Book excerpt: Introduction : La couple de frottement alumine-alumine dans la prothèse totale de hanche présente l'avantage potentiel de minimiser l'usure et l'ostéolyse, surtout chez les patients jeunes. Le revêtement d'hydroxyapatite (HA) montre des résultats encourageants quant à la fixation osseuse précoce. Le but de cette étude était d'évaluer les résultats à moyen terme d'un implant cotyloïdien avec un couple de frottement alumine-alumine. Matériel et méthodes : 68 prothèses chez 65 patients (27 femmes et 38 hommes) ont été posées en 1998. L'âge moyen des patients était 53 ans ± 12,3. Tous les patients étaient considérés actifs après la mise en place de la prothèse. Quarante-trois pourcent des patients avaient un Body Mass Index entre 25 et 30 et pour 21,5% il était supérieur à 30. Le composant cotyloïdien press-fit était hémisphérique, en Titane poreux, recouvert d'HA. L'insert en alumine était fixé dans la cupule par un cône morse inversé. Le composants fémoral était cimenté dans 50 hanches et non cimenté dans 18 hanches. L'évaluation a été réalisée par un chirurgien indépendant qui n'a pas participé aux interventions. Les résultats cliniques ont été évalués par le score de Postel-Merle d'Aubigné et les résultats radiologiques évaluaient l'ostéolyse, l'usure, la migration et l'aspect de l'interface os-implant. Résultats : 2 patients ont eu une révision, l'un pour descellement cotyloïdien 6 années post opératoire et l'autre pour un descellement fémoral 4 années post opératoire. Les 66 hanches restantes ont eu un suivi moyen de 7,3 ans ± 1,4. Les résultats cliniques selon la classification de Postel-Merle d'Aubigné montraient un résultat bon pour 7 hanches et excellent pour 59 hanches. La survie était de 96,9% (intervalle de confiance à 95% : 100% et 92,7%) à 8 ans. La survie du composant acétabulaire était de 98,4% (intervalle de confiance à 95% : 100% et 95,3%) à 8 ans. Les résultats radiologiques à la fin du suivi ne montraient ni ostéolyse ni usure. Discussion et conclusion : l'implant cotyloïdien press-fit, recouvert d'HA montre des résultats encourageants chez les patients jeunes et actifs. Comme observé dans les suivis à long terme de séries de couples de frottement alumine-alumine, aucune ostéolyse n'a été retrouvée sur les composants fémoraux et acétabulaires. Cette méthode de fixation de cotyle semble prometteuse et confirme les précédents résultats du couple alumine-alumine.

Book excerpt: Les implants verrouillés ont montré des résultats favorables pour la reconstruction osseuse fémorale lors de révisions de PTH, mais des défauts d’intégration ont été rapportés. Le but de ce travail était d’évaluer un pivot verrouillé disposant d’une couverture aux 2/3 proximaux par de l’hydroxyapatite. MATERIEL ET METHODE : Entre 1996 et 2007, 158 implants fémoraux du type LineaTM ont été implantés pour des révisions d’arthroplastie de hanche. Seuls les 143 cas ayant plus d’un an de recul ont été analysés (14 décès, un perdu de vue) : 83 révisions pour descellement aseptique, 41 fractures péri-prothétiques, 19 révisions en milieu infecté. Les patients ont été évalués rétrospectivement par un observateur non opérateur. Il s’agissait de situations complexes puisque 36 patients avaient déjà eu au moins 2 prothèses préalables, 25,9% avaient une atteinte osseuse sévère stade 3 et 4 selon la Sofcot et 61% avaient une atteinte diaphysaire. RESULTATS : L’amélioration clinique était significative, le score de Postel Merle d’Aubigné passant de 7,7 + 4,3 à 15,6 + 2,2 au recul moyen de 45,7 mois (14-131 mois) (p
