Book excerpt: Immunization against disease is among the most successful global health efforts of the modern era, and substantial gains in vaccination coverage rates have been achieved worldwide. However, that progress has stagnated in recent years, leaving an estimated 20 million children worldwide either undervaccinated or completely unvaccinated. The determinants of vaccination uptake are complex, mutable, and context specific. A primary driver is vaccine hesitancy - defined as a "delay in acceptance or refusal of vaccines despite availability of vaccination services". The majority of vaccine-hesitant people fall somewhere on a spectrum from vaccine acceptance to vaccine denial. Vaccine uptake is also hampered by socioeconomic or structural barriers to access. On August 17-20, 2020, the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 4-day virtual workshop titled The Critical Public Health Value of Vaccines: Tackling Issues of Access and Hesitancy. The workshop focused on two main areas (vaccine access and vaccine confidence) and gave particular consideration to health systems, research opportunities, communication strategies, and policies that could be considered to address access, perception, attitudes, and behaviors toward vaccination. This publication summarizes the presentations and discussion of the workshop.

Past research found that such resources are not always equitably utilized by communities, where oftentimes communities with high social vulnerability receive fewer resources. COVID-19 was one of the largest and most widespread public health emergencies. In response to the emergency, the United States (U.S.) government sponsored the creation and administration of COVID-19 vaccines. COVID-19 vaccines reduce the probability of severe illness and death, making them an important resource for community resilience. This study uses an explanatory sequential mixed methods research design to examine three research questions related to social equity in vaccine administration: (1) What is the relationship between community social vulnerability and COVID-19 vaccine administration?; (2) Did individuals trying to access the COVID-19 vaccine encounter administrative burdens?; and (3) How do the administrative burdens experienced by individuals when trying to access the COVID-19 vaccine provide a better understanding of the relationship between social vulnerability and COVID-19 vaccine administration? County level data for all U.S. counties were analyzed to examine the first research question. Findings indicate that there was an association between counties with higher wealth-related social vulnerability and lower county vaccination rates, but counties with higher employment-related and ethnicity-related social vulnerability were associated with higher vaccination rates. Qualitative interview data from 31 individuals revealed that few individuals faced administrative burdens when trying to access the COVID-19 vaccines, but a variety of resources and support services were used to access the vaccines. However, not everyone had equal access to resources, as individuals indicated that resources required wealth for access, and many resources were provided by employers. In addition, results revealed that ethnicity often presented psychological barriers to getting vaccinated. These results suggest that the resources invested in vaccination efforts materialized for some, but not all types of vulnerability. Emergency managers and policymakers should consider these results when providing resources meant to enhance community resilience following future emergencies and crises.

Book excerpt: The Equity-First Vaccination Initiative (EVI) aims to reduce racial disparities in coronavirus disease 2019 (COVID-19) vaccination rates in the United States and, over the longer term, to strengthen the public health system to achieve more-equitable outcomes. To accomplish these goals, The Rockefeller Foundation has committed $20 million over one year to fund anchor partners in five major cities- the Open Society Institute-Baltimore in Baltimore, Maryland; the Chicago Community Trust in Chicago, Illinois; Houston in Action in Houston, Texas; United Way of Greater Newark in Newark, New Jersey; and Roots Community Health Center in Oakland, California-to plan and implement hyper-local, place-based strategies to increase vaccine confidence and access for communities that identify as Black, Indigenous, and people of color. This interim report introduces the initiative and the anchor partners in each of the five demonstration sites, highlights the initial work of selected community-based organizations (CBOs) to which the anchor partners are making subgrants, synthesizes lessons learned across the EVI in its first three months, and suggests policy actions for decisionmakers to consider as they seek to support hyper-local, community-driven efforts to reduce inequities in COVID-19 vaccination. In just the first few months of the EVI initiative, the CBOs in the five demonstration sites held nearly 1,200 vaccine-related events, provided assistance more than 42,000 times to get people vaccinated (e.g., transportation, registration), made almost 2 million connections with community members through campaigns and information sessions, and administered almost 16,000 COVID-19 vaccinations.

Book excerpt: Urgent steps are needed to arrest the rising human toll and economic strain from the COVID-19 pandemic that are exacerbating already-diverging recoveries. Pandemic policy is also economic policy as there is no durable end to the economic crisis without an end to the health crisis. Building on existing initiatives, this paper proposes pragmatic actions at the national and multilateral level to expeditiously defeat the pandemic. The proposal targets: (1) vaccinating at least 40 percent of the population in all countries by the end of 2021 and at least 60 percent by the first half of 2022, (2) tracking and insuring against downside risks, and (3) ensuring widespread testing and tracing, maintaining adequate stocks of therapeutics, and enforcing public health measures in places where vaccine coverage is low. The benefits of such measures at about $9 trillion far outweigh the costs which are estimated to be around $50 billion—of which $35 billion should be paid by grants from donors and the residual by national governments potentially with the support of concessional financing from bilateral and multilateral agencies. The grant funding gap identified by the Access to COVID-19 Tools (ACT) Accelerator amounts to about $22 billion, which the G20 recognizes as important to address. This leaves an estimated $13 billion in additional grant contributions needed to finance our proposal. Importantly, the strategy requires global cooperation to secure upfront financing, upfront vaccine donations, and at-risk investment to insure against downside risks for the world.

Book excerpt: This paper empirically examines the economic effects of COVID-19 vaccine rollouts using a cross-country daily database of vaccinations and high frequency indicators of economic activity—nitrogen dioxide (NO2) emissions, carbon monoxide (CO) emissions, and Google mobility indices—for a sample of 46 countries over the period December 16, 2020 to June 20, 2021. Using surprises in vaccines administered, we find that an unexpected increase in vaccination per capita is associated with a significant increase in economic activity. We also find evidence for non-linear effects of vaccines, with the marginal economic benefits being larger when vaccination rates are higher. Country-specific conditions play an important role, with lower economic gains if strict containment measures are in place or if the country is experiencing a severe outbreak. Finally, the results provide evidence of spillovers across borders, highlighting the importance of equitable access to vaccines across nations.
