Book excerpt: In an effort to improve breastfeeding, the Oregon WIC Program tested whether a relatively low-cost telephone peer counseling initiative to support breastfeeding could increase the initiation and duration of exclusive breastfeeding among its participants. They conducted a large randomized field experiment (RFE) with over 1900 women from four WIC agencies in the state. In this study we use data from the RFE along with administrative data from the rest of the state to assess whether the results from the RFE can be extended to other agencies in the state. We find small or non-existent effects of peer counseling in the non-experimental settings, which suggest that the experimental estimates may reflect Hawthorne effects. We present evidence of selection into RFE in that exclusive breastfeeding among the controls is significantly greater than among women who were offered but declined to participate in the RFE as well as from women in the rest of the state who had no access to peer counseling. We conclude that despite the strong internal validity of the RFE, extending the program to other agencies in the state would have a limited impact at best on exclusive breastfeeding.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

That is, until researchers began to ponder the Asian Enigma- the question of why malnutrition is much more prevalent among children in South Asia than in Sub-Saharan Africa, even though South Asia surpasses Sub-Saharan Africa in most of the principal determinants of child nutrition. This report uses data from 36 countries in three developing regions to establish empirically that women's status, defined as women's power relative to men's, is an important determinant of children's nutritional status. It finds that the pathways through which status influences child nutrition and the strength of that influence differ considerably from one region to another. Where women's status is low, this research proves unequivocally that policies to eradicate gender discrimination not only benefit women but also their children.

Knowledge of the motivators and deterrents of EBF can facilitate development of strategies to promote EBF. We use the `Theory of Planned Behavior' (TPB) to understand factors and identify strategies that influence women's beliefs, attitudes, perceived power, intentions and actual practice of EBF. We conducted 22 focus group discussions with women and men enrolled in a before-and-after trial to determine whether intensified breastfeeding counseling enhanced EBF. The constructs of TBP were used to deductively identify prominent themes. Findings indicate that breastfeeding is not a problem; rather the exclusive part of it is challenging. Reasons for not exclusively breastfeeding: lack of education; compliance with cultural norms; and negative beliefs and attitudes to breastfeeding. Access to health education, peer counseling, desire to prevent infant HIV transmission, and male partner support motivate women to exclusively breastfeed. Consistent with the constructs of TPB, strategies that foster favorable beliefs and attitudes toward EBF ultimately promote women's intentions to practice EBF. Introducing interventions that enhance positive attitudes toward EBF could potentially increase EBF practice.
