The real anesthetic challenge arises when patients present to Labor and Delivery with unusual or complicated medical problems and, in recent years, a few of the larger institutions have developed an Obstetric Anesthesiology Consultation Service to prepare for the management of these patients. While most pregnant women who present to Labor and Delivery require anesthetic intervention, they typically meet the anesthesiologist for the first time in labor. Since the majority of laboring women are healthy without significant comorbidities, this does not present much of a challenge to the anesthesiologist and the anesthetic management tends to be straight-forward with favorable outcomes. However, using this new model, the anesthesiologist has the opportunity to discuss the various treatment modalities and potentially suggest diagnostic testing to be performed prior to delivery, similar to the pre-operative testing that is done in other surgical environments.

The highest risk is associated with severe inherited thrombophilia which, if diagnosed, will often affect the duration of treatment in patients with venous thrombosis. High-risk thrombophilia is associated with antithrombin deficiency homozygosity for FV Leiden or prothrombin G20210A mutation simultaneous heterozygosity for several gene defects (double heterozygosity) protein C, and possibly protein S, deficiency. Solitary heterozygous FV Leiden or prothrombin mutation will increase the thrombotic risk only slightly. The association between inherited thrombophilia and arterial occlusion is less clearly demonstrated. In practice, arterial obstruction should only raise a suspicion of inherited thrombophilia if encountered in a young person with no known risk factors. Predisposition to thrombotic events may also be caused by acquired disturbances in the coagulation system, the most important of which is the presence of circulating phospholipid antibodies.

Book excerpt: BackgroundThrombophilia refers to an abnormality of blood coagulation that increases the risk of thrombosis. Abnormalities of any component of Virchowu2019s triad (circulatory stasis, vascular wall injury or hypercoagulable state) can lead to thrombosis. Most heritable thrombophilias are heterozygous defects. Hereditary thrombophilia arise as a result of over-activity of coagulation factors. The aim of this study was to perform a screening of pregnant women hospitalized in our hospital for hereditary thrombophilia. Material and methodsOur study group included 819 pregnant women. Genetic tests for specific mutations included methylenetetrahydrofolate reductase (MTHFR) gene mutation, factor V Leiden mutation, prothrombin 20210 and factor XIII mutation. Results The incidence of high-risk inherited thrombophilias is low in our study, so the frequency of homozygous factor V Leiden mutation (n=2) is equal with the incidence of homozygous prothrombin G20210A (n=2) mutation- 0.24%. The incidence of heterozygous factor V Leiden was 17,09 % (n=140) , heterozygous prothrombin G20210A mutation was 3.29% (n=27) , homozygosity for the C677T polymorphism of methylene tetrahydrofolate reductase mutations was 5% (n=41) , homozygosity for the A1298C polymorphism is present in 3.17% (n=26) of pregnant women included in our study. Also heterozygous methylene tetrahydrofolate reductase mutations (C677T, A1298C) were present at 48.35%.ConclusionsHereditary thrombophilia is a genetic condition of heterogeneous etiology, determined by genetic mutations that have been found in all factors of coagulation. There is no curative treatment for genetic thrombophilia.Pregnant women that presented risk factors for thrombosis and were diagnosed with high risk hereditary thrombophilia received prophylactic anticoagulant treatment.
