Book excerpt: Background: Due to the high potential of transferring infectious diseases and/or organisms among patients, themselves, and the community, healthcare workers (HCWs) must be knowledgeable and confident in selecting the appropriate type of personal protective equipment (PPE), and the use in technique when putting on (donning) and removing (doffing) PPE based on the level of isolation precautions required for the patient being cared for. Project Purpose: The purpose of this project is to determine whether assessing the knowledge and actual practice with observing, and utilizing an innovative approach of video and educational tools to isolation precautions would improve the consistency of compliance with PPE selection and use among registered nurses (RNs) and nursing assistants (NAs) on a medical-surgical unit. Methodology: The theoretical framework incorporated in the study was Schon's Theory of Reflective Practice. A pre-intervention questionnaire was distributed, and pre- and post-intervention observations were conducted to evaluate PPE selection and use by RNs and NAs. Results: The pre-intervention data from both the knowledge questionnaire and the observations showed various inconsistencies in RNs' and NAs' PPE technique and selection choice. A comparison of the pre- and post-intervention observation data showed that there was significant increase in all four analyzed categories--hand hygiene (45% to 70%), selection of PPE (79% to 80%), sequence of putting on PPE (70% to 85%), and sequence of removing PPE (76% to 85%). Recommendations: Future research studies should plan for a longer period of time to assess and collect pre- and post-intervention data, and include a larger sample. An expanded research project should also examine the correlation between HCWs' compliance rates with PPE and isolation precautions, and the incidence of healthcare-associated infections (HAIs). Book excerpt: Research demonstrates that there is a high rate of non-compliance with regard to the usage of Personal Protective Equipment (PPE) in the healthcare setting. For example, a study undertaken in a U.S. Emergency Room (ER) observed 88 ER personnel via a video camera. During the study 304 procedures were observed. In spite of the high risk environment, 22% of the procedures were performed without any or inadequate eye protection and 32.2% were completed without a mask. In a separate peri-operative study in the U.S. that focused on Post Anesthesia Care Unit (PACU) Nurses the study revealed that only 31.3% of the nurses would use PPE for low-risk patients and 68.8% for high-risk patients and only 75% of the nurses would consent to wear PPE if they were actually anticipating contact with blood. 83.3% conceded that they would use goggles if they knew for sure that the patient was HIV positive (Neo, Edward, and Mills, 2012). These statistics are ubiquitous across the healthcare system in the United States. Studies have been performed comparing Personal Protective Equipment on-line didactic only training to on-line didactic training with an added interactive training component. In one particular study the resultant post-training testing of the concepts that included an interactive component demonstrated a far better retention rate at one month post training when students were asked to perform an unannounced demonstration (Williams and Carnahan, 2013). Healthcare Acquired Infections (HAIs) are prevalent in the United States and according to the Centers for Disease Control 1 in 25 patients on average, per day, acquired a hospital-based infection in 2011. This statistic adds up to 722,000 HAIs annually of which approximately 75,000 of those patients lose their lives to the infection or the sequelae of the process (""CDC HAIs: Data & Statistics,"" 2014). Non-compliance with the use of Personal Protective Equipment can put the healthcare worker at risk of exposure to infectious agents as well as put the patient at risk of contracting an infection. This research study proposal is being conducted to determine if adding an interactive training component to an existing annual on-line didactic only training module will improve compliance with the use of Personal Protective Equipment. The research process will involve an unannounced baseline testing of current knowledge retention of a current on-line only didactic training module, followed up by a new interactive training session with the final step will being an unannounced third testing session approximately one to two months post interactive instructional sessions. The proposed research project timeframe is expected to last 6 months.

Book excerpt: The outbreak of severe acute respiratory syndrome (SARS) was notable for its transmission within healthcare facilities, and in particular for infecting healthcare workers (HCWs) who provided direct patient care. This paper focuses on assessing the risk factors related to HCWs acquiring SARS and the effectiveness of infection control measures in protecting HCWs from disease transmission while caring for SARS infected patients. Risk factors associated with SARS transmission include contact with respiratory secretions, exposure to aerosol generating procedures, duration of exposure to SARS patients, duration of infection control training, and perceived inadequacy of personal protective equipment supply. Protective factors associated with SARS transmission include wearing of a mask and hand hygiene. In order to improve hospital preparedness for possible future disease outbreaks, efforts in enhancing infection control training must be a priority. Standardizing and improving the quality of research conducted in the face of a disease outbreak is another area deserving of attention.

The original Blue Book was a comprehensive publication used widely in health-care centers and government agencies to assist in the adoption of national guidance. It also provided support to committed medical directors and managers to make improvements and presented practical information on waste-management techniques for medical staff and waste workers. It has been more than ten years since the first edition of the Blue Book. During the intervening period, the requirements on generators of health-care wastes have evolved and new methods have become available. Consequently, WHO recognized that it was an appropriate time to update the original text. The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. This is no longer the situation.
