Book excerpt: Les édentements étaient initialement réhabilités selon des techniques de prothèse fixée ou amovible, tributaires des piliers dentaires. Ces dernières devaient s'adapter à la distribution des dents résiduelles au sein de l'arcade et à leur caractéristiques biologiques. Les facteurs de risque pour une dent augmentent en fonction de la complexité de la réhabilitation dont elle sera pilier. On procède donc à une étude préprothétique à travers la détermination de la valeur stratégique de la dent et l'évaluation de son pronostic à travers les paramètres endodontique, parodontal et prothétique. L'analyse discriminante de ces différents facteurs aiguille le praticien vers un choix de conservation ou d'extraction et permet d'affiner la décision thérapeutique. La prothèse conventionnelle est donc élaborée selon une notion de compromis entre les impératifs biomécaniques induits par let ype et l'étendue de l'édentement, les impératifs biologiques des dents piliers disponibles et les impératifs esthétiques dictés par le patient. Une nouvelle alternative s'offre désormais au praticien et lui permet de s'affranchir de certains compromis et de certaines contraintes liés à ces techniques conventionnelles. Il a la possibilité d'ajouter un nombre limité d'implants situés à des endroits stratégiques pour soutenir et/ou retenir une prothèse partielle amovible. Les implants remplacent des dents au pronostic douteux et/ou se placent en complément de la denture résiduelle pour optimiser les concepts de la triade de Housset et permettre un meilleur équilibre de la prothèse. La PAPSI permet d'améliorer significativement le confort et de simplifier la conception prothétique. Ces implants peuvent être liés à la prothèse amovible par le biais de différents connecteurs tels qu'un pilier de cicatrisation, une couronne fraisée, une couronne télescope ou une attache de précision.
