La nature et la qualité de la communication dépendent de la façon dont les parents et les jeunes enfants interagissent. Nous situons notre approche dans le champ de l'analyse systémique. Les sujets de cette étude sont constitués de 9 dyades mère-enfant réparties selon trois groupes : 3 enfants "standards", 3 enfants "déficients visuels", 3 enfants "déficients visuels avec un handicap associé". La mise en oeuvre d'une situation d'observation permet de distinguer des comportements chez l'adulte et chez l'enfant. Le même protocole est suivi par une personne extérieure à la famillet et les pères des trois enfants "déficients visuels" participent aussi à la recherche. Grâce au logiciel DynamicInterCoder, spécialement conçu pour cette recherche, nous avons analysé la dynamique des interactions et la qualité de la communication. La communication verbale et non verbale des dyades mère-enfant a été évaluée et comparée à la communication avec les autres adultes dans quatre épisodes différents. Les résultats ont montré : 1) une fréquence des interactions supérieure et la qualité de la communication plus riche avec la mère, dans tous les groupes d'enfants mais, lorsqu'un autre déficit s'ajoute à la déficience visuelle, la qualité de la communication est compromise. 2) une plus grande proportion de comportements auto-centrés, aussi bien dans les réactions de l'enfant face à sa mère que dans les réactions de la mère face à son enfant. 3) le rôle primordial de la kinesthésie dans la dynamique de la communication.

Book excerpt: L'objectif de cette étude est d'évaluer les représentations d'attachement des mères avec une déficience visuelle ainsi que la qualité de l'attachement de leurs enfants. Un suivi longitudinal et prospectif de 12 dyades de mères avec une déficience visuelle et leur bébé a été réalisé de l'âge de 3 mois de l'enfant à l'âge de 4 ans. Les représentations d'attachement des mères ont été explorees à l'aide du Ca-Mir qu'il a fallu adapter à la population. Un entretien semi-directif a permis d'aborder le vécu infantile des mères. Les comportements d'attachement des enfants ont été observés lorsqu'ils étaient âgés de 18 mois grâce à la Situation Etrange et lorsqu'ils étaient âgés de 4 ans, les représentations d'attachement ont été explorées à l'aide de la procédure des Histoires à Compléter. Une analyse des interactions précoces avec la CIB à 3, 6, 9 et 15 mois a également été réalisée pour évaluer, entre autres, la sensibilité maternelle. Les résultats mettent en évidence un profil d'attachement moins secure, plus détaché et moins préoccupé chez les mères avec une déficience visuelle que chez des mères issues d'un groupe témoin. A la Situation Etrange, les enfants apparaissent comme significativement plus insecures que les enfants d'un groupe témoin. La Tâche des Histoires à Compléter montre une modification des catégories d'attachement, entre 18 mois et 4 ans, en faveur d'une désorganisation. Enfin, l'analyse des interactions précoces montre que les mères du groupe clinique présentent une sensibilité maternelle de moins bonne qualité que celle de mères d'un groupe témoin. Les résultats de cette étude montrent l'importance de prendre en charge ces mères ainsi que leurs enfants.

Ce projet s’intéresse aux précurseurs de la communication et du langage. Notre travail a pour but d’étudier la mise en place de l’attention conjointe chez des dyades mère aveugle ou malvoyante-enfant voyant. En effet, cette compétence se basant essentiellement sur le regard, il est intéressant de chercher à comprendre comment elle se développe lorsque le canal visuel est défaillant. Cette étude s’est concentrée sur des enfants de douze mois, âge auquel l'attention conjointe se met en place. Des vidéos d’interaction libre entre une mère et son enfant ont été visionnées puis analysées avec le logiciel ELAN et selon une grille d’observation de l'attention conjointe. Afin de mettre en valeur d’éventuels moyens de compensation que la mère pourrait mettre en place pour pallier son handicap visuel, nous avons regardé le nombre de fois où elle initie une interaction avec son enfant, mesuré son temps de parole, et observé l’utilisation du toucher. Après que les données aient été extraites, des tests statistiques ont été effectués. Ils ont montré de manière significative que les enfants de notre population sont moins souvent et moins longtemps engagés dans des épisodes d’attention que les enfants témoins. De même, il a été confirmé que la mère met en place des stratégies pour communiquer et interagir au mieux avec son enfant (initiation des échanges, utilisation du toucher).

Although typical aging does not cause low vision, four major causes of vision impairment are directly related to the aging process: (1) age-related macular degeneration (AMD), (2) diabetic retinopathy, (3) glaucoma, and (4) cataracts. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision. Appendixes include selected diagnostic and billing codes and evidence tables.
