline and 11.55u00b12.07 at 4 month follow up (change=0.49mm). PES used to evaluate esthetics of the peri-implant soft tissues after four months was 13.5; this indicates an excellent cosmetic result.Conclusions:Immediate implant appears to be a safe and predictable procedure if performed with the right surgical protocol. The prosthetic protocol using the Socket Healing Guide technique with an immediate insertion of a screw-retained provisional restoration allows a healing guide of the peri-implant soft tissue to replace one single or multiple lost teeth. The excellent healing of the peri-implant soft tissues with this technique constitutes the seal and the prerequisite for the stability of the underlying hard tissues over time.Key Words: Immediate Implants, screw-retained provisional restoration, single and multiple unit restoration, guide of the soft tissues, periodontal clinical parameters.
