However, some labels may create false perceptions of the healthfulness of products and may confuse or mislead consumers. To address this challenge and to support Member States in implementing policy measures, as recommended by the Framework for Action from the 2014 Second International Conference on Nutrition, the World Health Organization (WHO) is in the process of developing evidence-informed policy guidelines on the food environment, including on nutrition labelling policies (with a focus on ingredient lists, nutrient declarations, supplementary nutrition information and health and nutrition claims). This review on contextual factors to be considered in the implementation of nutrition labelling policies was prepared as part of the required process for WHO guideline development.

Book excerpt: During the past decade, tremendous growth has occurred in the use of nutrition symbols and rating systems designed to summarize key nutritional aspects and characteristics of food products. These symbols and the systems that underlie them have become known as front-of-package (FOP) nutrition rating systems and symbols, even though the symbols themselves can be found anywhere on the front of a food package or on a retail shelf tag. Though not regulated and inconsistent in format, content, and criteria, FOP systems and symbols have the potential to provide useful guidance to consumers as well as maximize effectiveness. As a result, Congress directed the Centers for Disease Control and Prevention (CDC) to undertake a study with the Institute of Medicine (IOM) to examine and provide recommendations regarding FOP nutrition rating systems and symbols. The study was completed in two phases. Phase I focused primarily on the nutrition criteria underlying FOP systems. Phase II builds on the results of Phase I while focusing on aspects related to consumer understanding and behavior related to the development of a standardized FOP system. Front-of-Package Nutrition Rating Systems and Symbols focuses on Phase II of the study. The report addresses the potential benefits of a single, standardized front-label food guidance system regulated by the Food and Drug Administration, assesses which icons are most effective with consumer audiences, and considers the systems/icons that best promote health and how to maximize their use.

Book excerpt: Interpretive front-of-pack food labelling (FOPL) is a policy priority for promoting healthy diets. Research evidence indicates that consumers have a reasonable understanding of interpretive FOPL systems and their understanding improves with label familiarity and consistency within the market. A government-endorsed interpretive FOPL policy was found in 15 Member States of the WHO European Region and this report summarizes the evidence on their development and implementation to support policy-makers in navigating these processes. Most existing policies have been implemented under voluntary arrangements with variable penetration into the marketplace. Policy development that is led by government and based on formative research and that engages stakeholders and the public is most likely to lead to acceptable credible and effective policies. FOPL implementation is best supported by policy provisions that encourage widespread uptake of the system and allow for formal evaluation of both implementation and impact.

Book excerpt: The Nutrition Labeling and Education Act of 1990 (NLEA) changed the existing regulatory framework for food labeling requirements that was shared among federal, state, and local levels of government. In addition to creating a system of mandatory nutrition labeling for foods, NLEA provided a schedule for the preemption of state and local labeling requirements that were not identical to federal provisions. Six provisions were not to be preempted until a study on the adequacy of the federal implementation of those provisions was completed. Food Labeling is the result of that study. It presents recommendations concerning the Food and Drug Administration's implementation of the six provisions that were studied, suggestions for the future disposition of relevant state and local food labeling requirements, and views on the continuing importance of the working relationship among the various levels of government in assuring that consumers are protected from misleading label information.

Forty-four countries have introduced these types of labelling schemes with a view to promoting positive public health outcomes. Preliminary insights on the impact of these schemes show that they can influence reformulation efforts by the food industry as well as consumers' purchasing decisions. The schemes differ, however, across several dimensions including implementation methods (voluntary or mandatory), the extent to which they provide consumers with nutrient information versus nutrition advice, the conveyed message, targeted products, design and appearance, and accompanying national policy mix. Their rapid proliferation across the world has led to a diversity of schemes which can have implications for private sector decisions and trade. Mechanisms to improve the availability of information and reduce administrative hurdles should be considered in order to improve the integration and use of such schemes, and to facilitate trade. Improving consistency across the nutrient criteria that underpin the different schemes could be considered when reviews are undertaken, especially at the regional level.
