PU prevention consists of two interrelated steps: (1) risk assessment and (2) applying preventive interventions and measures based on the assumed PU risk level. Purpose: We conducted a review of research into interventions designed to identify effectiveness of the use of risk assessment scales for pressure ulcer prevention in clinical practice. Method: Search different internet data bases including: using “pubmed,” “science direct,” “highwire” and “scientific Google” database. Sample: 80 articles were found; the review utilized 12 articles which met the inclusion criteria. The incidence and prevalence rates of pressure ulcers are different from one hospital to another and from one country to another Studies included in this review focused on the Use of Pressure ulcer in term of assessment and prevention among adult patient in cancer. Result: Future studies should deal with different kinds of PU in order to be able to provide solid data for nursing practice within the vulnerable group of oncology patients. Furthermore, uniform interventions, assessment instruments, and designs should be used for collecting the results to enable comparability. Conclusion: There is no evidence that the use of risk assessment scales decreases pressure ulcer incidence. The Braden Scale offers the best balance between sensitivity and specificity and the best risk estimate. Recommendations: Complete an initial assessment of the individual with a pressure ulcer, to include the individual's and family's goals of care, Risk for developing additional pressure ulcers, Functional capacity, particularly in regard to positioning, posture and the need for assistive equipment and personnel and Pain related to pressure ulcers.

Book excerpt: Although some authoritative organizations have developed and promote evidenced-based guidelines on prevention and management of pressure ulcer, research has shown that non adherence to these guidelines has resulted to significant proportion of pressure ulcer among adults ages 18 and older in health care environment and in the community. The questions comes, Why the nonadherance? When health care professionals lack knowledge and skill in identifying high risk patients that makes it a problem. Recognition of high risk patient may be based on competency in assessment skill. Because prevention of pressure ulcer is an integral part of nursing, nurses have a role to play to achieve this goal therefore it is important for nurses to assess themselves in order to have a good perception of their knowledge about what they know, and understand hence nurses should embark on improving this skill. The uses of appropriate tools, initiating treatment based on establish protocols can prevent and reduce the prevalence of pressure ulcers and its complications. This will call for a change on how nurses perform initial assessment, document on daily basis especially on high risk patients and application of appropriate devices. Because patients and families can play a part in pressure ulcer prevention, nurses will have that responsibility in educating them on the preventive measures. The article discussed in this paper supports the fact that nurses have a greater role in prevention and management of pressure ulcer and improve the life of patients at risk. During the implementation stage, it will be necessary for nurses and other health care workers to apply evidence-based research for example, turning, cushioning the prominences, nutritional support to solve this problem. Nurses knowledge on proper documentation and application of devices will be evaluated from patients chart by using an approved audit tool and based, education intervention will be used as this will improve the quality of life of the patients as well as guide nurses in delivering of quality care. These interventions can also reduce length of stay, reduce health care cost as well as reduce burden to the patient's families.
