Book excerpt: Nursing engagement has been linked to many outcomes in the nursing work environment such as patient safety, job satisfaction, increased productivity, patient satisfaction, and decreased patient mortality. The importance of nursing engagement also has been demonstrated to retain nurses. The nursing shortage in the United States emphasizes the need for nursing leaders to focus on staff retention. This project examined the relationship between nursing engagement and the metrics of nursing satisfaction, patient safety and satisfaction, nursing retention, and productivity. The specific problem identified was: In the perinatal services registered nurse population of a suburban Houston hospital, does the level of engagement (isolated by department) positively correlate with nurse retention, staff productivity, nurse and patient satisfaction, and patient safety when compared with nurses that are not engaged in their work during the first quarter of 2017? This project was based on the social exchange theory which proposes that employees receive benefit from the exchange of resources in order to deliver effective patient care. The project utilized a non-experimental, quantitative correlational methodology. Findings were consistent with literature for each department (n=90). The highest engagement and nursing satisfaction were observed to be in the pediatric department which also reported the highest productivity at 103% and retention with 0% turnover. Implications for this project demonstrated that focus on the engagement of nurses can result in increased productivity, increased patient satisfaction as well as improved quality care outcomes such as decreased falls.

Book excerpt: Nurses are leaving the profession due to high levels of job dissatisfaction arising from current working conditions characterized by heavy workloads, limited participation in decision making and lack of development opportunities (Canadian Health Services Research Foundation [CHSRF], 2006a). To gain organizational support for workplace improvements and thereby improve nursing retention, evidence is needed to demonstrate the impact of the work environment on patient care. The purpose of this study was to determine the relationship between nurses' perceptions of their work environment and the quality and risk outcomes for both the patient and the nurse. Kanter's (1977, 1993) theory of structural empowerment guided the study. Empowering work environments for nurses were hypothesized to impact group processes and thereby work effectiveness as reflected in patient outcomes (patient satisfaction, therapeutic self care, falls and nurse-assessed risks). Empowering workplaces were also hypothesized to enhance the nurse's psychological empowerment and, in turn, engagement in empowering behaviours that lead to quality care and job satisfaction. A multi-level cross-sectional design was used to test the study model. Self-report surveys were used for a sample of nurses (n=679) and discharged patients (n=1005) affiliated with medical and surgical units from 21 hospitals in Ontario. Unit characteristics and falls data were obtained from existing hospital databases. Using multilevel structural equation modeling, the hypothesized model fit well with the data (2=21.074, df=10, CFI=.985, TLI=.921, RMSEA=.041, SRMR .002[within] and .054[between]). Empowering workplaces had positive effects on nurse-assessed quality of care and predicted fewer falls and nurse-assessed risks as mediated through group processes. These conditions positively impacted individual psychological empowerment which, in turn, had significant direct effects on empowered behaviour, job satisfaction and care quality. Theoretically, evidence supported the further evolution of structural empowerment theory to include group processes and empowered behaviour as mediators to various nurse and patient outcomes. The evidence from this study also reinforced the critical need to invest in improving nursing work environments for the benefit of patients and nurses. Theory-informed strategies for changes to the workplace have the potential to mitigate against projected nursing shortages and ensure a sustainable workforce to meet future demands for care.

The Medical-Telemetry Unit has had a decrease in patient satisfaction, increase in staff member turnover and lower morale levels, within the last year. The purpose of this proposal is to study the relationship of implementing a Unit-Based Resource Nurse program with the factors related to improved nurse satisfaction. Such contributing factors noted were those of mentoring, support, staff member and patient safety, inter-professional collaboration along with break and meal coverage. A literature search was conducted to assist with identifying factors that contribute to nurse satisfaction, as well as data retrieved from patient satisfaction surveys, staff member time card reports and safety reporting systems. Staff member satisfaction will be evaluated with monthly surveys, over the course of three months. The nursing implications, of the implementation of this program, may improve staff member turnover, nurse engagement, improved inter-professional collaboration and may reduce caregiver fatigue. When these factors are considered, better patient outcomes may be produced.

Book excerpt: The lack of an effective leadership style has been recognized as a leading contributor of reduced job satisfaction and high employee turnover. In the health care sector, this has been shown to lead to negative patient outcomes. The purpose of this DPI project was to establish the relationship between leadership style and the nurses' job satisfaction and retention, and the patient outcome of falls reduction in a long-term care unit in Chicago. The clinical questions sought to determine how the transformational leadership (TL) style, through the intervention of education/training impacted the other variables. The independent variable was the implementation of TL, while the dependent variables included the nurses' job satisfaction, retention, and the patient outcome of reduction in falls. The project employed two theories; Jean Watson's theory of human caring and Patricia Benner's novice to expert nurse theory. The sample consisted of 30 nurses (N=30) from the facility who completed the pre and post-test surveys on the NWI-R questionnaire. Using the Statistical Package for Social Sciences (SPSS) for data analysis, the findings indicated that the perception of nurses towards their leaders and working conditions improved with TL. In the three months after implementation, the number of reported patient falls reduced from 38 to 13. The test for significance was p=0.888, indicating a positive correlation between the variables. The project recommended that health care organizational leadership should understand and adopt the characteristics of TL since it enhances nurses' morale and performance leading to job satisfaction, which may lead to an increase in retention, and may lead to improved patient outcomes.

Hospitals that achieve Magnet recognition have better nurse retention rates, decreased turnover, and increased overall nurse satisfaction. In addition, these hospitals have better patient outcomes and patient satisfaction. Evidence shows that the characteristics of Magnet designation, transformational leadership, structural empowerment, exemplary professional practice, and new knowledge, innovations, and improvements result in this increase in satisfaction levels. An evidence based practice program to achieve Magnet recognition in an organization must be supported by nurse leaders, staff nurses, and hospital administrators. The process is long, complex, and a great financial commitment. It requires all members to be fully involved and committed to change. Organizational change theory may be used to convince the organization to adopt change by presenting the benefits in a clear and concise manner with measurable outcomes. The implementation program created, Get Magnetized, will assist the institution in achieving Magnet recognition. Stakeholders should be involved in the implementation and evaluation processes. Ongoing evaluation is necessary to achieve and maintain the characteristics of Magnet recognition and the success of the program.

Over the past decade, the measurement of patient satisfaction has led to an increased appreciation of the significance of the patient experience, their expectations and perceptions of the care they receive. Patient satisfaction is no longer simply the "nice" or "right thing" to do if you have time. In the current highly competitive environment, it is a necessary business choice. Satisfied customers are loyal and may be counted on for repeat business or for recommendations of the hospital leading to increased revenue, market share, profitability, and better clinical outcomes. Patient satisfaction, patient safety, and clinical outcomes all go together to formulate the patient experience and should not be viewed separately. The author like other hospital nursing administrators plans to focus the care delivery model around the patient experience. The setting for this study is a 49 bed acute care community hospital. A relationship-based care delivery model, incorporating patient centered care and nurse caring will be implemented on a 36 bed medical/surgical unit. Staff on this unit has been delivering standard care based on a biophysical model emphasizing clinical diagnosis and treatment. Patient satisfaction scores have been below goal as have the scores on the HCAPHS survey. Patient satisfaction has become an important indicator of quality care and financial success of healthcare institutions. Hospital administrators are focusing their delivery of medical and surgical services around the patient experience. The importance of nurse caring behaviors and that nurse caring is an important predictor of patient satisfaction needs to be acknowledged (Dingman et aI., 1999). Satisfaction with nurse caring is a measure of organizational effectiveness in creating a caring environment and meeting patient's expectations. Patients' expectations are those responses that are situation specific, influenced by environmental factors, experience, and properties of the situation. Nurse socialization regarding caring behaviors and attitudes and displaying them is needed to have a service-oriented culture. The match between patient expectations of nurse caring and the caring actually received is expressed as patient satisfaction (Greeneich, 1993). No universal definition of caring exists but Watson (1985) defined caring as a fundamental value that guides nurses' ethical decision making and provides a basis for nurse caring actions. Nurses provide direct patient care and spend more time with patients than any other healthcare worker and therefore, need to be concerned about meeting patients' needs in a caring manner. In recent years, the model of patient care shifted from a standardized, one-size-fits-all to a patient-centered approach, with the aim of improving the quality of care (McLaughlin & Kaluzny, 2000). Patient-centeredness goes beyond clinical care and provides the patient with a very personal experience. Scalise (2003) notes the following eight dimensions that encompass a complete patientcentered experience: Access to care, continuity and transition to home, coordination of care, emotional support, information and education, involvement of family and friends, physical comfort, and respect for patient preferences. Patient-centered care has been defined as health care that establishes a partnership among practitioners, patients and their families (when appropriate) to ensure that decisions respect patients' wants, needs and preferences and solicit patients' input on the education and support they need to make decisions and participate in their own care (Kohn, Corrigan & Donaldson, 1999).
