Various public health strategies to promote breastfeeding have been developed and implemented for over four decades, yet progress has stagnated, and exclusive breastfeeding rates remain low globally. From an evolutionary anthropological perspective, low breastfeeding rates seem like an 'evolutionary puzzle'; breastfeeding is a behaviour which confers survival and fitness advantage to children and mothers, yet so many mothers do not breastfeed exclusively or at all. Is this a globally maladaptive behaviour? Framing breastfeeding as a maternal investment behaviour, an evolutionary perspective directs us to consider the fitness costs of breastfeeding, together with the role of social learning and cultural norms. Indeed, an evolutionary anthropological perspective provides insights to why some breastfeeding-promotion strategies may have been ineffective, while pointing to potentially promising policies and practices which have been overlooked

Based on research studies, breast milk provides immune components for the health of the infant and encourages mother-child bonding. Literature has shown the main reason women do not breastfeed is because of inadequate breastfeeding education and support from health professionals or peers. Most nurses in the prenatal and postpartum units have limited knowledge about breastfeeding. The knowledge of the nurses was tested before intervention to gain insight on what the nurses know about breastfeeding and The Baby Friendly Hospital Initiative program. This project aims to improve breastfeeding rates and duration through nursing education. Research evidence supports improved nursing education on breastfeeding as a means of increasing breastfeeding rates. This project proposes breastfeeding education designed for nurses to educate and support mothers for the purpose of increasing breastfeeding rates. In addition, research evidence supports structured education such as The Ten Steps to Successful Breastfeeding to promote breastfeeding. In this light, the proposal suggests the adoption of the Baby Friendly Hospital Initiative program (BFHI) by the hospital to ensure adherence by nurses in their practice. Nurses should familiarize themselves with the problems associated with breastfeeding and the interventions. This is also a one of the main reasons women do not breastfeed and being able to handle these problems will remove some of the obstacles against breastfeeding. Women's' perceptions and cosmetic issues are problems that deserve attention and require nurse's encouragement and support. This project will describe how this will be implemented, the methods by which the outcome will be evaluated and how the results will be shared to stakeholders and larger nursing community.

Book excerpt: The benefits of exclusive breastfeeding until age six months include positive maternal outcomes such as decreased post-partum bleeding and risk of post-partum depression. Infants experience a reduction in sudden infant death syndrome, sepsis, and necrotizing enterocolitis, while development is improved. Despite the benefits to both health and cost, exclusive breastfeeding rates remain low worldwide. Latina immigrant mothers face unique challenges that further affect their success with breastfeeding. The purpose of this evidenced based quality improvement project was to improve exclusive breastfeeding rates in the local Latina population, by providing interventions based on self-efficacy theory. For this pilot program, a convenience sample of pregnant Latina women, of at least 36-weeks gestation, were given the opportunity to enroll in a program providing additional breastfeeding support. All women enrolled demonstrated intent to breastfeed prior to entering the program. Exclusive breastfeeding rates were measured, as well as pre-and-post intervention surveys were completed to assess from intervention effect. The project was an effort to promote exclusive breastfeeding through interventions that address self-efficacy. However, the lessons learned expanded well beyond improving exclusive breastfeeding rates. The results highlighted the challenges of caring for a marginalized patient population and how patient-provider relationships can be affected. The results can inform other clinical sites on how interventions must be tailored specific to individual cultures and communities.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

Book excerpt: Exclusively breastfeeding a newborn by providing no other form of nutrition other than breastmilk has been shown to provide a substantial number of benefits to both mom and baby. In the hospital setting, exclusively breastfeeding mothers are assisted by nurses to continue breastfeeding properly and feed their babies adequately. When using the theory of planned behavior, it is identified that patients may ask for formula when their attitude towards breastfeeding is not representative of the importance of exclusively breastfeeding when possible. Research shows that more education improves a nurse's knowledge and confidence in assisting patients and prevents unnecessary formula supplementation. The purpose of this project was to provide additional breastfeeding education to nurses and implement a breastfeeding checklist in order to help them better assist patients with breastfeeding and ensure every step has been taken to assist the patient. In doing so, the project aimed to increase exclusive breastfeeding rates upon discharge. Before implementation, the exclusive breastfeeding rate upon discharge was 63%, and three months of data following implementation showed rates of 67%, 69%, and 54%. While a marginal initial increase was seen, the exclusive breastfeeding rates did not continue to remain elevated. Many factors influenced exclusive breastfeeding rates, and the checklists were not completed for all patients as intended. Further study is necessary to understand the effect of a breastfeeding checklist, but initial increases in breastfeeding rates indicate that education and awareness for nurses to assist patients with breastfeeding can provide an increase in exclusive breastfeeding rates.

Despite ongoing efforts to promote breastfeeding, current breastfeeding rates in the U.S. are still low, and socioeconomic and racial/ethnic disparities in breastfeeding practices have persisted. Improving breastfeeding practices among socially disadvantaged groups remains challenging. The goal of this dissertation is to improve breastfeeding rates and reduce racial/ethnic disparities in breastfeeding practices in the United States. Specifically, this dissertation aims to: (1) assess the influences of breastfeeding support from family, hospitals and workplaces on breastfeeding duration; (2) determine the extent to which racial/ethnic disparities in breastfeeding duration could be explained by breastfeeding support; and (3) estimate the population impact of multifaceted breastfeeding promotion interventions. Using data on approximately 4,000 mothers enrolled in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC), from the triennial Los Angeles County WIC Survey (2014, 2017), this dissertation found that breastfeeding support from family, hospitals and workplaces was significantly associated with longer breastfeeding duration, and that Black mothers and English-speaking Latina mothers have significantly lower breastfeeding rates than White mothers, with the difference widening over increasing duration. Results from causal mediation analysis (specifically, g-computation) showed that lack of access to breastfeeding support from family, hospitals and workplaces accounted for approximately two-thirds of the difference in breastfeeding duration between White and Black mothers, and one-third of the difference between White and English-speaking Latina mothers. This dissertation also illustrated the use of agent-based modeling (ABM) for estimating the population impact of five selected breastfeeding promotion interventions (improving knowledge, implementing Baby-Friendly Hospital Initiative practices, providing breastfeeding counseling, strengthening family support, and fostering supportive workplace environments) implemented singly or in combination with each other. The ABM demonstrated that while improving knowledge and increasing the availability of Baby-Friendly Hospital Initiative practices improve breastfeeding initiation rates, breastfeeding counseling, family support and a supportive workplace environment are more effective in improving breastfeeding duration. Increasing the coverage of multiple interventions simultaneously had a synergistic effect on breastfeeding duration, with their effects being greater than the additive effects of increasing the coverage of these interventions singly.

Book excerpt: The rate of breastfeeding in the United States has risen and fallen over time as a result of changing lifestyles and the availability of substitutes for human milk. However, since the 1970s the rates of breastfeeding initiation and duration have increased, in part due to the increasing medical evidence of the significant health benefits both to mothers and children. The federal government's Health Objectives for 2010 are for 75% of mothers to initiate breastfeeding at the time of birth and for 50% to continue breastfeeding until their infants reach 6 months of age. Current rates are about 69.5% at the time of hospital discharge and 32.5% at 6 months of age. The health benefits to the infant include the high nutritional quality of human milk, and a decrease in various infectious and other diseases of infancy that are reduced by the anti-microbial, anti-inflammatory and immunological-stimulating agents present. Mothers seem to benefit with a more rapid return to the pre-pregnancy state of their bodies, improved glucose and lipid metabolism, delayed ovulation, and the possible reduction of certain cancers. This new book examines the impact on health, employment and society, laws and government support of this important health action.
