Book excerpt: Les sujets âgés représentent, du fait du vieillissement de la population, une part importante des sujets admis pour syndrome coronarien aigu (SCA). La fragilité est un paramètre complexe entrainant une réduction des réserves physiologiques et de la résistance au stress. La pertinence et l'impact de la fragilité dans cette population restent encore incertaines. Le but de cette étude a été de déterminer la prévalence de la fragilité et son impact sur la mortalité toutes causes à moyen terme chez les sujets de 80 ans et plus hospitalisés pour syndrome coronarien aigu. Matériel et méthodes : ce travail prospectif, mono-centrique, est basé sur le registre SCA-PA, évaluant des sujets de 80 ans et plus hospitalisé dans un centre tertiaire pour un syndrome coronarien. Une description globale de l'ensemble de la population a été réalisée puis une analyse de survie a été effectuée avec évaluation des déterminants de la létalité, notamment la fragilité au travers du score EFS (Edmonton Frail Scale). Résultats : 238 sujets ont été inclus avec un suivi médian de 16.1 mois. L'âge moyen était de 85.9 ans. Durant le suivi, 77 (32.4%) patients sont décédés. 119 sujets (50.4%) étaient considérés " non fragiles " (EFS 0 - 3), 68 (28.8%) " modérément fragiles " (EFS 4 - 6) et 49 (20.8%) " très fragiles " (EFS = 7). Book excerpt: Le vieillissement de la population est actuellement un fait avéré qui ira crescendo dans les prochaines décennies. A ce titre, les pathologies cardiovasculaires au premier rang desquelles les SCA seront une pathologie très fréquente. Le but de notre étude était d’évaluer de manière prospective, l’impact pronostique de la perte d’autonomie chez les patients âgés hospitalisés pour un syndrome coronaire aigu. Méthodes : Nous avons inclus 271 patients de plus de 70 ans hospitalisés pour SCA avec ou sans élévation du segment ST. Les données cliniques, biologiques, thérapeutiques et échocardiographiques ont été rassemblées de manière prospective. La perte d’autonomie était évaluée par le questionnaire ADL de Katz. Le critère primaire était la mortalité globale au terme du suivi. Résultats : La moyenne d’âge des patients était de 78 ans +/- 5 ans dont 58% d’hommes. 74 patients soit 23% présentaient une perte d’autonomie. En dehors du fait de la réalisation d’une coronarographie, il n’existait pas de différence thérapeutique médicale entre les 2 groupes. 66 patients sont décédés durant la période. Le suivi moyen est de 534 jours. La perte d’autonomie ressort comme facteur indépendant de mauvais pronostic en analyse multi-variée. Conclusion : La perte d’autonomie semble être un marqueur majeur indépendant de mauvais pronostic.
