Book excerpt: Contexte : L'optimisation échographique des délais atrio-ventriculaire (AV) et inter- ventriculaire (W) pourrait améliorer le taux de réponse à la resynchronisation cardiaque (CRT) mais son utilisation en routine fait débat. Objectif : Décrire un programme hospitalier d'optimisation clinique et échographique de CRT en « vie réelle », et évaluer son impact sur la réponse clinique. Méthode : Tous les patients implantés de CRT à l'hôpital Henri Mondor (Créteil, France) ont été reconvoqués pour optimisation à 3-12 mois. Le programme comprenait une évaluation clinique et biologique, un dépistage standardisé des causes potentielles de non-réponse et une optimisation échographique systématique des délais AV et VV. La réponse initiale à la CRT était définie par critère clinique composite, et l'amélioration à 6 mois de l'optimisation a été évaluée prospectivement selon le même critère. Les événements majeurs de l'insuffisance cardiaque (IC) ont été suivis pendant un an après optimisation. Résultats : Un total de 227 patients admis pour optimisation de CRT ont été inclus (âge 71 ± 11 ans, hommes 77%, FEVG 30.6 ± 7.9%, délai médian depuis l'implantation 5.4 mois), dont 111 (48.9%) non-répondeurs initiaux. Plusieurs complications ont été dépistées : déplacement de sonde ventriculaire gauche (n = 4), perte de capture biventriculaire ou taux de capture

Book excerpt: RATIONNEL L’insuffisance cardiaque est devenue un important problème de santé publique par son incidence qui ne cesse d’augmenter ainsi que par le coût de sa prise en charge. En corrigeant l’asynchronisme cardiaque, conséquence de la fibrose et du remodelage ventriculaire, la stimulation biventriculaire permet d’améliorer les symptômes et de diminuer la mortalité des patients insuffisants cardiaques, sous traitement médical optimal. OBJECTIF Vingt à trente pour cent des patients ne répondent pas à cette thérapeutique. La présence d’un asynchronisme mécanique est reconnue comme un important facteur prédictif de réponse. Cependant, il semble que cette condition ne soit pas suffisante pour prédire le maximum de répondeurs. En effet, la présence d’une fibrose et d’une viabilité sur la paroi ventriculaire gauche stimulée serait une donnée importante à prendre en compte. METHODOLOGIE Dix-neuf patients insuffisants cardiaques porteurs d’un stimulateur cardiaque biventriculaire ont été étudiés (neuf cardiopathies ischémiques). Une échographie avec mesures d’asynchronisme et de débit cardiaque a été effectuée (en mode spontané et en entraînement biventriculaire) et une réserve contractile sur la paroi stimulée a été recherchée par échographie sous faibles doses de dobutamine (5?/kg/min). RESULTATS La resynchronisation permet une amélioration du volume d’éjection systolique de 10% (p=0,02), cependant cette amélioration n’est présente qu’en présence d’un asynchronisme mécanique, intra ou inter-ventriculaire. Par ailleurs, on observe une diminution de ces délais qu’en présence d’une réserve contractile sur la paroi stimulée. De plus, l’amélioration hémodynamique n’est significative qu’en présence d’une réserve contractile. CONCLUSION La mise en évidence d’un asynchronisme mécanique est une étape indispensable avant une resynchronisation cardiaque, mais il semblerait qu’une réserve contractile pourrait, si elle est présente, permettre d’encore mieux sélectionner les patients répondeurs
