Book excerpt: En 2001, l'OMS avait émis des directives pour l'alimentation de complément (AC), et identifié le manque d'indicateurs comme un des facteurs freinant son amélioration. Divers indices et indicateurs ont été proposés mais des questions demeurent, notamment sur les hypothèses sur lesquelles ils reposent. Ce travail visait à y apporter des réponses. Une étude a été menée chez des enfants de 6-23 mois à Madagascar. Un indice composite de l'alimentation (ICFI) et plusieurs scores de diversité alimentaire ont été construits, et leur association avec la qualité de l'alimentation et la croissance des enfants ont été étudiées. L'ICFI est associé à l'ingéré énergétique et à la densité de l'alimentation en micronutriments. Il est stable au cours du temps, et est associé à la croissance lorsqu'il est construit à partir de mesures répétées, ou lorsque sa composante 'allaitement' est exclue. La diversité alimentaire est également un bon prédicteur de la densité nutritionnelle. L'ICFI doit être adapté à ses perspectives d'application. Des scores de diversité alimentaire sont utiles pour identifier les enfants ayant une alimentation de faible qualité nutritionnelle.

Il fait suite au document de 2008 « Indicateurs pour l'évaluation des pratiques d'alimentation du nourrisson et du jeune enfant – Parties I et II ». Au total, il y a 17 indicateurs ANJE recommandés dans l’édition 2021. Sept sont nouveaux et quatre des indicateurs de 2008 ont été exclus de la liste 2021 des indicateurs de l'ANJE. De plus, trois indicateurs de consommation d’aliments et de boissons malsains sont inclus. Contrairement à 2008, aucune distinction n'est faite entre les indicateurs de base et les indicateurs facultatifs dans cet ensemble de recommandations. Il est important d’évaluer les données à l’aide de l’ensemble complet d’indicateurs pour une population donnée et de rendre compte de tous les résultats. Ce document d’orientation fournit des outils pour la collecte et le calcul des indicateurs. Il est destiné à être utilisé par les gestionnaires d'enquêtes démographiques à grande échelle qui recueilleront des informations sur l'état des pratiques alimentaires chez les nourrissons et les jeunes enfants de moins de 2 ans. Les indicateurs peuvent être évalués au moyen d’enquêtes démographiques à grande échelle, notamment l’Enquête démographique et de santé (EDS) et l’Enquête par grappes à indicateurs multiples (MICS).
