The case involved the death of Mavis Flanders due to a drug overdose, leaving behind an infant who survived. The report considers three questions: whether any Ministry staff member misled the Minister, based on the information they had on hand at the time they reported; what caused the Minister to be provided with incorrect information; and whether this was a case of suspect practice or information cover-up by staff. A chronology of events is set out and recommendations are made with regard to release of information, staff communication, information sharing, and integrated case management, attempting to balance the privacy needs of the persons involved with the public's right to know.
