As a chronic disease with long term course and serious outcomes means a conflicting, serious, stressful and burning situation which negatively affects quality of life. In this work we are showing importance of social support for improving the quality of life for a diabetic patients.Aim: Determine the importance of different types of social support to improve the quality of life of diabetic patients using the new tool DIASSQL 23/20 (Diabetes Social Support Quality of Life 23/20) drawn up for the implementation of the milestones of the research.Method: In the first part we are describing epidemiologic course of diabetes mellitus. In the second part we are determining emotional, informational, instrumental and evaluating social support, provision, admission, perception, availability, obtainability of social support, level of social support in context of social needs. In the third part we are determining quality of life, aspects of quality of life and benefit of groups in system of quality of life. In fourth part we are focusing on research and the seven individual aims of the research and five hypothesis. We specify of the research, research models, methods of research, organization and analysis of the research in statistical program Statistical Package for Social Science SPSS. In the survey section, DIASSQL 23/20, we used the method of qualitative research in the form of a combined system of an anonymous questionnaire. Survey sample DIASSQL 23/20 completed of 160 respondents, 143 respondents participated in the survey. DIASSQL 23/20 contains 23 social support items, 20 qualiy of life items and 9 sociodemographic survey questions, which we have obtained together 17 303 informations. The results we processed by the software program Microsoft Excel, statistical software program Statistical Package for the Social Science SPSS and descriptive and inductively statistics into tables and graphs summarize in the discussion and suggested recommendations for practice.Results: We are evaluating the results of the research, appointing practical advantages and recommendations for the daily practice within the modern health education and application of the social work. We could show importance of social support in improving quality of life of diabetic patients. Social support is essential for improving the quality of life for diabetic patients and compensation of diabetes mellitus. New instrument DIASSQL 23/20 is suitable for use in the context of diabetology evidence-based and diabetes education. Will provide modern diabetology other options like even more to help diabetic patients who really need it.Discussion: Chronic disease causes significant interference in the daily lives of individuals. A diabetes mellitus for its pandemic character not only a crisis, stress and stress situation, but as a civilization, a chronic, metabolic, non-communicable diseases with long-term course, economic, psychological, social and health consequences of the deterioration of the overall quality of life and global problem. Namely negative impact on the life of individuals, families, groups, communities and of society. When challenging journey through life with diabetes mellitus significantly assisted by appropriate social support. It is depend not only the quality of social relationships and past experiences, as well as providers, whether from family, groups, communities in social networks, and by the volunteers and professional diabetes educators.

As a chronic non-communicable disease with pandemic occurrence, long-term course and serious consequences significantly affects the daily lives of diabetic patients, families, groups, communities and society. It reduces the overall quality of life and therefore requires specific care systematic and effective solution coping with everyday commands, prohibitions, restrictions, obligations and necessities. As the treatment of this disease with many adverse consequences and strict restrictions is increasingly concentrated in the domestic sphere, features professional diabetes education, counselling and health upbringing and the desired inherent part of personalized care in a spirit of evidence-based medicine. It plays an important role in the prevention of diabetes mellitus in the population, other health problems of diabetics, complications of diabetes mellitus and socio-pathological phenomena at any age.Aim: In this work we are presenting education, counselling and health upbringing as an inherent part of personalized care in a spirit of evidence-based medicine. In detailed we present diabetes education, counselling and health upbringing as a form of primary, secondary and tertiary prevention. By survey we found the need, scope and importance of professional education, counselling and health upbringing of diabetic patients, patients with pre-diabetic syndromes and risk population.Method: Education as a system of the transmission of specific knowledge and associated intellectual and manual skills and habits directs moral, aesthetic and physical development in accordance with the objectives and role in society. Diabetes education eliminates the deficit of knowledge and skills that have arisen and could persist due to the presence of diabetes. Specialized social counselling is focused to the specific needs and problems of diabetics whose solution requires a comprehensive approach. Health upbringing across the entire spectrum of activities and using modern methods of providing and expanding the theoretical knowledge of health, strengthens the attitudes and habits of prevention, protection and support of health not only hazardous but also population groups and significantly promotes positive and eliminates the negative elements relating to health.Discussion: Diabetes education, counselling and health upbringing play a crucial role in a specific diabetes care as a risk groups of individuals. Treatment in the spirit of personalized evidence-based medicine and the development of chronic and organ-specific complications changes. Therefore it requires constant updating and complementing theoretical knowledge, intellectual and manual habits and skills, as well as systematic guidance in eliminating the negative effects of the disease. Important role to play is effective communication with diabetological team. Our long experience has raised the need for lifelong systematic diabetes education, which would be managing university educated professionals. Because diabetic patients are living with disease and its difficulties all life, diabetes education and counselling undertaken by volunteers working off time in civil jobs is not enough to cover all needs and requirements. Therefore there is a need for the implementation of diabetes education and specialized social counselling in regular working time and in health care facilities for 345,475 (2015) diabetic patients in Slovakia and other individuals in the pre-diabetic stage.Results: The main objective of diabetes education is to train, educate and inform diabetic patients and with this improve compensation, extend and improve the quality of life, reduce the financial cost of a personalized treatment in a spirit of evidence-based medicine, the consequences of acute, chronic and organ-specific complications, but also safeguard the rights of diabetic patients as citizens, patients and consumers and thus to achieve their full integration into society. The aim of diabetes education is to get proper theoretical knowledge, practical skills and a good ability to implement them at home. Health problems and the resulting social consequences represent for most diabetic patients problem of social integration, therefore, a specialized social counselling is focused on coping with health problems and their consequences, improvement of adaptation to the disease, the area of support and social support, guidance, orientation in legislation and claims, if necessary for distribution to another entity. In providing different methods and techniques are used, as well as own instrument DIASSQL 23/20 (Diabetes Social Support Quality of Life 23/20) created in Diabetes Educational Centre DIAMART to measure and early detection of psychosocial problems of diabetics, which significantly decompensate diabetes mellitus and thereby reducing the quality of life.

Diabetes Self-Management Education (DSME) helps diabetics adhere to better levels of diabetes self-management resulting in improved blood sugar levels, thereby postponing complications of this progressive disease. Studies show that DM patients with adequate social support manage their diabetes more effectively and improve quality of life. Purpose and Scope: This evidence-based practice project sought to describe the level of diabetic self-care adherence before an after an educational intervention with or without a buddy. By integrating best research evidence with clinical expertise inviting buddies will enhance effectiveness of DSME classes at a selected healthcare facility. Goal: The goal of this practice change project was to provide evidence-based support that DM patients attending a DSME class with a buddy resulted in improved adherence to self-care activities compared to DM patients attending without a buddy. Objectives: Review research related to buddy support as a social support mechanism to enhance successful life-style change interventions. 1) Collect data as determined by the administration of the SDSCA to DM patients attending DSME classes. 2) Compare recorded responses to the SDSCA measure for DM patients attending DSME classes with and without a buddy at the initial class and at the 3-month follow-up. 3) Evaluate and compare participant AIC levels before and 3-months after the DSME. 4) Identify implications for clinical practice. 5) Identify study limitations and implications for further study. Plan: Phase One-Secure support from the diabetes educator and CEO of the hospital, seek approval for use of the SCSCA and obtain IRB approval. Prepare and print a demographics page and print the SDSCA measure. Phase Two-Collect SDSCA measure data with A1C levels at the initial class and at the 3-month follow-up class. Phase Three-Compare the results, perform the statistical analysis, and write up the findings. Outcomes and Results: T-test for the effect of the buddy on the A1C levels revealed a significant difference for those attending with a buddy. General diet choices were not improved for those attending with a buddy. Specific diet choices for those attending with a buddy were statistically significant. Each of the other constructs showed positive improvements and while not statistically significant, they were clinically significant for participants attending with a buddy compared to those attending without a buddy.

DSME classes are not sufficient to sustain required lifelong self-care activities. Social support has been shown to enhance self-care activities in type 2 diabetic patients. Studies show that type 2 diabetes (DM) patients with adequate social support manage their diabetes more effectively and improve quality of life. This evidence-based practice project will evaluate a standard of care practice change. Naturally occurring groups will be compared to determine if there is a difference in self-reported diabetes self-care activities between DM patients who are ages 21-65 years and who choose to attend DSME classes with a buddy compared to those attending without a buddy. The Summary of Diabetes Self-Care Activities (SDSCA) measure, a self-reported questionnaire assessing diet, exercise, blood-glucose testing, foot care, and smoking status has been reliable used for 20 years. SDSCA has adequate reliability as well as validity and sensitivity.
