This subject has always been in the public domain, accentuated by dramatic appeals, from time to time, to search for matched volunteer donors in the public at large. Unfortunately, the availability of such donors is limited, due to the remarkable genetic diversity of humans. Thus, although registries of such volunteers now include more than seven million individuals, we still face the problem of finding a matched donor for about 30% of patients in need. To address this burning issue, extensive clinical and basic research is performed in leading institutes around the world. This book presents updated accounts of the different aspects of this research. The scope of the book is very wide, including strategies to overcome graft vs. host (GVH) disease and graft rejection, cell therapy to prevent leukemia relapse, and a range of modalities to improve immune reconstitution after transplantation. In addition, new approaches to induce immune tolerance towards organ transplants by means of hematopoietic stem cell transplantation is extensively reviewed.

Book excerpt: In the last decade, remarkable advances have been made in bone marrow transplantation (BMT), which is now becoming a powerful tool in the treatment of diseases such as leukemia, aplastic anemia, and congenital immunodeficiency. In animal experiments, it has been found that BMT can be used to treat not only systemic autoimmune diseases but also organ-specific autoimmune diseases. In humans, it has recently been shown that rheumatoid arthritis, ulcerative colitis, and Crohn's disease can be successfully treated after BMT. This volume contains new information on how to prevent graft rejection, how T cell functions can be completely restored, and how concomitant BMT can prevent the rejection of organ allografts without the use of immunosuppressive agents. BMT will become an increasingly useful and powerful treatment for various currently intractable diseases, and this book will contribute by providing details of the latest research in the field.

Information for those reviews was mainly derived from the Hiroshima and Nagasaki experience (3), from radiation accidents as a con sequence of testing nuclear weapons and the application of nuclear energy (9-13) and from the intentional irradiation of patients, mostly for the treatment of malignancies and/or as conditioning for transplantation (14-20). Further, a large body of data derived from animal experimentation has led to cautious extrapolations to the human situation (14). An elaborate review dealing with early somatic effects of radiation in a man was published by UNSCEAR ) in 1962 (2). It was supplemented in 1969 with a similar document dealing mostly with radiation-induced chromosome aberra tions, the effects of radiation on the nervous system and, radio-active contamination of the environment by nuclear testing (21). In a more recent UNSCEAR report (1972) the main topics were genetic effects, carcinogenesis and effects of radiation on the immune response (22). The early effects of radiation in man were also expertly and comprehensively reviewed in a report by the United States National Research Council in 1967 (23).
