Formerly, the common sense was that low income in developing countries causes their bad condition concerning healthiness, but recent economic research increasingly considers the causal link between this two phenomena running the other way round. That is, poor health has a negative effect on per capita income, as it leads to lower labour productivity, lower investments in human capital and, by affecting the people's saving behaviour, in physical capital, and furthermore has an influence on the country's demography. The purpose of this paper is to identify the impact of the HIV/AIDS crisis on the South African economy and how this impact affects South Africa's economic growth and therefore its prospects for development. Economic impact "can be defined as that which causes the diversion of resources too uses that would not have been necessary in the absence of HIV/AIDS, and decreased production due to the disease". As this economic impact occurs through various channels, on different levels, it seems reasonable to approach the problem on these different levels, namely households, firms and business, and the macro economy."--p. 3.

Recent demographic work summarized in two reports prepared by ING Barings (1999, 2000) estimates that, since the onset of the AIDS epidemic, more than 500,000 South Africans have died of AIDS-related causes. By 2015, this number is projected to grow by a factor of 20, to more than 10 million deaths. By 2008, overall life expectancy in South Africa is forecast to fall from its pre-epidemic high of 65 years to only 40 years. While modification of high-risk behaviours could reduce AIDS-related death rates, the long delays between infection and death mean that behaviour change now would only beg in to reduce the number of AIDS deaths in five years time with the full affect lagging by a decade or more. With an HIV infection rate currently estimated at almost 20 per cent of the adult population (and projected to increase), prospects for avoiding a major human development crisis over the coming decades are dim. The key question now is how to deal with the impending crisis. The epidemic has moved beyond its earlier status as a health issue to become a development issue, with social, political, and economic dimensions. This paper focuses on the economic aspects. As we begin to recognize the magnitude of the crisis (as measured by the number of AIDS-related deaths), characteristics of those affected (adults in their prime working years), and the many different channels through which the epidemic impacts on the economy (skills availability, savings, demand patterns, productivity, etc.), it is important to try to sort through and evaluate how these myriad of factors are likely to affect key economic outcomes: sustainable growth, employment creation, poverty and income distribution. In addition, it is critical to consider public policy choices in the context of the epidemic.

The report, "Challenging the Challenger: Understanding and Expanding the Response of Universities in Africa to HIV/AIDS," based on seven case studies (Benin, Ghana, Kenya, Namibia, South Africa, and Zambia), seeks to understand how the disease is affecting African universities and to identify responses and coping mechanisms that might profitably be shared with sister institutions in similar circumstances. The studies show that HIV/AIDS is having a serious impact on the fiscal situation of the universities in much the same ways as it does on other institutions. The disease increases operating costs, reduces productivity, diverts resources, and threatens sources of income. The case studies insist that committed leadership among the university's top management is the foremost requirement to combat this crisis.

This paper traces the social and economic channels through which AIDS is likely to have its impact on the development prospects of Uganda, one of the first African countries to confront the seriousness of the epidemic. The paper revises and updates an earlier study to incorporate results of the country's 1991 population census. The study recommends that key policymakers in ministries other than health, such as finance and planning, must factor in the consequences of AIDS when considering development strategies. It examines the effects of AIDS on health expenditures in light of projections of essential drugs required for treatment, as well as the impact of the epidemic on agricultural production from a household and farming system perspective. Recommendations and suggestions of areas for further research conclude the study.

In some countries with high HIV prevalence rates, life expectancy has declined by more than a decade and in a few cases by more than two decades. Even in countries with HIV prevalence of around 5 percent (close to the average for sub-Saharan Africa), the epidemic can reverse gains in life expectancy and other health outcomes achieved over one or two decades. This volume highlights work conducted under the umbrella of a World Bank work program on “The Fiscal Dimension of HIV/AIDS,” including country studies on Botswana, South Africa, Swaziland, and Uganda. It covers four aspects of the fiscal dimensions of HIV/AIDS: First, it aims for a comprehensive analysis of the fiscal costs of HIV/AIDS, with a wider scope than a costing analysis focusing on only the policy response to HIV/AIDS. Second, it embeds the analysis of HIV/AIDS costs in a discussion of the fiscal context, and interprets these costs as a quasi-liability, not a debt de jure, but a political and fiscal commitment that binds fiscal resources in the future and cannot easily be changed, and very similar to a pension obligation or certain social grants or services. Third, it develops tools to assess the (fiscal dimensions of) trade-offs between HIV/AIDS policies and measures that take into account the persistence of these spending commitments. Fourth, most of the fiscal costs of HIV/AIDS are ultimately caused by new infections, and this study estimates the fiscal resources committed (or saved) by an additional (or prevented) HIV infection. Building on these estimates, the analysis here is able to assess the evolving fiscal burden of HIV/AIDS over time.

Book excerpt: One in six adults in sub-Saharan Africa will die in their prime of AIDS. It is a stunning cataclysm, plunging life expectancy to pre-modern levels and orphaning millions of children. Yet political trauma does not grip Africa. People living with AIDS are not rioting in the streets or overthrowing governments. In fact, democratic governance is spreading. Contrary to fearful predictions, the social fabric is not being ripped apart by bands of unsocialized orphan children. AIDS and Power explains why social and political life in Africa goes on in a remarkably normal way, and how political leaders have successfully managed the AIDS epidemic so as to overcome any threats to their power. Partly because of pervasive denial, AIDS is not a political priority for electorates, and therefore not for democratic leaders either. AIDS activists have not directly challenged the political order, instead using international networks to promote a rights-based approach to tackling the epidemic. African political systems have proven resilient in the face of AIDS's stresses, and rulers have learned to co-opt international AIDS efforts to their own political ends. In contrast with these successes, African governments and international agencies have a sorry record of tackling the epidemic itself. AIDS and Power concludes without political incentives for HIV prevention, this failure will persist.
