Book excerpt: INTRODUCTION : Les Services d'Accueil des Urgences (SAU) en France doivent faire face à un afflux de patients en constante augmentation. Le poste d'Infirmier d'Accueil et d'Orientation (IAO) est indispensable pour évaluer, au sein de flux souvent massifs, la gravité des patients afin d'assurer un délai de prise en charge médicale en adéquation avec la pathologie rencontrée. Cependant, ce poste est décrit comme éprouvant par les soignants, car en tant que premier interlocuteur, l'IAO doit souvent faire face à l'inquiétude et parfois l'agressivité des patients et de leur entourage ; manifestations souvent liées aux délais d'attente très longs, conséquence directe de la surcharge de travail des SAU. Tous ces phénomènes exposent au risque non négligeable de démotivation et d'épuisement des IAO, ce qui pourrait, à court terme, être délétère sur la prise en charge des patients. Notre étude avait pour but d'évaluer l'impact de la mise en place d'un Médecin d'Accueil et d'Orientation (MAO) posté à l'accueil des urgences, sur le ressenti des IAO dans différents aspects de leur travail mais aussi dans leur relation avec les patients et leurs accompagnants. MATERIEL ET METHODE : Un MAO a été mis en place à partir de janvier 2019 aux urgences adultes du CHR Orléans. Les IAO ont été interrogés par le biais de 15 questions auxquelles ils devaient attribuer une note de 1 à 10. Ce questionnaire a été rempli par 33 IAO, lors de 2 phases, chronologiquement distinctes, à savoir une première phase en décembre 2018 et une deuxième en mai 2019. Il a également été demandé aux IAO de s'exprimer sur leur ressenti de l'utilité du MAO. Nous avons ensuite procédé à une analyse descriptive de notre population d'IAO et de leurs réponses, puis à l'évolution de celles-ci entre les 2 phases, par des tests de comparaisons de moyennes. Enfin nous avons effectué des analyses en sous-groupes selon les caractéristiques de la population (âge, sexe, ancienneté d'exercice de la fonction d'IAO, type d'exercice (100% SAU versus 50% SAU-50% SMUR). RESULTATS Notre population d'IAO a une moyenne d'âge de près de 32 ans et une moyenne d'ancienneté au poste d'IAO d'un peu plus de 6 ans. 52% ont un poste d'infirmière partagé 50% SAU-50% SAMU alors que 48% ont un exercice 100% SAU. Avant la mise en place du MAO, avec des moyennes d'environ 7, les IAO se sentent dépassées par la charge de travail et considèrent ce poste comme épuisant. Cela ne les empêche pas d'apprécier ce poste et de se sentir utiles pour les patients et le service. La mise en place du MAO améliore le ressenti des IAO sur tous les points recensés, mais ce, de manière significativement plus marquée pour les IAO les plus jeunes et celles exerçant uniquement au SAU. 70% des IAO considèrent le poste de MAO comme indispensable. CONCLUSION : La mise en place d'un MAO aux urgences adultes du CHR d'Orléans a permis d'augmenter le ressenti favorable des IAO, en majorant leur satisfaction personnelle à travers une meilleure gestion de l'orientation des patients, une diminution du sentiment d'épuisement et d'isolement. Cela a démontré aussi une diminution du ressenti d'insécurité et d'agressivité des patients et de leur entourage. Ces améliorations plus marquées dans la population des IAO les plus jeunes et exerçant à temps plein au SAU, soulignent l'aspect éprouvant de ce poste et incitent, au-delà de la mise en place du MAO, à réfléchir sur un possible fractionnement des temps de travail sur une même journée, afin de diminuer l'épuisement et majorer l'efficacité des IAO.

Book excerpt: By modeling pedagogical scenarios as directed geometrical graphs and proposing an associated modeling language, this book describes how rich learning activities, often designed for small classes, can be scaled up for use with thousands of participants. With the vertices of these graphs representing learning activities and the edges capturing the pedagogical relationship between activities, individual, team, and class-wide activities are integrated into a consistent whole. The workflow mechanisms modeled in the graphs enable the construction of scenarios that are richer than those currently implemented in MOOCs. The cognitive states of learners in two consecutive activities feed a transition matrix, which encapsulates the probability of succeeding in the second activity, based on success in the former. This transition matrix is summarized by a numerical value, which is used as the weight of the edge. This pedagogical framework is connected to stochastic models, with the goal of making learning analytics more appealing for data scientists. However, the proposed modeling language is not only useful in learning technologies, it also allows researchers in learning sciences to formally describe the structure of any lesson, from an elementary school lesson with 20 students to an online course with 20,000 participants.
