Book excerpt: "Lesbian, gay, bisexual, and transgender (LGBT) patients experience significant health disparities that affect morbidity, mortality, and overall quality of life. Nurses who express negative biases in the delivery of care influence many of the disparities facing this vulnerable patient population. Educational interventions that are focused on increasing nurses' knowledge of LGBT healthcare needs have proven effective in improving patient compliance and enhancing patient care outcomes. This evidence-based practice change project was designed to educate nurses (n=25) on the healthcare needs of the LGBT population. The project included a pretest and posttest with an educational intervention. The project manager, in collaboration with subject matter experts, developed a questionnaire, which consisted of 15 knowledge and five attitude questions. The questionnaire was administered before and after an educational intervention to measure any changes in nurses' knowledge and attitudes toward LGBT patients. Findings indicated that knowledge scores increased from a baseline of 69.1%, which was below the 80% benchmark, to a post intervention score of 97.6%, which was above the intended benchmark. The average mean scores measuring attitudes increased significantly, t = 2.06 and p = .04. The findings of this evidence-based practice change project have significant implications on the effects of educating nurses on the healthcare needs of the LGBT population, that dramatically influence practice and patient care outcomes. Book excerpt: The focus of this capstone project is to explore the impact of registered nurses' bias and lack of knowledge associated with the care of the LGBT patient. A Quasiexperimental design was used to evaluate the cause-and-effect relationship of an educational intervention. This intervention provided LGBT cultural knowledge and provides evidence regarding how homophobia and transphobia among nurses creates and perpetuates disparities among LGBT people. Attitude and knowledge assessment tools were used to collect responses from participant's pre and post intervention. Using parametric and descriptive statistics, the participants' data were analyzed. There were no statistically significant differences between the pre and post intervention scores. Although statistical significance was lacking, clinical significance was inferred by the participants' knowledge gap in their posed questions at the conclusion of the educational intervention. The implementation of similar training sessions, offered in a recurring fashion, will likely be necessary to effectively decrease the healthcare disparities currently being experienced by the LGBT population.

The Health of Lesbian, Gay, Bisexual, and Transgender People assesses the state of science on the health status of LGBT populations, identifies research gaps and opportunities, and outlines a research agenda for the National Institute of Health. People identifying as lesbian, gay, bisexual, transgender, and queer (LGBTQ+) have increased over the past few years, with continued growth expected. Consequently, nursing faculty need the necessary knowledge, skills, and attitudes to teach nursing students to provide quality nursing care to LGTBQ+ patients who are less likely to receive equitable health care because they are members of a diverse population. Therefore, addressing their healthcare needs in the nursing curriculum is essential in alleviating healthcare barriers and biases. Methodology: The equity, diversity, and inclusion (EDI) guidelines were presented to the nursing faculty, applied by simulation course chairs in clinical simulations, and used to review the associate degree nursing (ADN) program student application to encourage qualified applicant diversity. Qualtrics quantitative pre- and post-survey questions were offered to nursing faculty to assess faculty comfort in teaching nursing students how to support LGBTQ+ healthcare needs and the importance of including EDI in the nursing curriculum. In addition, the simulation course chairs and ADN director were asked verbal qualitative survey question(s). Results: Overall, nursing faculty were receptive to increasing EDI in the curriculum, simulation, and ADN application. Implications for practice: Utilizing the Nursing EDI Guidelines for the LGBTQ+ Population increase the faculty awareness and address healthcare disparities to bridge a nursing education gap. The guidelines are a resource accessible online, aiding nursing faculty in incorporating EDI into the nursing curriculum.

LGBT cultural competence training is recommended for integration in all health professional curricula. Improving knowledge is linked to aid in eliminating LGBT health disparities. A gap exists between recommendation and the practice of educating on LGBT cultural competence. The purpose of this evidence-based change project is to initiate LGBT cultural competence education in a nursing course aimed to improve knowledge among nursing students. Methods: Last-term nursing students (n=10) participated in a one-group pretest-posttest educational intervention including simulation. The guiding framework was Campinha-Bacote's Process of Cultural Competence in the Delivery of Healthcare Services (2007) and FIRST2ACT simulation model. Instruments used for data collection included The LGBT Cultural Competency Questionnaire, simulation reflections, and program evaluations. Results: Results from the pre/posttest LGBT questionnaire revealed a positive improvement between the mean pretest 7.8 (48.8%) and mean posttest 15.4 (96.3%) scores with an overall group mean difference between pre and post means of 7.6 (97.4%). Conclusion: Students' knowledge of LGBT cultural competency improved post-educational intervention, indicating that the teaching methods used had a positive impact and further investigation is needed to assess how the intervention, when applied on a larger scale across other institutions, can improve nursing and other health professional knowledge of LGBT cultural competence. Each of these populations has their own distinct and unique health care needs. Transgender persons may present their gender differently than what is considered the societal norm. As a result, they are more prone to discrimination, harassment and acts of violence than other marginalized populations. Prior experiences with health care professionals may cause transgender persons to avoid seeking health care when needed. As more persons identify as transgender, the need to educate health care providers on the appropriate and sensitive care of this population takes on greater importance. In an effort to address the disparities of sexual and gender minorities, learning to care for this population should begin early on in nursing education. However, if nurse educators are not familiar with or have bias towards this population, it is unlikely that appropriate content to address the specific needs of this population will be included in course content. Educators may not be well versed on this topic owing to the lack of inclusion in the literature which may be a barrier. Nurses must recognize the variances that exist among diverse populations in order to effectively treat and affect positive health care outcomes. The purpose of this qualitative descriptive study was to explore the attitudes, perceptions, knowledge and biases of nursing faculty regarding care for transgender persons and inclusion in nursing courses. The participants were nursing faculty who were currently teaching part-time, full-time or adjunct in baccalaureate programs. This study was conducted to determine what factors influence inclusion or exclusion of this content in nursing education.
