Book excerpt: "Background: Type 2 Diabetes (T2D) is a prevalent health care condition, affecting many people across the world. Despite the severity of this health condition, medication non-adherence among T2D patients has remained high, hindering them from achieving blood sugar control and desired health outcomes. Aim: To implement text messaging as an Evidence-based Intervention (EBI) to improve medication adherence and blood sugar control among T2D patients. Methods: Fifty T2D patients aged 18 years and above who had been identified as non-compliant, were engaged in the project for a period of 10 weeks. Participants' demographic information was collected using demographic survey forms. The initial step of the project involved the PM performing pretests for blood glucose. Later, blood glucose tests were performed at week 6 and week 10. Throughout the project, the participants were reminded through text messaging to take their medication, eat balanced meals, exercise, check their blood sugars, ask them how they are feeling, check their feet, remind them about clinical checks, ask them if they are feeling jittery and record blood sugar in the logs. Changes in blood sugar levels at weeks 6 and 10 were analyzed by PM. Findings: Thirty-four percent and 60% of the participants recorded improved blood sugar levels at weeks 6 and 10 respectively indicating that text message intervention at the end of the project was effective. The intervention exceeded the 50% benchmark at week 10. Implications: Text message intervention should be implemented as an evidence-based intervention to improve medication adherence among T2D patients. Nonetheless, there is a need to integrate the intervention with other interventions for more improvements in medication adherence. At a private practice in Maryland, medication adherence was noted to be a challenge among Type 2 diabetes patients; strategies such as one-on-one teaching was incorporated by the providers to facilitate adherence. This article discusses the implementation process and evaluation phase of an evidence-based educational intervention using individualized teaching with a 20-minute PowerPoint presentation on Type 2 diabetes using Virginia Henderson's Need Theory to guide the practice change framework. A total of 50 participants completed the pre/post questionnaires for the evidence- based project (EBP) change project. Although, a patient cannot be forced to adhere to their medication regimen, healthcare professionals can make sure that the patients make an educated decision. If healthcare providers assist patients with making an accurate decision when making decisions related to their healthcare and comply with their medication, management of DM will be better. The manner in which this assists is that it provides more information on this topic and improves education techniques that can be more comprehendible.

Book excerpt: Uncontrolled diabetes mellitus is the seventh leading cause of mortality and the leading cause of blindness, kidney failure, and non-traumatic amputations in the United States. A high prevalence of type 2 diabetes mellitus (T2D) has placed a strain on health care systems due to costs associated with anti-diabetic medications as well as diabetes-associated morbidities and disabilities. Traditionally, medical care providers have prescribed lifestyle and medication changes during clinical face-to-face visits, however these visits are costly and are often not effective for producing desired changes in self-management techniques. Evidence shows that the current standard of care often fails to deliver on achieving evidence-based recommendations for glycemic control for patients with diabetes. Recent advancements in telemedicine technologies have emerged as promising platforms which can deliver diabetes management services while reducing unnecessary use of health care resources. Different technological approaches may vary with regard to patient glycemic control outcomes, and cost differences should be taken into consideration when selecting the technology that may provide the greatest overall benefit for the patient. Many newer glucometers have transmission capabilities, allowing these meters to link to smartphone Apps or websites. Patients can measure their glucose levels, share results with their healthcare team in real time, and talk over the phone or through video visits for medication or lifestyle interventions, all in a more expedient manner compared to traditional face-to-face visits. Remote monitoring of blood glucose levels by clinicians has been shown to be feasible and acceptable for patients with both type 1 diabetes mellitus (T1D) and T2D. With this background in mind, the aim of the current review was to evaluate the effectiveness of remote blood glucose monitoring compared to continuous glucose monitoring (CGM) for lowering HbA1c in adult patients with T2D. PubMed was searched for randomized controlled trials, clinical trials, and systematic reviews that included either remote blood glucose monitoring, CGM, or both, and individual interventions had to be longer than six weeks in duration. Studies also had to include adult patients with T2D and had to examine the outcome of change in HbA1c as the primary or secondary outcome of interest. Inclusion and exclusion criteria were determined a priori, and searches included a variety of search terms yielding 92 records, of which 27 articles met the inclusion criteria. Study findings suggested that both remote blood glucose monitoring and CGM are effective for reducing HbA1c in patients with T2D compared to controls. Both the absolute treatment means, and the average treatment mean differences suggest larger reductions in HbA1c in the remote blood glucose monitoring interventions as compared to the CGM interventions. In agreement with previous research, side by side comparisons of the included studies revealed a trend toward greater absolute reductions in HbA1c among all studies where patients had higher baseline HbA1c levels, frequent engagement with the clinical team for more timely and responsive management, as well as algorithm-based treatment plans. Future studies should include a comparison of feasibility, cost of care to implement the interventions, and cost savings to inform clinical decision making, thereby identifying the technology with the greatest overall benefit for patients with diabetes.

The use of evidence-based practice (EBP) for developing interventions and the multiple implications on advance nursing practice will be discussed. The lack of available office time to provide proper education to the diabetic population was identified as a clinical issue. A web-based program was anticipated to assist in patient education. The PICO (population, intervention, comparison and outcome) question provided the structure guiding a comprehensive literature review producing multiple supporting articles on models, nursing theories, standards of care and various successful interventions and outcomes providing the framework for developing and implementing a web-portal power point education presentation on diabetes aimed at the poorest controlled practice site diabetic patients. Thirty diabetic patients with a Hemoglobin A1c value of greater than 8.0% participated in the EBP change project. Data collected were pre and post-test questionnaire scores before and after viewing the power point education presentation. The data collected produced a finding of a 9.6% improvement in post-test scores after the intervention. The description statistical analysis of the collected data indicated an 80% (p = 0.10) confidence level in the intervention increasing the knowledge base. The success of the EBP change project has allowed the dissemination of the project to various other patient groups both within and outside the practice site setting. A powerful combination of factors is driving this change. These include rapid advances in mobile technologies and applications, a rise in new opportunities for the integration of mobile health into existing eHealth services, and the continued growth in coverage of mobile cellular networks. According to the International Telecommunication Union (ITU), there are now over 5 billion wireless subscribers; over 70% of them reside in low- and middle-income countries. For the first time the World Health Organization's Global Observatory for eHealth (GOe) has sought to determine the status of mHealth in Member States; its 2009 global survey contained a section specifically devoted to mHealth. Completed by 114 Member States, the survey documented for analysis four aspects of mHealth: adoption of initiatives, types of initiatives, status of evaluation, and barriers to implementation. Fourteen categories of mHealth services were surveyed: health call centres, emergency toll-free telephone services, managing emergencies and disasters, mobile telemedicine, appointment reminders, community mobilization and health promotion, treatment compliance, mobile patient records, information access, patient monitoring, health surveys and data collection, surveillance, health awareness raising, and decision support systems.

The need for comprehensive community-based models for prevention, detection, and treatment of diabetes and associated co-morbidities is critical. Diabetes is the most costly chronic disease in the United States, costing an average $15,000 per patient per year; the disease afflicts 1 in 100 Americans, and its incidence has risen by 65% over the last ten years. The purpose of this paper is to present concepts for implementing a reminder system to recall individuals with type 2 diabetes mellitus (T2DM) and to discuss project implementation of such in a clinical setting. The vast numbers of individuals with diabetes and their need for regular exams and lab testing challenge providers to adequately manage this group. In this paper is a discussion of the application of an evidence-based practice systematic approach to treatment for diabetes, commonly called Staged Diabetes Management (SDM) that helped set the stage for implementing a new reminder system in the quest to reduce morbidity/mortality of patients with diabetes. After a literature search that confirmed the need for a practice change using structured guidelines, clinic staff developed tools for identifying and tracking patients. The Iowa Model of Evidence-Based Practice guided the steps to successful design, tool development, and methods for implementing and evaluating the process. The same-patients six-month project compared number of visits to the two previous years before the intervention. The result was that 29 of 30 patients in the project increased their number of visits allowing the opportunity to implement SDM protcols. Post project results indicated steady improvement of HgA1c results.

Diabetes self-management education has been involved in the prevention of diabetes-related complications among type 2 diabetes mellitus (T2DM) patients. Emphasis was on healthy food choices, increase in physical activities, and medication adherence. Purpose: The purpose of the evidence-based practice (EBP) project was to evaluate the effect of education on T2DM inmates 18 years and older on lifestyle changes such as weight loss. Methods: This was an EBP project conducted on 20 participants with T2DM. Project implementation lasted a total of 10 weeks. Interventions were individualized education sessions focusing on various aspects of diabetes control. Pre- and post-test utilized diabetes knowledge test 2 (DKT2) for interventions. Results: The mean percentage score on the pre-test was 63.65% with a SD of 8.35 compared to post-test score of 89.47% and SD of 3.64. There was a 25.82% increase in participants' knowledge post-intervention. The secondary outcome measured weight changes among the participants. Mean of pre-intervention weight was 234.31 pounds, median of 225 pounds, mode of 214 pounds, and a SD of 40.38. Post-intervention weight had a mean of 231.37 pounds, median of 219 pounds, mode of 256 pounds, and SD of 40.24. The difference between pre-and post-intervention weights was 1.25%. Implications for practice: Nurse practitioners play major roles in leading the interprofessional team and in providing health promotion policies and principles to various groups of patients. Successful planning, implementation, and sustaining continuity of change requires multidisciplinary involvement. It is estimated that about 90% of the currently diagnosed adults have type 2 diabetes, and based on the World Health Organisation (WHO) report, about 422 million adults were living with diabetes in 2014 compared with 108 million in 1980; this condition caused about 1.5 million deaths in 2012. In the United States of America, it is estimated that about 30.3 million adults are living with diabetes, with a further 1.5 million new diabetes cases diagnosed every year, representing an increasing prevalence of this condition. Diabetes represents a major public health challenge, despite advances in technology and the pharmaceutical industry. These problems may be in the form of acute or long-term complications. Therefore, in order to attenuate the problems of diabetes, management strategies usually include lifestyle changes such as increased physical activity and dietary interventions. Studies which evaluate the role of nutrition in the management of type 2 diabetes often involve human and animal models as these approaches enable us to have a broader and more in-depth understanding of the condition. In some cases, diabetes may co-exist with other conditions, such as stroke, and these may present unique challenges with regard to nutritional interventions. This Special Issue aims to evaluate the risk factors associated with type 2 diabetes and the role of the diet in the management of people with this condition. This evidence is drawn from both human and animal studies.

Medication adherence, which is correlated with patients' health literacy, should be consistently practiced in order to achieve optimal diabetes control. However, no literature has systematically explored the multiple psychosocial and communication barriers and facilitators at the individual level that mediate the linkage between health literacy and medication adherence. A deeper understanding of the specific communication and psychosocial factors related to medication-taking behaviors across different levels of health literacy among people with T2D will guide the development of effective interventions and customized strategies to enhance medication adherence. Objectives: (1) To examine whether the barriers and facilitators associated with medication adherence differ among people with T2D across different levels of health literacy. (2) To explore patients' perceptions of the barriers and facilitators of medication adherence across different levels of health literacy. (3) To understand how the qualitative interview data complement the quantitative survey results about the barriers and facilitators of medication adherence. Methods: This mixed-methods study used an explanatory sequential design, including a quantitative study with a survey questionnaire followed by a qualitative study with semi-structured interviews. The Health Literacy Pathway Model was used to identify the psychosocial and communication factors that may influence medication adherence. The target population was patients who were 20 years of age or above, were diagnosed with T2D, understood English, and were prescribed at least one oral diabetes medicine daily. Analysis of covariance and direct content analysis were used respectively in quantitative and qualitative analysis. A joint display was used to present the integrated findings from quantitative and qualitative data. Results: In the quantitative phase, 205 participants provided complete information in the survey questionnaire. In the qualitative phase, 23 participants completed semi-structured interviews. Confirmed by quantitative and qualitative data, holding stronger self-efficacy, having fewer concern beliefs about medication, and possessing fewer perceived barriers to medication-taking are necessary for better medication adherence. Linking medication-taking to daily routine and focusing on the internal locus of control is an imperative approach to foster self-efficacy of medication use. Addressing patients' life experience and clarifying medication misinformation may help participants cope with their concerns with medications. Solving the high cost of medications and therapy-related problems could lessen participants' perception of barriers to medication-taking. Conclusion: To optimally support patients' diabetes care, practitioners should address concern beliefs among non-adherent patients with low health literacy, as well as emphasize self-efficacy and perceived barriers to medication adherence among all non-adherent patients with T2D.

Due to the increasing prevalence and incidence of diabetes, the increase in the number of people with diabetes in United States combined with non-compliance to medication and lifestyle changes result in to suboptimal care with medical complications. The purpose of the evidenced-based practice project is to increase the self-management behavior rates in adults ages 18-40 years with type 1 diabetes (T1D). Text message reminders with cellular phones were used to ensure the best possible outcomes for non-compliant T1D patients. Methods: The EBP project took place in an outpatient diabetic treatment center located in a hospital in Washington DC. Text message reminders were sent to 13 participants ages 20-38 years with diagnosis of T1D three times a week as well as collecting self- behavior activity logs and two questionnaires; Patient Activation Measure (PAM) a 13-item questionnaires was used to measure activation level and Morisky Medication Adherence Scale (MMAS) an 8-item questionnaire was used to measure adherence level, both questionnaires were given pre and post interventions. Data was collected after 5 weeks of intervention and analyzed with descriptive summary and paired T test. Conclusion: There was significant change in the rate of self-management behaviors such as self-medication, self-blood sugar check, and follow up appointment. Participants that achieved post intervention PAM measure of {601}60 were 100%, participants that accomplished post intervention MMAS level of {601} 6 were 82% and participants also attained a post self-behavior activity of 90%. The incorporation of this new idea into practice improved self-management behaviors in diabetes. " -- Abstract.

Book excerpt: This report is a systematic review and critical appraisal of the literature on the use of formal outpatient diabetes education as a therapeutic tool for self-management in the adult patients with type 2 diabetes. Formal patient diabetes education to promote self-management (PDE) at different levels of complexity and comprehensiveness has emerged from instructions by physicians, nurses, and dieticians in physicianþs offices and hospitals. Since 1990 there has been a shift from traditional approaches of PDE to focus on patient-centered perspectives, self-efficacy, self-management and empowerment issuses. Quantitatives research on the use of formal PDE as a therapeutic tool promote self-management in adults with type 2 diabetes has produced mixed results in terms of improved metabolic control and reduced risks for cardiovascular disease. The long-term diabetes control outcomes of formal PDE when used to promote self-management in adults with type 2 diabetes have yet to be established. Further investigations are needed to determine which types of programs or what components are most effective in improving the ability of adults with type 2 diabetes to self-manage their disease; or which category of patients might benefit most. Findnings from qualitative research studies helped to better understand the context in which formal PDE should be applied to be successful in promoting self-managemenet behaviors in adults with type 2 diabetes.

Studies have shown that patients with diabetes mellitus who participate in nurse-led educational sessions improve their medication adherence and day-to-day diabetes management. The literature suggested that nurse-led educational sessions improved medication adherence and management skills among patients suffering with diabetes mellitus. The goal of an evidenced based practice change project was to increase knowledge and medication adherence in adult patients with diabetes through a nurse-led educational project. Twenty-six adult participants were enrolled in the evidenced based practice change project. Eleven of the participants completed the medication adherence post-test. All participants completed the diabetes mellitus knowledge portion of the evidenced based practice change project. Results reflected the intervention had a positive impact by increasing knowledge level and medication adherence in the participants. The results of the evidenced based practice change project exceeded the benchmark of a 28% increase in post-test scores set by the literature which suggests that implementation of a nurse-led educational intervention can improve medication adherence and patients' knowledge of diabetes mellitus management. The findings could be applied to other areas of the healthcare arena to improve medication adherence in patients with other chronic diseases.
