Book excerpt: Au sein des réanimations adultes du CHU de Clermont-Ferrand, une étude sur l'implantation d'un nouveau soin de bouche chez les patients intubés est effectuée. Ses objectifs sont de déterminer l'amélioration de la qualité du soin de bouche, l'impact sur les Pneumopathies Acquises sous Ventilation Mécanique (PAVM) et le coût-bénéfice de son implantation en réanimation. La qualité du soin de bouche est mesurée à l'aide d'une grille d'évaluation de l'état buccal en évaluant 4 critères (langue, muqueuses, gencives et dents). Les données démographiques et les facteurs de risques des PAVM sont collectés. Un suivi des PAVM est réalisé auprès du réanimateur réfèrent hygiène de chaque service. L'analyse d'incidence budgétaire (comparaison des coûts et des recettes) et l'évaluation des coûts évités ont été calculés. Un total de 2030 patients sont inclus dans l'étude (p1=908 et p2=1122). Les 2 populations sont statistiquement similaires. L'analyse des grilles d'évaluation du soin de bouche, montre qu'à partir de 48h d'hospitalisation, une amélioration significative de l'état buccal est constatée pendant p2 avec le nouveau DM. Une réduction significative des PAVM est observée entre les 2 périodes (p1=12,40%, p2=8,30%, p=0,002; a=0,05, score de propension OR=0.6). Le coût d'implémentation du protocole a été estimé à 11 400 € par année. Les recettes en cas de PAVM varient de 28 à 45k€ et le coût moyen de 39 à 42 K€. Le coût bénéfice annuel calculé est d'environ 1,9 millions d'euros en faveur du nouveau soin de bouche. Cette étude démontre que l'implantation du nouveau soin de bouche améliore la prise en charge des patients, les pratiques de soin des personnels soignants et est économiquement viable. Les informations financières collectées ont fourni des arguments nécessaires à l'achat de dispositifs pour la prévention des PAVM ainsi que de nouvelles données sur les coûts et recettes liés aux PAVM.
