Among these is a state of chronic inflammation that is endured by the brain during the aging process. The use of non-steroidal anti-inflammatory drugs (NSAIDs) decreases the amount of neuroinflammation sustained by the brain, and greater levels of NSAID use have been demonstrated to be associated with decreased probability of developing AD. This study looked at whether greater rates of NSAID use were also associated with decreased rates of cognitive and functional decline and survival in a population-based sample of persons with AD. Linear mixed models failed to find any association between NSAID use, duration of use, or timing of use (before or after AD onset) and cognitive and functional outcomes. Cox regression models did not find any association between any NSAID use, NSAID use before or after AD onset, or duration of NSAID use and participant survival. The conclusion of this project is that NSAIDs do not affect AD progression or survival rates of persons with AD. These results are discussed within the scope of the current literature.

Some two dozen clinical studies have strongly suggested that conventional anti-inflammatory drugs may be useful to delay the onset or slow the progression of the disorder. Moreover, virtually all the major systems of the innate immune response appear to be present, and most are upregulated, in pathologically-vulnerable regions of the AD brain. These new findings are described in this volume - first in overview form, followed by chapters on topics of special interest. In many ways, to understand AD brain inflammation, one need only review a text on peripheral inflammation biology, leaving out the chapters on humoral medi ators and substituting microglia for macrophages. In several other key respects, however, AD brain inflammation is unique, due primarily to idiosyncratic interac tions of inflammatory mediators and mechanisms with classical AD pathology: amyloid ~ peptide(A~) deposits and neurofibrillary tangles (NFTs). For this reason, some key concepts about the inflammation that occurs in AD may warrant discus sion in preparation for the more detailed chapters that follow.

A total of 17 epidemiologic studies have been performed in which non-steroidal anti-inflammatory drugs (NSAIDS) or corticosteroids were examined as a negative risk factor. The data from both case-control and population-based studies provide strong evidence that anti-inflammatory drugs may be beneficial to Alzheimer's patients.

Consequently, NSAID-induced increases in apoE protein may enhance apoE-mediated immunosuppression and compensatory synaptic plasticity, potentially resulting in decreased risk and delay of disease onset." --
