Clinical research has been focusing on the long term results of implant therapy conducted in university or hospital settings, which often involves predefined patient selection. However, little data exist on implant survival, prevalence of biological and technical complications and patient-reported outcomes for implants placed and restored in private practice settings.Aim The aim of the present retrospective study was to assess the survival, complications and patient satisfaction of dental implants placed and restored in a private practice setting after an observation period of at least 5 years . Material and Methods203 parallel wall implants (89% Biomet 3i, 8,4% Straumann, 2,5% Nobel Biocare) were placed by the same practitioner in 58 patients. 93,5%% of the restorations delivered were screw-retained and 6,5% cemented. All restorative procedures were performed by a second practitioner. Biological parameters evaluated for each implant included radiographic bone loss (RBL), plaque index (PI), bleeding on probing (BOP), probing depth(PB), keratinized tissue width(KTW), recession, suppuration, mobility and peri-implant radiolucency .The prosthetic parameters evaluated were porcelain fracture , prosthetic and abutment screw loosening/fracture, metal framework fracture, resin plug replacement, loss of contact point and decementation. Patient-related parameters were recorded through patient questionnaires rating satisfaction with appearance, function and general satisfaction. The median follow-up time was 7.8 years. Descriptive statistics and chi square analysis for exploring associations were performed.ResultsThe implant survival rate was 95,7%. Plaque was present in 15,9% and BOP in 69.7% of the sites. PD was u22653 and u2264 5 mm in 33% of the sites and u2265 5mm in 11.3% of the sites. In 24.6% of the sites KTW was u2264 1,5mm, whereas in 21% of them recession was recorded. Book excerpt: Since their introduction, the use of endosseous implant for replacements of missing teeth has increased dramatically. To date, however, there is no evidence-based answer to the important clinical question: What implant-supported prosthesis designs provide the best outcomes? Cochrane systematic reviews have been used to critically assess scientific literature, to answer a wide array of questions. This approach was used to focus on implant-related treatment outcomes, including patient-based outcomes. Data showed that in most cases clinical and patient-mediated outcomes did not differ significantly from one another regardless of the prosthesis used. However, long-term prognosis of implant-supported overdentures using magnet attachments may produce inferior outcomes when compared to other attachments.In this dissertation it was shown that long-term results following immediate loading of endosseous implants with overdentures were successful based on biological and clinical parameters, cost to patient, and patient-based outcomes. Moreover, it was noted that well-designed clinical trials evaluating efficacy of immediate loading with fixed prostheses are uncommon. Furthermore, a parallel randomized controlled clinical trial focusing on this issue was carried out. In this trial, the author investigated the effects of immediate and delayed loading of implants with mandibular fixed prostheses. The implant-related outcomes underscore that even when implants are loaded during the healing period (i.e. immediate loading), successful osseointegration can be achieved and maintained. Patients. satisfaction and oral health-related quality of life indicated that the two loading schemes addressed patient's need equally.
