La réhabilitation par prothèse de l'enfant édenté semble donc indispensable pour rétablir ses fonctions, son esthétique et sa psychologie. La prothèse amovible pédiatrique vise à remplacer les dents absentes ou perdues. Elle reste encore peu courante dans nos cabinets dentaires. La réhabilitation doitêtre précoce afin de permettre un développement physiologique, psychologique et relationnel adéquat. Les étiologies possibles des pathologies nécessitant une thérapeutique par prothèse amovible pédiatrique sont variées : perte prématurée suite à un traumatisme, avulsions précoces dues à des lésions carieuses, agénésies, anomalies de structure dentaire (MIH, amélogénèse ou dentinogénèse imparfaite). Dans la littérature, très peu d'articles recensent les indications et contre-indications de la prothèse amovible pédiatrique. L'objectif de ce travail est ainsi de réaliser une synthèse de l'ensemble des indications et contre-indications de la prothèse amovible pédiatrique dans la littérature afin de proposer des recommandations en pratique clinique. Différents moteurs de recherches ont été explorés. La méthode de référence consistait en l'élaboration d'équations de recherche. Un filtre sur 20 ans ne ciblant que les articles en français et anglais, a été appliqué. Il a été décidé de ne retenir que les articles qui traitaient des indications et contre-indications de la prothèse amovible pédiatrique. Les rapports de cas ont été écartés. Les articles obtenus ont été classés sous forme de tableau. Les données obtenues dans les articles ont été traitées de manière qualitative. La recherche a permis d'identifier un total de 1148 articles, toutes bases de données confondues. Parmi ceux-ci, 20 articles répondaient aux critères de sélection et ont été inclus. Un tableau résumant les résultats des études retenues a été réalisé. La prothèse amovible pédiatrique est indiquée pour l'enfant jeune à partir de 2-3 ans et coopérant, l'enfant possédant des agénésies dentaires +/- associées à la DE, l'enfant ayant subi des avulsions précoces pour raisons carieuses ou traumatiques. Les contre-indications à la prothèse pédiatrique sont quant à elles peu nombreuses. Elles sont essentiellement liées à la coopération de l'enfant lors des étapes de prothèses, l'acceptation de la thérapeutique prothétique par l'enfant et ses parents, leur motivation, le respect du suivi régulier, des manœuvres d'hygiène bucco-dentaire et des conseils de nutrition.

De même, les anomalies de forme et de structure dentaires, les polycaries et les traumatismes peuvent mener à des avulsions dentaires multiples. La prothèse amovible pédiatrique, généralement de conception simple et rapide, permet de rétablir les différentes fonctions orales tout en réduisant l'impact esthétique et psychologique de l'édentement pour une meilleure intégration sociale de l'enfant. Cette thérapeutique implique une bonne connaissance de la physiologie dentaire et osseuse, afin de ne pas faire obstacle à l'éruption de la dent permanente sous jacente, ni entraver la croissance des maxillaires. Le protocole opératoire est sensiblement le même que chez l'adulte, bien qu'il faille l'adapter à cette jeune population (empreinte optique, gestion comportementale, optimisation des techniques). En outre, l'adhésion des parents à la thérapeutique et un suivi régulier et pluridisciplinaire sont indispensables pour adapter l'appareil à la croissance de l'enfant (prothèse évolutive). Concernant l'instabilité et le manque de rétention des prothèses mandibulaires complètes ou sub-totales, des implants pourront être posés précocement dans la région symphysaire.
