Longtemps cantonnée aux milieux aisés et aux usagers très marginalisés, sa consommation s'est élargie à d'autres milieux plus modestes. Pour un chirurgien-dentiste, parler de drogue avec ses patients est une démarche aujourd'hui nécessaire. Aucun d'eux n'échappe ou n'échappera, au cours de son exercice, à ce problème de société qu'est l'addiction. En plus des répercussions psycho-sociales négatives qu'elle implique, la consommation de drogues telles que le crack et la cocaïne confronte le chirurgien dentiste à des pathologies générales qu'il devra prendre en compte dans l'organisation des soins et l'établissement de ses prescriptions. Au niveau de la sphère buccale, la substance en elle-même et les conséquences de sa consommation sur l'hygiène, l'immunité et la nutrition induisent des lésions et complications. Celles-ci touchant aussi bien les tissus durs avec des caries multiples, des érosions et abrasions dentaires, un bruxisme ; que les tissus mous avec des atteintes gingivales et des pathologies de la muqueuse orale. Dans les cas de consommation chronique, des nécroses septo-platines peuvent être rencontrées. Les chirurgiens dentistes sont pour leurs patients, dans le cadre de soins prodigués, des interlocuteurs privilégiés sur ce sujet. En outre, ils peuvent contribuer à rendre aux patients une estime d'eux-mêmes indispensables à leur rétablissement. Ce travail propose une description des manifestations générales et buccales associées à la consommation de cette substance, des précautions à prendre au cabinet, des mesures de traitement et de prise en charge existantes et en perspective, en terminant par l'étude descriptive d'un groupe d'usagers martiniquais ; la Martinique étant le premier département français consommateur de crack, dérivé de la cocaïne.

L'état bucco-dentaire des personnes utilisant cette drogue se dégrade très rapidement. La prise en charge au cabinet dentaire de consommateur de méthamphétamine est particulière. Effectivement, les polycaries sont récurrentes chez ces patients dues au manque d'hygiène bucco-dentaire, à l'utilisation quotidienne de boissons sucrées en plus de l'effet de cette drogue. Ce travail a pour but d'étudier les manifestations bucco-dentaires de la méthamphétamine, ainsi que la prise en charge du patient consommateur au cabinet dentaire.
