A decade later, in 1938, it was observed that patients with rheu matoid arthritis exhibited a higher than normal serum copper concentration that returned to normal with remission of this disease. Thirteen years later, it was found that copper complexes were effective in treating arthritic dis eases. The first report that copper complexes had antiinflammatory activity in an animal model of inflammation appeared twenty-two years after the dis covery of essentiality. In 1976, it was suggested that the active forms of the antiarthritic drugs are their copper complexes formed in vivo. This sugges tion has been confirmed and extended in the interim in over 200 recent publications. Individual biomedical scientists from many countries who have published in these areas recently saw a need for a meeting to exchange current re supporting new hypotheses. We search results and discuss the evidence met on the University of Arkansas Medical Sciences campus in Little Rock, Arkansas, August 10-13, 1981. Participants came from Australia, England, France, Germany, Israel, Italy, The Netherlands, Norway, Poland, Scotland, Sweden, Switzerland, Wales, Zimbabwe, and over twenty of the United States.

Meantime also copper and zinc complexes receive continued attention for their potential anti-inflam matory actions. Since the previous major titles were published in this field some years ago (e. g. 1,2) it was considered timely to consider progress which has been made in the interceding period. Thus in this volume aspects are re viewed of the metabolism and biodisposition of copper and zinc, especially as they may be influenced by inflammatory processes, the mode of action of copper and zinc compounds in inflammatory states, and the actions of some newer copper complexes. While we still understand relatively little about how copper and zinc complexes work in inflammation, and indeed what the fate of the ligands and their complexed ions is in inflammation, it is hoped that this volume will be useful for giving a current view of the "state of art" in the field. In addition new data concerning antimicrobial, antiinflammatory, antiulcer, anticancer, anticarcinogenic, analgesic, and radioprotectant activities of copper complexes were presented. These activities are consistent with the notion that they represent the facilitation of normal copper-dependent metabolic processes in dis ease states. That physiological disabilities could be caused by a lack of exogenous substances which could be supplied by foods is a concept of relatively recent origins. It is not surprising, therefore, that, until the last few years, much of nutritional science research was tied to: 1) establishing a cause and effect relationship between a physiological problem and its cure/prevention by a chemical substance in food; 2) quantifying the amount of the substance (nutrient) needed to prevent deficiency symptoms; and 3) quantifying the amounts of nutrients found in various food substances. That a nutrient might be present in apparently adequate amounts in foods consumed by an individual but could not be fully utilized because of the concurrent consumption of anti-nutrients has been recognized as being an important problem as, for example, iodine-deficiency goiters resulting from consumption of gOitrigens. That less specific, less dramatic interactions among nutrients and among nutrients and other food components might enhance or inhibit the absorption of nutrients from the intestines or of the metabolism of nutrients within the body is an area of current concern.

Book excerpt: Endothelial activation with increased expression of cellular adhesion molecules, chemokines and pro-inflammatory cytokines critically contributes to vascular inflammation and the initiation and progression of atherosclerosis. By affecting redox-sensitive cell signaling pathways and transcription factors, redox-active transition metal ions, such as copper and iron, may play an important role in these processes. The goals of my studies were to investigate the role of copper in systemic and vascular inflammation, endothelial activation, and atherosclerotic lesion development, using tetrathiomolybdate (TTM), a clinically-used copper chelator, as a specific means to manipulate cellular copper levels in vitro and in vivo. Incubation of human aortic endothelial cells (HAEC) with copper induced activation of the redox-sensitive transcription factors, nuclear factor kappa B (NF-kB) and activator protein-1 (AP-1) and up-regulation of mRNA and protein levels of adhesion molecules and the chemokine, monocyte chemotactic protein-1 (MCP-1), in a dose- and time-dependent manner. Furthermore, TTM inhibited TNF[alpha]-induced increases of mRNA and protein levels of adhesion molecules and MCP-1 in a dose-dependent manner. TNF[alpha]-induced activation of NF-kB and AP-1 also was dose-dependently attenuated by TTM, and inhibition of NF-kB activity was associated with decreased phosphorylation and degradation of its cytosolic inhibitory subunit, IkBa. In an in vivo model of systemic inflammation using of C57BL mice, oral TTM administration for three weeks significantly reduced bioavailable copper and inhibited lipopolysaccharide (LPS)-induced upregulation of inflammatory gene expression in aorta and heart. Tetrathiomolybdate also significantly inhibited LPS-induced increases of serum levels of soluble adhesion molecules, MCP-1, and pro-inflammatory cytokines. Similar inhibitory effects of TTM were observed on NF-kB and AP-1 activation in the heart and lungs. In apolipoprotein E-deficient (apoE-/-) mice, a well established animal model of human atherosclerosis, dietary supplementation with TTM for ten weeks significantly reduced bioavailable copper and attenuated atherosclerotic lesion development, which was particularly pronounced in the descending aorta. This anti-atherogenic outcome of TTM supplementation was accompanied by several anti-inflammatory effects, such as significantly declined serum levels of soluble adhesion molecules, reduced aortic gene expression of inflammatory mediators, and less aortic macrophage accumulation. Importantly, TTM supplementation for ten weeks did not cause liver toxicity or anemia. In conclusion, my studies provide the proof of concept that copper plays an important role in systemic and vascular inflammation and the pathogenesis of atherosclerosis, and demonstrate that tetrathiomolybdate exerts anti-inflammatory and anti-atherogenic effects in preclinical animal models. The possible implications of these findings for cardiovascular and inflammatory diseases in humans remain to be investigated.

When we first considered the possible program for the meeting, we felt that a forum to integrate the recent advances in molecular understanding of copper transport with the older knowledge of copper metabolism was needed.
