L'objectif de cette étude est d'évaluer l'impact cardiorespiratoire, anthropométrique et fonctionnel d'un programme de réentrainement à l'effort standard chez une population atteinte de pathologies rhumatologiques diverses, et plus spécifiquement les patients atteints de spondyloarthrite (SPA) et de fibromyalgie. Méthode : cette étude observationnelle a inclus l'ensemble des patients suivis pour une pathologie rhumatologique chronique et ayant intégré le programme de reconditionnement à l'effort à l'hôpital Jean Verdier entre Mars 2017 et Juillet 2020. Les patients étaient évalués en début de programme (MO), fin (M3) et 3 mois après la fin (M6). Le critère principal de jugement était la variation du pic de consommation d'oxygène à l'épreuve d'effort (V02 pic) entre MO et M3. Les critères secondaires comprenaient des paramètres anthropométriques, cardio respiratoires à l'effort et au repos, des tests fonctionnels et le questionnaire G-PAQ. Résultats : Soixante-quatre patients ont été évalués à MO, 47 sont revenus à M3 et 20 étaient présents à l'évaluation de M6. L'activité physique en prévention primaire a prouvé son efficacité avec la COVID-19. À notre connaissance, rare sont les essais contrôlés menés en prévention secondaire et tertiaire et il n'y a pas de conclusion claire pour le moment sur l'efficacité de l'exercice physique en post-COVID. Objectif. Comparer l'amélioration des performances physiques des patients COVID-long sans exacerbation de symptômes post exercice, après une réhabilitation physique avec des patients coronariens et fibromyalgiques Méthodes. Un suivi prospectif d'une population de COVID-long a été comparé à deux groupes contrôles : coronariens et fibromyalgiques lors d'une réhabilitation à l'exercice de quatre semaines, à raison de trois sessions par semaine. Les paramètres de performances en aérobie [consommation maximale d'oxygène (VO2), puissance maximale aérobie et premier seuil ventilatoire], les paramètre de performances en anaérobie (force musculaire du biceps) et les paramètres du système nerveux autonome (baroréflexe et variabilité de la fréquence cardiaque) ont été évalués avant et après intervention. Résultats. Au total, 38 patients COVID-long ont été recrutés et comparés à 38 patients coronariens et 38 patients fibromyalgiques. Les paramètres cardiorespiratoire et musculaire ont été améliorés après un entrainement adapté dans les 3 groupes. Aucun des patients du groupe COVID-long n'a présenté de PESE. Aucune différence significative n'a été observée à la fin de l'expérimentation pour le système nerveux autonome. Les patients ont été globalement tous satisfaits après le programme de réhabilitation à l'exercice. Discussion. À travers cette étude comparative avec groupe contrôle, nous confirmons et renforçons l'intérêt de soins aux patients COVID-long avec une réhabilitation par des exercices de force et d'endurance, en adaptant et personnalisant le programme d'exercice au patient et à ses symptômes.

L’évolution des modes de vie a modifié les comportements des individus en matière d’activité physique. Plusieurs études ont montré une insuffisance d’activité physique favorable à la santé chez plus de la moitié des Français et des difficultés à rendre les inactifs actifs et à les autonomiser dans la pratique d’une activité physique. Objectifs : L’objectif principal était d’évaluer la satisfaction d’un programme d’activité physique adaptée chez des patients atteints de maladies chroniques, dans l’hypothèse de le rendre pérenne, par l’élaboration d’un score de satisfaction. Les objectifs secondaires étaient d’évaluer l’impact du programme sur le poids, l’IMC, le comportement en activité physique et la qualité de vie. Matériel et méthodes : Etude d’évaluation de type « avant-après » d’un programme d’activité physique adaptée, effectuée entre janvier 2010 et juin 2011, auprès de 33 patients atteints de maladies chroniques. Recueil des données par questionnaires (satisfaction, poids, IMC, activité physique et sédentarité, qualité de vie). Résultats : 67.7% des patients ont eu un score de satisfaction maximal. Il n’y a pas eu de différence constatée pour le poids et l’IMC. Le score global d’activité physique a augmenté significativement (p=0.0055). La fréquence de la pratique du sport a augmenté significativement (p=0.0238) mais il n’y a pas eu de différence pour la fréquence de pratique de l’activité physique dans ses autres composantes et la fréquence des activités sédentaires. Le score de qualité de vie mental et social a diminué (p=0.0095) alors qu’il n’a pas été constaté de différence pour le score de qualité de vie physique. Conclusion : Le programme d’activité physique adaptée à majoritairement satisfait les patients et a permis une autonomisation dans la pratique de l’activité physique, par le biais du sport, et particulièrement du sport adapté.

Book excerpt: Les patients atteints de maladies rhumatologiques chroniques ont souvent une activité physique insuffisante alors que la participation à une activité physique régulière est un point-clé de leur traitement. De nombreux freins empêchent cette activité et les stratégies visant à augmenter durablement le niveau d'activité physique dans cette population ont une efficacité très modeste. Dans cette thèse, nous nous sommes attachés à 1) identifier et évaluer les déterminants de l'activité physique et 2) évaluer des stratégies permettant d'augmenter le niveau d'activité chez les patients atteints de pathologies rhumatologiques chroniques. Le premier axe de la thèse nous a permis de développer et valider un questionnaire de 10 barrières et facilitateurs à l'activité physique pour les patients atteints de rhumatismes inflammatoires chroniques. Ce questionnaire peut être utilisé en pratique clinique pour identifier les principaux déterminants de l'activité physique et pour construire des programmes favorisant le maintien d'une activité adaptée avec une meilleure chance d'adhésion sur le long cours. L'observation du comportement d'adhésion au médicament et à l'activité physique chez 152 patients atteints de rhumatismes inflammatoires chroniques nous a permis de constater une absence de corrélation entre les deux. Ceci démontre que les déterminants de l'activité physique conditionnant le mode de vie sont surement plus complexes que ceux de la prise de médicament. Le 2ème axe de cette thèse nous a permis de constater par le biais d'une revue systématique que l'utilisation de traceurs d'activité entrainait un bénéfice de 1500 pas par jour. L'utilisation de ces dispositifs est encouragée au vu de leur efficacité et du faible coût d'utilisation, bien que leur efficacité à long terme reste à démontrer. Une dernière étude nous a permis d'observer que chez les patients présentant des freins importants à l'activité physique, l'utilisation d'une stratégie supervisée multidisciplinaire et adaptée aux disponibilités des participants apportait un bénéfice sur la santé. Les futures stratégies gagneront à combiner les interventions supervisées et les dispositifs technologiques pour plus d'efficacité. L'enjeu actuel de l'activité physique en rhumatologie n'est plus de démontrer sa pertinence mais d'identifier les stratégies permettant son maintien sur le long cours. À l'avenir, d'autres stratégies gagneront à être étudiées pour favoriser le maintien de l'activité physique, telles de l'utilisation d'applications smartphone ou l'entretien motivationnel. La prescription d'activité physique gagnera également à être développée via la formation des médecins prescripteurs et des professionnels de santé à l'utilisation de techniques éprouvées.

The unique needs of dying children are particularly evident across Africa, where the scale of the problem is overwhelming, and the figures so enormous that they are barely comprehensible: over 400,000 children in Africa died from AIDS in 2003, and out of the 166,000 children a year diagnosed with cancer, 85% of these are in the developing world. Despite the enormous need, provision of children's palliative care in Africa is almost non-existent, with very few health workers trained and confident to provide care for dying children. The challenges of providing palliative care in this setting are different to those in more developed countries, contending with the shortage of physical and human resources in addition to the vast scope of the care needed. Written by a group with wide experience of caring for dying children in Africa, this book provides practical, realistic guidance by improving access to, and delivery of, palliative care in this demanding setting. It looks at the themes common to palliative care--including communication, assessment, symptom management, psychosocial issues, ethical dilemmas, end of life care, and tips for the professional on compassion and conservation of energy--but always retains the focus on the particular needs of the health care professional in Africa.
