Book excerpt: Primary care providers have not customarily provided oral assessments or offered anticipatory guidance about oral health care; dentists manage that task. Myths and access-to-care issues often result in limited or no dental care during pregnancy. Improving the oral health of pregnant women has a positive correlation with fewer dental caries in their children. Currently, evidence-based practice (EBP) oral health guidelines exist for pregnant women and children. While health care providers (HCPs) want to implement best practices, they find practice change challenging. This project utilizes a planned change theory to guide the implementation of oral health guidelines with a methodical approach. Retrospective and prospective chart audits evaluate HCP behavior change. The chart audits reveal HCPs change behaviors after a practice intervention with a Primary Care Oral Assessment Tool (PCOAT), and again after introduction of EBP guidelines. Poor oral hygiene leads to poor oral health, which is directly linked to overall health and well-being. Residents in long-term care settings who experience poor oral health are at a greater risk for experiencing respiratory illnesses, systemic infections, uncontrollable glucose levels, cardiovascular disease and strokes. Poor oral health may also contribute to poor nutrition due to difficulty chewing and swallowing, as well as a decrease in self-esteem. Researchers have concluded that barriers to proper oral hygiene care include: staff's lack of knowledge and technique pertaining to oral hygiene care and oral health, staff's negative perception about oral hygiene care, lack of resident cooperation, lack of time to complete proper oral care and lack of readily available supplies. A thorough literary research was completed to support the suggested implementations. It has been shown that oral care focused education and training, such as an oral health program, improves staff perception and technique in completing oral care. Implementing oral hygiene and oral health education and training programs, establishing guidelines and routines for the provision of oral hygiene care, designating staff for the provision of oral hygiene care, and the use of an oral hygiene cart greatly improved the quality of care and the oral health status of residents in long-term care settings. These implementations not only improved the oral health of the residents, but also improved their overall well-being, resulting in an improvement to their quality of life.

The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care. These recommendations support the creation of a diverse workforce that is competent, compensated, and authorized to serve vulnerable and underserved populations across the life cycle. Book excerpt: Background: Millions of Americans continue to live with one or more oral diseases, including dental caries, which lead to periodontal disease. Periodontal disease continues to have a significant impact on those with chronic disease states, such as diabetes. Multiple reasons exist for why a significant improvement in oral health has not occurred since the original call to action was released. These reasons include lack of provider buy-in, access to dental providers, poor patient oral health literacy, and lack of finanical resources. Purpose: The purpose of this scholarly project was to integrate an oral health-screening tool for healthcare providers to use to guide oral health screening examinations in an effort to improve interdisciplinary care for patients who have been diagnosed with diabetes. This project was also designed to improve the documentation of discrete data elements of the oral health exam within the electronic health record (EHR) utilized in the Coordinated Care Organization (CCO). Methods: Using the Rosswurm and Larrabee Model for Change to Evidence Based Practice (EBP), a process improvement project was conducted implementing the Oral Health Assessment Tool (OHAT) in primary care practice setting within the CCO. Lewin's Change theory provided additional guidance in asking the healthcare providers to implement this change project into their practice. Findings: The project was conducted as a pilot project in a clinic located in Southern Oregon over a four-week period of time where a total of fifty-five patients with a diagnosis of Diabetes Mellitus were evaluated. Of this number, thirty-one patients were screened using the OHAT flowsheet. A healthcare provider experience questionnaire was also administered at the conclusion and provided necessary feedback in how to improve upon the processes that were employed during the rollout of the project. Recommendations: Further study is needed in using the OHAT in additional clinical sites within the primary care practice setting in order to compare the original results with the new data. Additional continuing education on oral health for healthcare providers should include how to conduct oral health assessments, thus improving the comfort level of the primary care providers (PCP) and clinical staff in using the OHAT. Further work is also warranted on how to improve the communication between medical and dental care providers.
