Book excerpt: Assuring public health services is primary duty of every government and as such, the government has taken steps to maintain public health, by opening health centers, hospitals, mobile hospitals, organizing mass awareness camps on health and so on. In this paper we will discuss Infant mortality rate is an excellent indicator of the socio-economic development of a country. India is facing severe problems related to the infant mortality. The statistics revealed that neonatal death rate is the highest in the world (43 per 1000 live births). A quarter of world's neonatal deaths (one million) each year take place in India, mostly at home (65.4% of all births and 75.3% of births in rural areas occur at home). It may be noted that despite the great importance of the subject, no information is available regarding the details of the causes of deaths. As discussed above, infant mortality is a major health problem and government is more concerned towards solving such health problem by reducing infant mortality rate. Further, the reasons for infant mortality include socio-cultural beliefs, education of mother, regular health check-up, lack of proper health care facilities, etc. Further paper will evaluate the child mortality patterns which do vary for the urban and the rural areas. The relation between the female and male mortality rates hold quite strongly in rural areas whereas in the urban areas these are weakly linked. It can be concluded that infant mortality is the result of socio-economic characteristics of mothers and households, demographic characteristics of children, and health-care behaviour of mothers, availability of health care facilities, etc. will be evaluated and concluding remarks and suggestions will be carried out in this paper.

Before 1960, there were substantial improvements in life expectancy, but fertility declines were very rare. Few people used modern contraceptives, and couples had large families. Since 1960, however, fertility rates have fallen in virtually every major geographic region of the world, for almost all political, social, and economic groups. What factors are responsible for the sharp decline in fertility? What role do child survival programs or family programs play in fertility declines? Casual observation suggests that a decline in infant and child mortality is the most important cause, but there is surprisingly little hard evidence for this conclusion. The papers in this volume explore the theoretical, methodological, and empirical dimensions of the fertility-mortality relationship. It includes several detailed case studies based on contemporary data from developing countries and on historical data from Europe and the United States.

Book excerpt: Water, sanitation and hygiene (WASH) investments are widely seen as essential for improving health in early childhood. However, the experimental literature on WASH interventions identifies inconsistent impacts on child health outcomes, with relatively robust impacts on diarrhea and other symptoms of infection, but weak and varying impacts on child nutrition. In contrast, observational research exploiting cross-sectional variation in water and sanitation access is much more sanguine, finding strong associations with diarrhea prevalence, mortality and stunting. In practice, both literatures suffer from significant methodological limitations. Experimental WASH evaluations are often subject to poor compliance, rural bias, and short duration of exposure, while cross-sectional observational evidence may be highly vulnerable to omitted variables bias. To overcome some of the limitations of both literatures, we construct a panel of 442 subnational regions in 59 countries with multiple Demographic Health Surveys. This large subnational panel is used to implement difference-in-difference regressions that allow us to examine whether longer term changes in water and sanitation at the subnational level predict improvements in child morbidity, mortality and nutrition. We find results that are partially consistent with both literatures. Improved water access is statistically insignificantly associated with most outcomes, although water piped into the dwelling predicts reductions in child stunting. Improvements in sanitation predict large reductions in diarrhea prevalence and child mortality, but are not associated with changes in stunting or wasting. We estimate that sanitation improvements can account for just under 10% of the decline in child mortality from 1990-2015.

This study also recommends capacity building, priority research, and institutional and global efforts to reduce the incidence and impact of birth defects in developing countries.

Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: The level of infant mortality has been regarded as an explicit indicator to measure the extent of health problems associated with a population as well as its socio-economic development. Since the Indian states are quite diverse in the social, economic, cultural geographical and environmental conditions, they also differ in the pace of mortality transition. Some states have very less infant, neonatal and postnatal mortality like Kerala and while some has very high infant, neonatal and postnatal mortality like Madhya Pradesh. Although the overall infant mortality rate of India is declining, there is some retardation with the pace of decline of infant mortality rate. The present levels of infant and child mortality are quite high and need immediate attention. Consequently, for the policy makers, this remains a difficult task to tackle. National family health survey-1992-93, 1998-99 and 2005-06 data are used for the analysis. The result shows that there is clustering of infant deaths in the major Indian states and it is not uniformly distributed with different socio-economic characteristics. Infant death clustering increased significantly over the time.
