Book excerpt: Les neuroleptiques sont fréquemment prescrits chez les patients atteints de maladie neurodégénérative présentant des symptômes psycho-comportementaux. Cependant, ces traitements sont nocifs dans cette population et ont une efficacité très modeste. Devant ce constat, la prise en charge des maladies neurodégénératives par les thérapies non pharmacologiques est en plein essor. La musicothérapie suscite un intérêt croissant dans le traitement de ces symptômes. L'objectif de l'étude est de montrer que la musicothérapie permet de diminuer un traitement neuroleptique chez des patients atteints de syndrome démentiel compliqué d'agitation. Méthode : Essai comparatif, contrôlé, randomisé en deux groupes, en simple aveugle, multicentrique. Le groupe musicothérapie (n=17) bénéficie de 3 séances de musicothérapie réceptive par semaine pendant 8 semaines. Le groupe contrôle bénéficie dans les mêmes conditions de séances de discussion autour de l'actualité ou de photos personnelles. La posologie de neuroleptique est notée à l'inclusion, à 1 mois, à 2 mois du traitement et 1 mois après l'arrêt des séances. Nous avons évalué l'agitation à travers les échelles Cohen-Mansfield et NPI-ES. Résultats : Il n'y a pas de différence significative en terme de réduction de posologie de neuroleptique entre le groupe expérimental et le groupe contrôle (p=0,69). Dans les 2 groupes, on observe que l'aire sous la courbe, qui est le reflet de la variation de posologie du neuroleptique est de 0,9 ce qui signifie qu'en moyenne sur la durée de l'étude la posologie est légèrement inférieure à la posologie initiale. On observe une amélioration significative des scores à l'échelle NPI dans le groupe contrôle (p=0,007) mais pas de différence significative entre les 2 groupes à l'échelle Cohen-Mansfield (p=0,11). Conclusion : Dans notre étude, nous n'observons pas de réduction significative de neuroleptique dans le groupe expérimental comparé au groupe contrôle. Nous avons cependant montré la faisabilité de notre étude et sa reproductibilité. Notre travail doit être complété par d'autres études à plus grandes échelles.

Book excerpt: Objectif : Évaluer les effets de la musicothérapie réceptive, individuelle et personnalisée sur les troubles psycho-comportementaux, chez les personnes atteintes de la maladie d'Alzheimer, à un stade modéré à sévère, et résidents en EHPAD. Type d'étude : Il s'agissait d'une étude observationnelle, prospective, bicentrique, contrôlée, randomisée, en simple aveugle, en groupe parallèle avec un groupe bénéficiant de séances de musicothérapie réceptive et un groupe témoin recevant les soins standard. La durée d'étude était de 12 semaines avec une période de suivi de quatre semaines. Méthode : 47 patients ont été randomisés en deux groupes. Un groupe témoin et un groupe recevant des séances de musicothérapie, d'une durée de 30 minutes, trois fois par semaine, pendant 12 semaines avec un période de suivi de quatre semaines. Notre critère de jugement principal était le NPI, mesuré avant, à la fin des séances, et quatre semaines après la fin des séances. L'objectif secondaire était d'évaluer si les séances de musicothérapie réceptive pouvaient faire diminuer la prescription de psychotropes. Résultats : Nous avons observé une diminution du NPI de façon statistiquement plus importante dans le groupe musicothérapie comparé au groupe témoin (p=0.048). L'effet bénéfique de ces séances avait tendance à persister quatre semaines après la fin des séances (p=0.029). En revanche, il n'a pas été mis en évidence de diminution de prescription des psychotropes. Conclusion : La musicothérapie réceptive a permis de faire diminuer la fréquence de survenue des troubles psycho-comportementaux mais pas de diminuer la prescription des psychotropes.
