Book Feeding Back Learning from Complaints Handling in Health and Social Care

Download or read book Feeding Back Learning from Complaints Handling in Health and Social Care written by Great Britain. National Audit Office and published by The Stationery Office. This book was released on 2008 with total page 56 pages. Available in PDF, EPUB and Kindle. Book excerpt: There are currently two separate statutory processes for handling complaints about health and social care services. NHS organisations are accountable to the Department of Health and social care services are accountable through their local authority, whilst adult social care rests with the Department. There are differences in the numbers of stages and timescales involved, and in the arrangements for advocacy support and independent investigation. The Health Service Ombudsman is responsible for the ultimate review and decision on NHS complaints and the Local Government Ombudsman for social care complaints. The NAO is this report (HCP 853, session 2007-08), has undertaken an evaluation of existing performance, capability, capacity and costs of complaints handling in both health and adult social care. The NAO has set out a number of findings and recommendations, including: that where people are dissatisfied, there is a low number who make formal complaints; that navigating the complaints systems is not straightforward, partcularly for health service users; only a small proportion of NHS complainants are aware, or receive national advocacy support; that the culture and attitudes of the organisations are often a barrier to responsive complaint handling; neither the health or social care organisations know the cost of complaints handling; that pursuing a complaint requires considerable time, determination and resilience.

Book excerpt: This report comes in the middle of the biggest overhaul to the NHS in over 60 years due changes brought about by the Health and Social Care Act 2012. As the changes in the NHS take place, the Ombudsman's Office caseload suggests that embedding good complaint handling will be essential to avoid the risk of patient complaints going unheard. Last year the Ombudsman received 50% more complaints from people who felt that the NHS had not acknowledged mistakes in care. 16, 333 complaints were resolved. This report, as well as providing statistics and case studies, outlines the learning from the casework in 2011-12. It also suggests how the NHS can improve it's complain handling and sets out ways in which the Ombudsman's own work is change to enable to us to share more information more widely.

Book Report of the Mid Staffordshire NHS Foundation Trust Public Inquiry

Download or read book Report of the Mid Staffordshire NHS Foundation Trust Public Inquiry written by Mid Staffordshire NHS Foundation Trust Public Inquiry and published by The Stationery Office. This book was released on 2013-02-06 with total page 126 pages. Available in PDF, EPUB and Kindle. Book excerpt: This public inquiry report into serious failings in healthcare that took place at the Mid Staffordshire NHS Foundation Trust builds on the first independent report published in February 2010 (ISBN 9780102964394). It further examines the suffering of patients caused by failures by the Trust: there was a failure to listen to its patients and staff or ensure correction of deficiencies. There was also a failure to tackle the insidious negative culture involving poor standards and a disengagement from managerial and leadership responsibilities. These failures are in part a consequence of allowing a focus on reaching national access targets, achieving financial balance and seeking foundation trust status at the cost of delivering acceptable care standards. Further, the checks and balances that operate within the NHS system should have prevented the serious systemic failure that developed at Mid Staffs. The system failed in its primary duty to protect patients and maintain confidence in the healthcare system. This report identifies numerous warning signs that could and should have alerted the system to problems developing at the Trust. It also sets out 290 recommendations grouped around: (i) putting the patient first; (ii) developing a set of fundamental standards, easily understood and accepted by patients; (iii) providing professionally endorsed and evidence-based means of compliance of standards that are understood and adopted by staff; (iv) ensuring openness, transparency and candour throughout system; (v) policing of these standards by the healthcare regulator; (vi) making all those who provide care for patients , properly accountable; (vii) enhancing recruitment, education, training and support of all key contributors to the provision of healthcare; (viii) developing and sharing ever improving means of measuring and understanding the performance of individual professionals, teams, units and provider organisations for the patients, the public, and other stakeholders.

Download or read book Disentitlement written by Timothy S. Jost and published by Oxford University Press, USA. This book was released on 2003 with total page 306 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book is about health care entitlements in the US-and about the threat of disentitlement. It describes the history and legal character of our Medicare and Medicaid programs, and of the tax subsidies that have brought health insurance to most working Americans. It examines the thread that our entitlements face from privatization, individualism, and devolution. It considers the models that other countries have developed for health care entitlements and what we can learn from them. The book concludes by proposing a redesigned entitlement-based health care system for the future.

Download or read book Listening and learning written by Great Britain: Parliamentary and Health Service Ombudsman and published by The Stationery Office. This book was released on 2011-10-18 with total page 88 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Health Ombudsman resolved a total of 15,186 complaints about the NHS in England in 2010-11. This report shows how, at a local level, the NHS is still not dealing adequately with the most straightforward matters. As the case studies illustrate, minor disputes over unanswered telephones or mix-ups over appointments can end up with the Ombudsman because of knee-jerk responses by NHS staff and poor complaint handling. While these matters may seem insignificant alongside complex clinical judgments and treatment, they contribute to a patient's overall experience of NHS care. The escalation of such small, everyday incidents represents a hidden cost, adding to the burden on clinical practitioners and taking up time for health service managers, while causing added difficulty for people struggling with illness or caring responsibilities. Two particular themes stand out this year. Poor communication - one of the most common reasons for complaints in the last year - can have a serious, direct impact on patients' care and can unnecessarily exclude their families from a full awareness of the patient's condition or prognosis. Secondly, in a small but increasing number of cases, a failure to resolve disagreements between patients and their GP has led to their removal from the GP's patient list - often without the required warning or the opportunity for both sides to talk about what happened. As GPs prepare to take on greater responsibility for commissioning patient services, this report provides an early warning that some are failing to handle even the most basic complaints appropriately.

Book Health Policy and the NHS

Download or read book Health Policy and the NHS written by Judith Allsop and published by Routledge. This book was released on 2018-10-08 with total page 384 pages. Available in PDF, EPUB and Kindle. Book excerpt: Health Policy and the NHS provides a thorough and up-to-date review of the changes in the structure and organisation of the health service. It focuses on how sucessive governments have approached problems of health care, their policy assumptions and the economic and political context of their decision making. Divided into four parts the text considers in turn: the foundations and framework of the NHS, policy issues within the NHS that dominated the government's policy agenda until the late 1980s, health and society and the critiques of health policy which developed in the late 1970s and 1980s, and new directions for health policy in the future.

Book No Fault Approaches in the NHS

Download or read book No Fault Approaches in the NHS written by Sonia Macleod and published by Bloomsbury Publishing. This book was released on 2023-01-12 with total page 321 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book explores how concerns can be raised about the NHS, why raising concerns hasn't always improved standards, and how a no-fault open culture approach could drive improvements. The book describes a wide range of mechanisms for raising concerns about the NHS, including complaints, the ombudsman, litigation, HSIB, and the major inquiries since 2000, across the various UK jurisdictions. The NHS approach is contextualised within the broader societal developments in dispute resolution, accountability, and regulation. The authors take a holistic view, and outline practical solutions for reforming how the NHS responds to problems. These should improve the situation for those raising concerns and for those working within the NHS, as well as providing cost savings. The no-fault approaches proposed in the book provide long-term sustainable solutions to systemic problems, which are particularly timely given the impact of the COVID-19 pandemic on the NHS. The book will be of interest to academics, researchers, ADR practitioners, practising lawyers, and policy makers.
