Book excerpt: The Honduras AIN-C (Atencion Integral a la Ninez-Comunitaria) Program is a preventive health and nutrition program of the Honduras Ministry of Health that relies on volunteers to help mothers and communities pro-actively monitor and maintain the adequate growth of young children. A 2000 experimental-design based evaluation found that the program achieved near-universal coverage and was effective in improving mothers' child-rearing knowledge, attitudes and practices, including feeding practices and appropriate care-giving and care-seeking practices for children with diarrhea and acute respiratory illness. The program is widely regarded as a model. This study was undertaken to provide the first comprehensive estimates of the cost of AIN-C, with the goal of providing a program and financial planning tool for Honduras, as well as other countries interested in implementing an AIN-C Program. Expressed in mid-2002 U.S. dollars, the study found: (1) the long-term, annual, recurrent cost per child less than two participating in the program is US$6.82, (2) the long-term, annual, incremental budget requirements per child less than two participating in the program is $4.00, (3) the long-term, annual recurrent cost per capita is $0.44, and the cost of an AIN-C monthly growth monitoring and counseling session per child is 11 percent of the cost of a traditional MOH, facility-based growth and development consultation per child.

Book excerpt: Background: In Honduras, over 95% of mothers initiate breastfeeding but less than a third meet the World Health Organization recommendation to exclusively breastfeed through 6 months. Understanding the breastfeeding knowledge, attitudes, prior exposure and future intent of young adults may provide significant insight into the relatively low exclusivity rates reported in the Honduran population and may be important targets for future interventions in this population. To the authors' knowledge, this is the first study to explore these concepts among young adults in Honduras. Objective: The objective of this study was to describe, identify, and explore any relationships between demographics, breastfeeding knowledge, attitudes, prior exposure, and future breastfeeding intent in a sample of undergraduate students in Honduras. Methods: This was an exploratory, cross-sectional study, conducted with a convenience sample of 283 undergraduate students attending a major public university in Honduras in February 2015. Results: Honduran undergraduate students have moderate breastfeeding knowledge, neutral breastfeeding attitudes, high prior breastfeeding exposure, and high intention to breastfeed or support their partners to breastfeed in the future. Knowledge on the health benefits of breastfeeding was low. Less than a third of students knew that women who have breastfed have a lower risk of breast cancer (28.8%) and only 47.9% knew that breastfeeding helps prevent respiratory infections in the infant. Slightly over half of students considered breastfeeding in public to be acceptable (53.4%) and less than a third reported that seeing a woman breastfeeding in public would make them uncomfortable (28.2%). The develop ment of the digestive enzymes is detailed in Chapter 4. The significance of this immaturity first emerged as a consequence of the pioneering work of Professor Robert McCance and Dr Elsie Widdowson in human nutrition, when they studied the electrolyte and nitrogen excretion of babies and young animals. To quote Professor McCance, 'Inefficient though the kidneys were by adult standards, they were capable ofmaintaining homeo stasis, provided the infant and animals were growing while being fed on food of exactly the right composition - that is, the milk of the mother. ' (Ashwell, 1993). One should not forget that although the mother protects the developing baby against much nutritional abuse, the baby may be still be affected by matemal nutrition, and this is discussed in Chapter 2. The superior qualities of breast milk are still recognized, and research continues to discover yet more factors in breast milk significant to the health of the baby for possible inclusion in formulas. The immature stage of development ofthe baby means that while enough nutrients for optimal growth of all tissues are required, excessive quantities may cause intoxica tion. Tight specifications are essential, since unlike the adult the newbom depends on a single food.
