The Bronx was one of the hardest hit boroughs of New York City, with significant negative impact of the COVID-19 pandemic on its indigent population. SBH Health System (formerly known as St. Barnabas Hospital) is an integrated system of an acute care hospital, ambulatory care center, trauma center, dialysis center, stroke center and other services and facilities, serving the community of the Bronx. The story of SBH in preparing for and managing the rapidly escalating surge of severely ill patients is a treasure of lessons in health crisis preparedness and management at all levels: clinical, administrative, financial, etc. These lessons can be used for other acute care hospital settings and other potential health crises that may arise in the future. Within a short 3 weeks, SBH increased its in-patient capacity by 50%. However, during the same short time, it increased its critical care capacity by over 500%, providing critical care to severely ill patients on ventilators. This book chronicles the situation step by step and describes how this accomplishment was done. Book excerpt: The COVID-19 pandemic has brought rapid changes, increased stress, and ethical challenges to nurses across the globe. These factors may place nurses at increased risk for developing moral distress and substance use disorder. This stress can also increase nurse vulnerability to substance use. Previously there was little evidence about the rates of moral distress and substance use disorder among nurses during the COVID-19 pandemic. The primary objective of this evidence-based project was to describe levels of moral distress and substance use among nurses in a community hospital during the COVID-19 pandemic and to make recommendations for interventions to improve nurse wellbeing and patient safety based on findings. An online survey, consisting of demographic questions, the Measure of Moral Distress in Healthcare Professional Tool, the Alcohol Use Disorder Identification Tool, the Drug Use Screening Tool-10 items 1 and 2, and a single item asking about amount of time caring for COVID-19 patients was sent to inpatient and emergency department nurses working at a rural, community hospital. Frequency statistics and measures of central tendency are used to describe the rates of moral distress, substance use disorder risk for alcohol and drugs, and time spent caring for patients with COVID-19. A total of 57 nurses completed the survey. Nurses were found to be experiencing various levels of moral distress, with the collective group experiencing scores in the middle of the moral distress range. One-third of the nurses reported an intention to leave their position due to moral distress. In addition, a third of nurses who participated in the survey reported risky alcohol use, while 5.3% reported harmful alcohol use. Fully 21.1% of nurses reported using illicit substances, while 5.3% reported using illicit substances daily or nearly daily. Given the literature on the crescendo effect of moral distress and the nature of substance use disorder, the lasting effects to nurses during the COVID-19 pandemic will be important to the profession for years to come. Nursing leadership must commit to implementing resources to help prevent and care for nurses who experience moral distress and substance use disorder.Keywords: Moral distress, substance use disorder, substance misuse, COVID-19

After causing significant morbidity and mortality in China, by February 2020, it had spread to numerous other countries, infecting millions of people and causing a large number of deaths across the world. The COVID-19 pandemic put a burden on almost all areas of the world including healthcare systems, education, industry, travel, etc. The pandemic revealed the vulnerability of the world’s healthcare systems and affected healthcare personnel significantly. The virus is able to attack not only the respiratory tract, but almost all the organs including the brain. Impacts on gut biota have also been noticed. The virus has caused both morbidity and mortality in humans without any geographical, cultural, or religious barriers. The emergence of new variants due to mutations in the virus has aggravated the problem. While the delta variant brought a second wave and killed a large number of people due to various factors such as lowering of saturated oxygen in blood and other physiological emergencies, the omicron variant proved to be less lethal. Though the pandemic has subsided, the emergence of the subvariants BA1 and BA2 and now their hybrids has started to increase the number of cases at exponential levels and has forced new lockdown measures in places such as China. As the conditions laid down to combat the pandemic have been relaxed, the virus may reach other countries and cause additional countries to resort to lockdown again. COVID-19 became the focus of the scientific community with the aim of developing new drugs, repurposing available drugs to be used against the virus, and developing a series of vaccines in a short time. The mild effect of omicron might have been due to the extensive vaccination programmes carried out in various countries. However, there is genuine fear that newly emerging variants may evade the immune system and cause damage to the body. This book highlights the impact of COVID-19 on science, industry, and healthcare systems. The chapters included in the volume come from dedicated experts belonging to basic sciences, biotechnology, pharmaceutical sciences, and other fields of sciences. These include discussions on how the virus evolves and attacks various organs in the body. A separate chapter explains the emergence of various strains of virus.
