Book excerpt: The aetiology of the chronic inflammatory bowel diseases - Crohn's disease and ulcerative colitis - is still enigmatic. The therapeutic approach has therefore traditionally focused on anti-inflammatory principles, including corticosteroids and aminosalicylates. Since a significant proportion of patients is steroid-dependent or refractory and because of the problematic side-effects of long-term systemic steroids, active immunosuppression has gained acceptance in the field. The classical immunosuppressants azathioprine and 6-mercaptopurine have long been evidence-based in IBD, but underused. Recently, methotrexate and cyclosporine have also been proven to be effective in certain situations. Newer drugs like tacrolimus, mycophenolate and others may be similarly useful but their potential is still unclear. Finally, the immune modifiers including IL-10 and TNF-antibodies have been successfully subjected to controlled trials. Other experimental drugs discussed in this book are on the horizon. This volume is the Proceedings of Falk Symposium 119 held in Freiburg-im-Breisgau, Germany, October 3-4, 2000, and covers systematically the field of immunosuppression in inflammatory bowel diseases. It is hoped that it will further the prudent use of these drugs in the proper clinical situations by increasing our understanding of both the mechanisms of action and the clinical benefit to the patient.

The senior surgeon at Mount Sinai Hospital in New York City, the same institution at which Burrill Crohn, Leon Ginzburg, and Gordon Oppenheimer had first described the disease “terminal ileitis,” had - doubtedly done countless operations on patients with inflammatory bowel disease in the past. Yet as we both gazed down into the patient’s open abdomen, at the “creeping fat” that seemed to be wrapping its sticky fingers around the young man’s intestines, he stated, “this is the m- tery of Crohn’s disease—no two patients are ever the same. ” What is it about the inflammatory bowel diseases, Crohn’s disease, and ulcerative colitis, that we find so intriguing? Is it the young age of the patients, many who are younger than even the medical students - tending to them? Or is it the elusive etiology, the theory of a “mystery organism” that has yet to be identified? The format of the Falk Symposium 106 on `Advances in Inflammatory Bowel Diseases', held in Brussels, Belgium, June 18-20, 1998, was somewhat innovative as each session attempted to link the new insights into pathogenetic mechanisms with new therapeutic approaches, resulting in optimal information transfer. The classic therapeutic schemes were updated with a special focus on step-wise build-up of therapy.

This book encompasses the proceedings of two recent symposia held in Victoria, British Columbia, Canada. The symposia were the fifth and sixth international meetings focused on inflammatory bowel disease in Canada in the last eight years. Once again they were sponsored by Axcan Pharma, Inc. (formerly Interfalk Canada, Inc.) and endorsed by the Canadian Association of Gastroenterology. As has become traditional at such meetings the faculty was drawn from an international roster ofleaders in the field of inflammatory bowel disease and gastroenterology. The chapters of the proceedings provide a timely, up-to-date review of the major issues, including those within the realm of basic science and others dealing with clinical problems. The first symposium, 'Basic Research and Clinical Implications', was co-ordinated by John Wallace in association with Stephen Collins and Stephan Targan. The themes of this section were organized under the general topics of predisposing factors (genetics, animal models, infection, permeability, and immune deficits) and the pathophysiology of intestinal inflammation. The second symposium, Trends in Therapy' was organized by Lloyd Sutherland along with Franc;:ois Martin, Robin McLeod and Noel Williams.

Book excerpt: Inflammatory bowel disease (IBD) encompasses a spectrum of autoimmmune diseases which include ulcerative colitis, Crohn’s disease, and indeterminate colitis. Ulcerative colitis is a relapsing nontransmural inflammatory disease restricted to colon, which may be classified as proctitis, left-sided colitis, or pancolitis depending on the extent of involvement. Some patients also develop ileal inflammation (backwash ileitis), which occasionally complicates its differentiation from Crohn’s ileocolitis. Crohn’s disease is defined as a relapsing transmural inflammatory disease of gastrointestinal mucosa that may affect the entire gastrointestinal tract (GIT) from the mouth to the anus. There is discontinuous involvement of various portions of the GIT and may get complicated by formation of strictures, abscesses, or fistulas. It closely resembles intestinal tuberculosis both clinically and on investigations. GI tuberculosis is often suspected on the basis of its clinical, radiological, and endoscopic features, but histological or microbiologic proof of the disease is often difficult to achieve. Therefore, the majority of patients are treated with antitubercular drugs on presumptive diagnosis only. Similar clinical, endoscopic, radiological, and histological features of GI tuberculosis and Crohn’s disease pose a serious challenge to the treating physician. The rate of misdiagnosis of these conditions using conventional diagnostic measures has been reported to be around 50–70%. Thus differentiation and diagnosis of these conditions is difficult but very important for correct treatment of the patients. Recently some serological tests have come up for diagnosing Crohn’s disease and ulcerative colitis, which may help to distinguish these disorders from each other and from GI tuberculosis. Besides diagnosis, treatment for IBD is also an emerging field in which active research is going on. The traditional drugs used in treatment of IBD include steroids and aminosalicylates. Recent studies have evaluated the role of azathioprine, methotrexate, and cyclosporine. Apart from improving efficacy of therapy in short term, these agents have raised debate on complications of therapy, long-term efficacy, and cost. Management of IBD during pregnancy and fistulising Crohn’s disease are other areas of concern.
