Book excerpt: These are the Proceedings of the 2nd International Metastasis Congress of the Metastasis Research Society which took place in the town hall (Stadthalle) of Heidelberg, FRG, in September, 1988. This first Metastasis Congress in the FRG was organized in conjunction with the German Association of Cancer Research (SEK) of the German Cancer Society. The congress topic generated tremendous interest and attracted about 400 scientists from 22 countries. Most participants came from Europe, Israel, and the United States. Why did we organize the Metastasis Congress? Only about 50% of all people who develop some form of cancer are curable. Despite improved patient care and increasingly innovative and effective techniques for diagnosing and treating primary cancers, the development of secondary cancer colonies, i. e. , metastasis, can not be prevented and is the major cause of death. In the Federal Republic of Germany there are still as many as 160 000 cancer patients per year who succumb to their disease, often after periods of terrible suffering, and this overall figure is not improving. Partly because of the complexity of the process, basic research on metastasis has lagged behind other disciplines such as carcino genesis and cancer genetics. Metastasis formation involves the ability of malig nant cells to invade adjacent tissue and to penetrate into lymphatic or blood circulatory systems, or both, and to spread to near or distant sites to form new tumor colonies. Meanwhile, research on metastasis is receiving much attention.

The book includes the topic of the emerging roles of endogenous TLR4 ligands whose functions are shared intriguingly by metastasis and auto-inflammatory diseases such as rheumatoid arthritis. For example, most cancer therapies established so far are effective initially. However, they eventually face the great obstacles of drug resistance, immune evasion, and metastatic progression. One of the endogenous TLR4 ligands is thought to contribute to all three processes. The most important features of the book are to explain a concept of homeostatic inflammation, disturbance of which in the lungs gives rise to the establishment of a pre-metastatic pulmonary microenvironment based on primary tumor-mediated hijacking of the innate immune defense system against respiratory infection. In addition, structure-based thinking is another important feature of this book. It is proposed that inflammation forms a functional triangle with angiogenesis and coagulation, in the center of which cancer is located. Given the proposal of precision medicine by President Obama in the United States in 2015 and the recent success of immune-modulator-based therapy, this book will appeal to researchers in a variety of fields with the title of the book connecting the worst disease (metastasis) and the most fundamental event (inflammation) that is common to many diseases.

Radiotherapy is one of the main treatment modalities to achieve locoregional tumor control. However, the treatment of distant metastases further remains to be a challenge. In this special topic we are interested to elucidate immunological aspects which are initiated and affected by radiotherapy. We also aim to describe the development of innovative immunological strategies from a preclinical stage to clinical application which could be combined with standard radiotherapeutic approaches. A special interest will also deal with the effects of radiotherapy on tumor initiating cells as well as on the tumor microenvironment. Last but not least the effects of different irradiation sources and qualities such as photones, protones and heavy ions will be analyzed with respect to immunological outcome.

While recent improvements in surgical techniques and a more aggressive approach to resection of liver metastases have improved long term survival for some patients, most patients with hepatic metastases still succumb to their disease. To improve these dismal statistics, a better understanding of the biology of liver metastasis, particularly the early stages that can be targeted for prevention, is essential. Once cancer cells enter the liver, several different scenarios may occur. The cancer cells may be immediately destroyed by local defence mechanisms, they may enter a state of dormancy as solitary cells and never produce a metastasis, initiate a short-lived process of proliferation that is aborted before a metastasis is established or actively proliferate to form macrometastases. The chapters in Part I of this book provide insight into the cellular/molecular mechanisms that determine which of these scenarios prevails. Written by experts researchers in the filed of metastasis, these chapters provide state-of-the art reviews on the cellular and molecular processes that impact the early stages of the metastatic process. The unique microenvironment of the liver, its various anatomical, cellular and molecular features and the impact they have on metastasis are highlighted.
